Exploration for the Optimal B Value in Diffusion-Weighted MRI Scanning for the Differentiation Between Benign and Malignant Vertebral Fractures

Abstract:Objective To explore the optimal b value in diffusion-weighted imaging (DWI) for the differentiation between benign and malignant vertebral fractures.Methods Twenty-two consecutive patients with vertebral compression fractures, including 10 benign fractures due to osteoporosis and/or trauma and 12 malignant fractures due to metastases, were collected for the prospective study. Sagittal DWI with different b values was performed in all patients on a 1.5-T MR unit. The image quality with different b values was analyzed and compared, qualitatively by three scales system and quantitatively by signal-to-noise ratio (SNR). Apparent diffusion coefficient (ADC) between benign and malignant vertebral fractures was also measured. Results The smaller the b value was, the better the DWI image quality would be. Mean SNR of images obtained with b value 0, 100, 200, 300, 400, 500, 600, 700, and 800 s/mm 2 were 22.7±3.4, 24.6±3.6, 22.2±3.2, 19.2±2.6, 10.3±2.1, 8.8±2.7, 6.8±2.7, 5.8±1.7, and 5.8±2.1, respectively. The difference among them was statistically significant (F=150.13, P0.01). The scales obtained with b value below 300 s/mm 2 were significantly higher than that of images with b value above 300 s/mm 2 (P0.01). The mean ADC values of malignant fractures were significantly lower than that of benign ones on DWI with b values less than 400 s/mm 2 (P 0.05). Four cases with benign vertebral fractures were misdiagnosed as malignant, as the lesions displayed slight hyperintense signal when b value of 0 or 100 s/mm 2 was used. Conclusion A b value about 300 s/mm 2 is recommended when diffusion-weighted MRI scanning is used to differentiate benign from malignant vertebral compression fractures. If used b value were larger than that, the images with some noise would get worse, causing it difficulty to observe the fractured vertebrae. Too small b value will reduce diffusion weighting of water molecule, which may make false-positive hyperintense osteoporotic fractures.
