Infectious endocarditis complicated with congenital heart defects

Abstract:Objective The infectious endocarditis with congenital heart defects demonstrates varied syndromes and signs resulting from different clinical stages. It is kind of difficulty for physicians to make the early diagnosis. The aim of this report was to discuss affecting factors of early diagnosis and effective treatment for this type of disease.Methods Fifty two cases, 6 months to 12 years old, diagnosed as infectious endocarditis complicated with congenital heart defects during the last 10 years were collected to make further analysis. Of those thirty had no surgery. Others had either surgery or cardiac catheter diagnosis.Results Fever, aggressive heart failure, heart murmur changing, skin petechia and ecchymosis were the most clinical manifestations. Laboratory examinations found that the anemia happened in almost all the patients and half of the cases showed increment of ESR and CRP. Blood culture was positive in 28 cases (54%). Staphylococcus and streptococcus were detected in most cases (82%). Vegetation was found in 34 cases (65%). Refractory heart failure, neurological problem and peripheral vessel embolism happened in 26 cases (50%) due to the valve destruction. Only antibiotic treatment was given in 30 cases. Seven cases were operated after combined antibiotic treatment. Fourteen cases (27%) were failure in treatment and died.Conclusion Those patients with congenital heart defects (including postoperative cases) should be checked up carefully to find out the heart murmur changing and peripheral vessel embolism if the unknown fever lasted more than one week and no extra cardiac infectious focus could be found. The blood culture and 2DE should be done as early as possible to clear up the diagnosis.
