The Study on Way of Nerve Invasion of Pancreatic Carcinoma and Its Clinical Significance

Abstract:Objective To study the characteristics of nerve distribution inside and outside the pancreas and the way of nerve invasion for pancreatic carcinoma. Methods Gross anatomical examination, S-100 immunohistochemistry and electron microscopy were used to observe the nerve distribution and changes in 8 normal individuals and 35 patients with pancreatic carcinoma. Results The innervation of both head and body of the pancreas mainly consisted of nerve fibers from the right celiac neuroganglion and the right half of the superior mesenteric arterial plexus. The pancreas was full of nerve fibers ending at acinar lobules. The positive rates for invasion and metastasis were 73.3% and 60.0%, respectively. Pancreatic carcinoma had a tendency of invading and transferring along or around nerves through the vessels of the perineurium/the perineurium and synaptic membrane of nerve endings. Conclusions It is necessary to clean the right abdominal plexus in duodenopancreatectomy.
