Effects of Intraoperative Irrigation with Different Solutions on Prevention of Postoperative Infection after Joint Replacement

Abstract:Objective To evaluate the effect of intraoperative irrigation with different solutions on prevention of postoperative infection after joint replacement.Methods 54 rabbits were divided into 5 groups and a cobalt-chromium-molybdenum alloy prosthesis was implanted into the distal femur of the right knee in each rabbit and followed by the instillation of 100 μl staphylococcus aureus suspension of 1×108 cfu/ml into the joint cavity.In the control group,the wound was closed after the implantation and instillation.In the saline group,chlorhexidine group,povidone iodine group and gentamycine saline group,3 min after the instillation of the staphylococcus aureus suspension,the wound and joint cavity were irrigated respectively with 150 ml solution of saline,0.05% chlorhexidine,0.10% povidone iodine and gentamycine saline (0.48 g/L) respectively for 2 min.14 days after operation,the joints were assessed by observing the local appearance of infection,the character of the pus and by bacterial culture.Results Every operated joint was infected in the control group and in the groups irrigated respectively with saline solution and 0.1% povidone iodine solution.In the group that had irrigation with 0.05% chlorhexidine,the incidence of infection was 90% and in the group irrigated with gentamycine saline solution,the incidence was 9.1%,which was significantly lower than the control and the other groups (P0.01).Conclusion Intraoperative irrigation with gentamycine saline solution can significantly reduce the incidence of infection in the situation of wound contamination by staphylococcus aureus in the wound containing metal prosthesis,and thus can effectively prevent postoperative infection after joint replacement.
