Abstract:Objective To observe the clinical efficacy and side-effects of ropivacaine with fentanyl or sufentanyl used for post cesarean section patient-controlled epidural analgesia(PCEA).Methods Ninety full-term nulliparous women who underwent cesarean section,ASA Ⅰ～Ⅱ,were included into this study.They all received post-operative PCEA after combined spinal-epidural anesthesia(CSEA) and were divided into three groups.Patients were randomly assigned to receive 0.15% ropivacaine(the ropivacaine group,R,n=30);0.125% ropivacaine+fentanyl 2 μg/ml(the ropivacaine-fentanyl group,RS,n=30);0.125% ropivacaine+sufentanyl 0.5 μg/ml(the ropivacaine-sufentanyl group,RF,n=30).The PCEA settings were as follows:background infusion 2 ml/h,demand bolus 3 ml and lockout interval 20 min.At 4 h,8 h,12 h,24 h and 48 h after operation,the following data were observed and collected:analgesia effects assessed by the visual analog scale(VAS);motor block assessed by Bromage Motor Score;degree of sedation scales assessed by Rmsay score;PCEA demands and drug consumption of PCEA;side effects such as nausea,vomiting,tickling sensation and respiratory depression.Results Eighty-four patients were enrolled in this clinical investigation finally,six of the 90 patients were excluded because of catheter displacement.The analgesic effect at rest in group RS was better than group R at 12 h after operation(P 0.05),during ambulation,analgesic effect in group RF was better than group R at 24 h after operation(P 0.05).The total drug consumptions and actual PCEA demands of group RS were lower than group RF at 0～4 h after operation(P 0.05),the total drug consumption of group RS was lower than group R at 12～24 h after operation(P 0.05).There was no significant differences in the Bromage Motor Score,the degree of sedation,the incidence of nausea,vomiting and tickling sensation among the three groups.None of the patients in the three groups had respiratory depression and urinary retention.All patients had exhausted within 48 h after operation.Conclusions All these three solutions used in post cesarean section PCEA could provide satisfactory analgesic effect.0.125% ropivacaine with fentanyl 2 μg/ml or sufentanyl 0.5 μg/ml have better analgesic effect than 0.15% ropivacaine,while without increasing of side-effects.Ropivacaine with fentanyl or sufentanyl is safe and effective in PCEA.
