The Study of Anticoagulant and Fibrinolytic Function in Patients with Deep Venous Thrombosis

Abstract:Objective:To study the value of anticoagulant and fibrinolytic factors in patients with deep venous thrombosis(DVT).Method:72 DVT patients diagnosed by ultrasonography and/or venougraphy and 70 normal controls were studied. All the patients and controls were evaluated respectively by examining their blood antithrombin-Ⅲ (AT-Ⅲ) activity, protein C(PC) activity, total protein S (PS) quantities, plasminogen(Plg), plasminogen activator inhibitor-1(PAI-1) level, urokinase-type plasminogen activator(u-PA) and urokinase-type plasminogen activator receptor(u-PAR) level.Result:The anticoagulation protein (AT-Ⅲ, PC and PS ) activities of DVT patients were significantly lower than that of controls(P 0.01). The incidence of anticoagulant protein(AT-Ⅲ, PC and PS) deficiency in DVT patients was significantly higher than that of controls(P 0.01). The incidence of anticoagulant protein deficiency in recurrent patients was significantly higher than that of newly diagnosed patients(P 0.01). The plasma plasminogen activities of patients were significantly decreased compared with controls(P 0.01), and the PAI-1, D-dimer, uPA and uPAR levels of DVT patients were significantly increased compared with that of controls(P 0.01). D-dimer levels in 97.2% patient higher than 500 ng/L, but only 1.4% were controls.Conclusion:The levels of anticoagulant protein(AT-Ⅲ, PC and PS) in DVT patients were significantly lower than that of controls suggested that the patients' anticoagulant fuction was decreased. The higher incidence of anticoagulant protein deficiency in patients and the higher incidence in recurrent patients suggested that anticoagulant protein deficiency was an important factor of occurring and recurrence in DVT patients. The patients' lower Plg level and higher PAI-1 level showed that the patients' fibrinolytic fuction was decreased. The higher levels of uPA and uPAR showed that the fibrinolytic activity of uPA/uPAR system played an important role in the recovery of the vessel wall and angiogenesis after DVT. Testing of D-dimer quantities has a negative exclusion value for diagnosis.
