Five years' evaluation of cracked teeth: analysis of factors and clinical treatment

Abstract:Objective This study highlighted the characteristics and distribution of cracked tooth syndrome( CTS)and the associated factors of cracked teeth,to analyze the outcome of different treatment protocols,and to assess a conservative management protocol. Methods Two hundreds and twelve patients aged 21 years to 70 years with CTS-like conditions were included and studied. The location of the cracked teeth,age and sex of the patients,restoration materials,a diagnosis of pulp and apex,and the periodontal probing depth were analyzed. Cracked teeth were treated by different treatment protocols depending on the pulpal and periapical diagnoses. 78 teeth without pulpitis and periapical periodontitis were treated for 3M Z350 resin restoration( A group) or crowns( B group) only,and 211 teeth with irreversible pulpitis and periapical periodontitis treated with root canal therapy and crowns( C group). Results CTS was seen most often in the 31 to 50 years age band,in the first maxillary molar( 55. 7%),and there was a significant difference between CTS and hard food and restorations( P 0. 001). According to pulp status,different groups of teeth with or without pulpitis and periapical periodontitis were treated for resin restoration( A group) or crowns( B group) only,or root canal therapy and crowns( C group),which had achievement ratio of 7. 7%,92. 3%,and 89. 1% respectively. And the difference between group A and B was statistically significant. Conclusion Cracked teeth without pulpal and periapical should be restored with crowns as soon as possible. And root canal treatment and crowns will be necessary for teeth with irreversible pulpitis and periapical periodontitis. Crown restoration is a key strategy to cracked teeth.
