Intrapulmonary concentration of ciprofloxacin in patients of lower respiratory tract infection with ventilation support

Abstract:Objective To investigate the pharmacokinetics and lung penetration of ciprofloxacin in patients of severe lower respiratory tract infection with ventilation support. Methods A single 0. 2 or 0. 4 g dose of ciprofloxacin was given to 12 patients of lower respiratory tract infection with ventilation support by infusion administration route. The drug was administrated routinely. Plasma samples from these patients were scheduled to be drawn after 3 daysu0027 intravenous infusion. Bronchoalveolar lavage was obtained at 3 h after intravenous infusion. Blood and bronchoalveolar lavage samples were proceeded by protein precipitation and the concentrations of ciprofloxacin were determined by HPLC method.Results A total of 84 serum concentrations and 12 bronchoalveolar lavage concentrations of ciprofloxacin were determined from 12 patients.For the dose of ciprofloxacin 0. 2 g,Cmaxin plasma and alveolar epithelial lining fluid at 3 h were( 4233. 4 ± 1165. 9) and( 1202. 5 ± 291. 5)ng·m L- 1,and the penetration ratios were( 0. 72 ± 0. 54). For ciprofloxacin 0. 4 g,Cmaxin plasma and epithelial linging fluid at 3 h were( 5342. 7 ± 1210. 2) and( 2219. 4 ± 612. 7) ng·m L- 1,respectively,and the penetration ratios were( 0. 90 ± 0. 67). The differences in the penetration ratios of the two different doses were significant( P 0. 05)at 3 h. Conclusion It showed that ciprofloxacin demonstrates good penetration into ELF in patients of lower respiratory tract infection with ventilation support,and a higher dose can increase the penetration ratio.
