Myocardial Preconditioning Induced by Volatile Anesthetics in Rabbits Via Activa Tion of KATP Channels

Abstract:Objective:To observe the potential role of K ATP channels in volatile anestheticsinduced myocardial preconditioning.Methods:Sixtyfour rabbits were randomly allocated to eight groups:ischemiareperfusion group (group IR),isoflurane preconditioning group (group IP),sevoflurane preconditioning group (group SP),desflurane preconditioning group (group DP),isoflurane preconditioning plus glyburide group (group IP+G),sevoflurane preconditioning plus glyburide group (group SP+G),desflurane preconditioning plus glyburide group (group DP+G),and glyburide group (group G).Animals were subjected to three hours of left anterion descending coronary artery occlusion followed by three hours of reperfusion.Groups differed only in the pretreatments given,and group IR received no pretreatment.Three volatile anesthetics preconditioning groups were pretreated with 30 minutes endtidal 1 1% isoflurane,2% sevoflurane or 6% desflurane,and then 15 minutes of washout.Three volatile anestheticsplusglyburide groups were administered 0 33 mg/kg glyburide intravenously before pretreatment.Group G was administered identical doses of glyburide,but it was not pretreated.Infarct size and area at risk were defined by staining.Results:Infract size,expressed as a percentage of the area at risk (IS∶AR) was 0 61±0 11 in group IR.Isoflurance,sevoflurane,and desflurane preexposure significantly reduced myocardial infarct size to 0 39±0 06,0 31±0 07,and 0 32±0 08 (all P 0 05),respectively.Glyburide pretreatment eliminated the preconditioning effect of volatile anesthetics (IS∶AR=0 64±0 12,0 57±0 12,0 62±0 11,respectively all P 0 05).Glyburide alone did not increase infarct size (IS∶AR=0 63±0 13, P 0 05).Conclusion:The volatile anesthetics including isoflurane,sevoflurane,and desflurane could protect the rabbit myocardium subjected to coronary occlusion from infarction via activation of K ATP channels in vivo.
