Surgical Treatment of Ebstein Syndrome in Patients with Wolff-Parkinson-White Syndrome

Abstract:Objective To report the experience of surgical treatment of Ebstein syndrome in patients with Wolff-Parkinson-White (W-P-W) syndrome. Methods Eleven patients were 5 males and 6 females with age ranged 14-43 years (mean 26.8±12.2 years). Tachycardia was present in 11 patients with W-P-W syndrome which included A type pathways in 4 patients and B type in 7. Electrophysiological studies showed there were 12 accessory pathways in 11 patients. Accessory conduction pathways were localized in the following manner: the right free wall area in 6, the left free wall area in 3, the posterior septal area in 1, and the right free wall and posterior septal area in 1. The pathways were successfully ablated in all patients during cardiopulmonary bypass. The Danielson method was used to repair the Ebstein syndrome in 9 patients and tricuspid replacement in 2 patients. Results There were no deaths and no relapse. Follow-up ranged from 4 months to 6 years (mean 3.4±1.3 years). There was mild tricuspid insufficiency in 2 patients. Nine patients were in NYHA classⅠand 2 in classⅡ. Conclusion The combined approach of tricuspid valve repair or replacement and surgical ablation of accessory atrioventricular connections was an effective treat modality for Ebstein syndrome and W-P-W syndrome.
