Abstract:The structural reliability or safety assessment analysis of a system is primarily dependent on more detailed understanding of the failure more. Abstract:Quality evaluation plays a very important part in Design For Quality (DFQ). Based on the concept of two types of quality and the model of DFQ by M.Morup, the author discusses four basic elements of quality evaluation in DFQ such as objects, rules or knowledge, valuator and scheduling (for the progress of desgin). This paper presents an elementary model of the quality evaluation process. This new model is focus the elementary strategy on activity planning, task representation and information organization which relate the quality evaluation process in order to make them simplification and more effective. It proposes a framework of computer aided quality evaluation system (CAQES) by using object-oriented techiques.

Abstract:Failure mode and effect analysis (FMEA) adapt to analyses the potential failure modes, causes and relative effects of product or process, then take measures to prevent scrap (failure) from happening in advance. Based on Total system, which consists of quality management and quality monitoring, research of FMEA applicated in advanced enterprise environment has been carried out. Computer aided FMEA (CAFMEA), which characterized by preventive design and process monitoring, is proposed and established, and then the better developing direction of FMEA in total management is discussed.

Abstract:There are a lot of risks in the software developing process. To control the risk, the sorts of risk are defined from the software development and project management. The paper presents four main parts of risk identification and risk analysis. The risks during software development process and possible solutions are analyzed while the risk quantity method, parameter model and experience data are presented. According to these risk identification methods and every phase situation of software development, the strategy of risk control is proposed. About eighty percent risk can be identified and controlled in the statistic application.

Abstract:In this Paper, we introduce a novel FMEA (Failure Mode and effect Analysis) system, It can achieve FMEA more practicable and valuable compared with current FMEA application status as record archives. The novel FMEA basic unit is module and related modules are combined to form a whole FMEA, Six new link/experience function modules are introduced into the standard FMEA format to integrate the database, and the module unit exists independently so that different FMEA file cross share the similar failure modules. Three new link function modules can connect FMEA system with other related production systems to embed FMEA useful resource into production as guidance, this link function can prevent potential and old failure modes timely occurring timely, further reduce defect and improve production efficiency and quality. This novel FMEA tool can make FMEA more value and important functions in semiconductor process.

Abstract:Computer finite element simulation is a type of important scientific research methods which saves much time and money. First, the basic theory of finite element was presented. Then, the magnetic flux leakage (MFL) of sticks without flaws and different dimension flaws were analyzed using finite element method. It is proved that flaw MFL can be analyzed using finite element method, and the relation of the flaw dimension and MFL is introduced , and is good base provided for the quantitative inspection of flaws.

Abstract:In practice, engineers find that software quality depends heavily on the maturity and reliability of the software process. Therefore, organizations are placing increased attentions on finding an efficient way of integrating elements of software process in order to produce high quality software with lower cost and risk. Various kinds of techniques are used to manage, monitor and analyze software process. We proposed an idea of modeling software process in Little-JIL language, and then introduce automatic Fault Tree Analysis (FTA) technique and Failure Mode and Effect Analysis (FMEA) technique, two fully-fledged and widely used safety analysis techniques, to analyze software process. Results of these two techniques can be combined to improve software process, which may lead to software products with higher quality and reliability, also lower cost and risk.

Abstract:Failure mode and effects analysis (FMEA) helps to identify the weak points in the processing, manufacturing, and assembly of products and plays an important role in improving product reliability. To address the shortcomings of the existing FMEA methods in terms of the uncertainty treatment of information and not considering the weights and correlations between risk factors, we propose a new FMEA method. In this paper, the Fermatean fuzzy Z-number (FFZN) is proposed by fusing the Fermatean fuzzy number and Z-number. Extending it to the Bonferroni mean (BM) operator, the Fermatean fuzzy Z-number-weighted Bonferroni mean (FFZWBM) operator is proposed. A new FMEA method is proposed based on this operator. In order to overcome the factors not considered in the FMEA method, two new risk factors are proposed and added. The ability of experts to express fuzzy information is enhanced by introducing the FFS. The weights and correlations between the influencing factors can be handled by aggregating the evaluation information using the FFZWBM operator. Finally, the proposed method is applied to an arithmetic example and the accuracy of the proposed method is proved by teaming it with other methods.

Abstract:Reliability technology plays a significant role in ensuring the safe operation of the subsea control system. To perform a comprehensive analysis of the reliability of complex systems, a combination of Failure Mode and Effects Analysis (FMEA) and Fuzzy Fault Tree Approach (FFTA) is introduced. Firstly, the FMEA method is used to analyze the potential failure modes and causes of system failure by completing the qualitative analysis of system reliability from the perspective of multi-factor failure modes. And the risk matrix diagram is applied to determine the degree of harm of different failure modes to the system. Then, the system reliability is quantitatively analyzed using FFTA, and a fault tree model is established by dividing the system into "system-subsystem-component" and solving for the minimum cut set. In addition, the failure probability of the top-level event is quantitatively calculated by introducing fuzzy set theory, and the probabilistic importance of the bottom-level event is analyzed to find out the weak points of each subsystem. Finally, a qualitative and quantitative reliability analysis is conducted by using FMEA-FFTA method for subsea control system. Effective measures should be taken to focus on preventive protection and regular testing for the high risk, medium–high risk and medium risk modes for subsea control system.

Improving Product Quality Based on QFD and FMEA Theory

Abstract:This research is based on Quality Function Deployment (QFD) and Failure Mode and Effects Analysis (FMEA) to improve the quality of electro-fusion fittings. QFD is an effective tool to explore customer demand. In this study, QFD is used to identify the key technical characteristics that affect customer satisfaction of electro-fusion fittings, and then AHP is used to rank the key technical characteristics of customer satisfaction. FMEA is a systematic activity in the product design stage and process design stage to analyze the subsystems, parts and processes of the product one by one, and then find out all potential failure modes and analyze their possible consequences, so as to take necessary measures in advance to improve product quality and reliability. In this study, FMEA is used to analyze the causes and consequences of possible failure modes in the process of improving the quality of electro-fusion fittings. CRs were then transferred to specific technical attributes (TAs) by House of Quality (HoQ). Manufacturing processes of electro-fusion fittings were analyzed by FMEA and relative solutions were given to failure models with high Risk Priority Number (RPN) indices. By the integration of FMEA into QFD, the product qualification ratio of electro-fusion fittings has increased by 12% based on the improvement of the technical design and manufacturing processes.

Abstract:As a proactive reliability analysis technique, the purpose of failure mode and effect analysis (FMEA) is the detection of all failures and their effects within a system, and the determination of the causes of these failures. However, the traditional FMEA method shows some important drawbacks regarding failure mode evaluations, risk factor weights and risk priority ranking, etc. This paper aims to develop a novel FMEA to improve the performance of FMEA by using spherical fuzzy sets (SFSs) and spherical fuzzy weight correlation coefficient (SF-WCC). The method mainly includes three stages. The SFS is applied to capture risk evaluation information because it comprehensively considers the information expression in different scenarios in the first stage. A spherical fuzzy projection model is introduced to calculate the expert weights objectively for reducing the information difference caused by experience, psychology and other cognitive factors of experts in the second stage. Meanwhile, the spherical fuzzy entropy is defined to determine the incomplete weights of risk factors by constructing an optimization model in the second stage. The SF-WCC is proposed to prioritize the failure modes to avoid the prioritization controversy of failure modes by using different distance formulas in the third stage. In the experiment, a practical example is presented to illustrate the applicability and effectiveness of the proposed method, and results show that the proposed approach offers a reliable tool for practical FMEA problems. The conclusion is that the proposed method is applicable to multiple actual scenarios as a reliable risk assessment technology.

Abstract:Based on the failure data of the CNC system in two years, the failure positions, failure modes, failure causes, failure sources of the CNC system hardware and software were analyzed. The more failure positions were analyzed further by FMECA method, proposing content, key point and method which should be paid attention to while using and maintaining the CNC system. The weak parts and its correlative components were also pointed out, so as to provide the basis for improving the reliability of the CNC system hardware and software.

Abstract:Nowadays enterprises are encountering diversified customer requirements, and using market information well is critical to product development. It is extremely important to conduct risk planning, prevent potential failures, and reduce uncertain factors by transforming customer requirements into exact development targets. This study is based on the method of quality function deployment (QFD) and failure modes and effects analysis (FMEA). The package design for semi-finished products of thin film transistor-liquid crystal display (TFT-LCD) modules in the photoelectricity industry was selected as a case study. First, customers conducted a questionnaire survey which helped determine the items they demanded. Second, QFD has been combined with FMEA to serve as the standard of evaluating the importance of customer requirements. Through mutual assessments and investigations by QFD and design failure mode and effects analysis (DFMEA), the key factors of a product design can be determined. The results of design of experiments verified that this method indeed effectively reduced defective products and customer complaints. The average number of customer complaints per year has been reduced from 9.5 to just only one after the introduction of the proposed method, which has been verified to assist in rapidly developing new products during the early design stage. This approach also reduces possible losses and enhances competitiveness.

Abstract:<p>This paper proposes a two-stage Failure Mode and Effect Analysis (FMEA) technique as a basis for implementing the failure analysis of offshore wind turbines. At the first stage, critical failure causes and failure modes of each component of offshore wind turbines are identified. In the next stage, critical components and systems of offshore wind turbines are ascertained by a cost-and-risk-based index that considers both risk priority and failure costs of components. The objective is to overcome some weaknesses of the traditional FMEAs including: (i) Risk-based FMEA ignores practical information extracted in the operation stage of offshore wind turbines such as failure cost and, (ii) Cost-based FMEA addresses mainly failures of components and systems and cannot deepen to failure modes and failure causes of offshore wind turbines. A methodology towards conducting uncertainty analysis of FMEA results is developed to provide a new insight into a good understanding of FMEAs and their results. The developed uncertainty analysis methodology reveals that the proposed two-stage FMEA technique is adequate to reduce the uncertainty of FMEA results and is superior in failure analysis of offshore wind turbines. The application of the methodology can provide recommendations toward corrective actions and condition-based maintenance implementations.</p>

Abstract:As a preventive maintenance technique, failure mode and effect analysis (FMEA) is widely applied to determine and eradicate possible failures. In recent years, many FMEA methods have been developed to address the limitations of traditional FMEA methods in failure mode (FM) assessment, weight calculation, and FM prioritization in complex uncertain environments. However, due to the complexity of equipment, systems, services, etc., there is an inevitable influence on causal relationships between FMs, which is rarely considered by existing methods. Therefore, a novel FMEA method, which integrates the technique for order preference by similarity to the ideal solution (TOPSIS) of q-rung orthopair fuzzy set (q-ROFS) and q-rung orthopair fuzzy cognitive map (q-ROFCM), is proposed for reasoning the relationships between FMs. The design of the novel method, named q-ROFCM FMEA, involves three stages. The q-ROFS is applied to evaluate the risk factors for capturing more ambiguous information in the first stage. The q-ROFCM is proposed and the causal relationship between FMs is integrated to describe the impact of the relationships on the evaluation results through the dynamic behavior of FMs in the second stage. In the third stage, the TOPSIS method is applied to determine the priority ranking of FMs based on more reliable similarity measurement results of the projection measure. The proposed new method is applied to the FM risk assessment of power units in the railway turnout system. The competitiveness and effectiveness of the proposed method have been demonstrated by comparison with other classic methods and sensitivity analysis.
