Effect of Gut-Derived Endotoxin Change of Cytokines in Liver Diseases

Abstract:Objective To research for the relationship between endotoxin and cytokine levels and so as to clarify the possible cause of change of cytokines in liver diseases. Methods Endotoxin level was determined by limulus amebocyte lysate test in serum of 89 cases of patients with acute and chronic liver diseases. All patients were divided into two groups based on oral administration of lactulose or not. The mean concentration of endotoxin and cytokines were compared before and after 20 days lactulose therapy.Results Serum endotoxin level in patients with cirrhosis (69.3±23.6pg/ml) was higher than those with acute (44.6±14.3pg/me) and chronic hepatitis (28.4±7.9pg/ml). Serum endotoxin levels were positively correlated with those of TNF α ( r =0.555, P 0.05), IL 6( r =0.531, P 0.01),IL 8( r =0.440, P 0.05) and G CSF( r =0.440, P 0.05), but not with IL 2( r =0.101, P 0.05). The serum levels of endotoxin and cytokines were parallelly reduced after 20 days lactulose therpay.Conclusion Serum levels of endotoxin were positively correlated with those of cytokines. Endotoxin may play a major role in leading to changes of cytoknes in liver diseases. Oral lactulose is beneficial to patients with liver diseases by reducing both endotoxin and cytokine levels.
