The Role of Microcoils in the Treatment of Various Acute Bleeding Diseases

Abstract:Objective To evaluate the usefulness of microcoils in the embolization treatment of acute bleeding-hemorrhagic disorder. Methods Twelve patients with acute bleeding/hemorrhagic diseases were embolized by microcoils. The diseases included duodenal bleeding (n=4), jejunal bleeding (n=2), hepatic bleeding (n=2), renal bleeding (n=2), cystic bleeding (n=1), and priapism (n=1). Superselective catheterization was carried out by using coaxial catheter system, and embolization of the target vessels was performed by using microcoils (1-5 coils, 2.1 in average). Spongel particles were used in some cases before deployment of microcoils. Results Complete hemostasis was achieved in all cases after embolization by microcoils. Embolization by spongel alone could not stop the bleeding completely in most of the cases. A young patient was operated after successful embolization for eliminating the cause of bleeding. No re-bleeding was observed in all of the cases during the 1 week-10 months (mean 4 months) follow-up duration. No definite complication related to the procedure was noted. A patient presenting with hematochezia for 3-day showed symptoms of cerebral embolization when he arrived at the angiographic session. This was considered to be due to coagulative therapy. Conclusion Embolization treatment of acute bleeding-hemorrhagic diseases by microcoils is effective, less-invasive, and safe. It could play an important role in the clinical practices.
