Abstract:Background: Epigenetic mechanisms such as 5-methylcytosine (5mC) DNA methylation, histone acetylation and long non-coding RNAs (lncRNAs) are associated with aging processes and cardiovascular disease. Modification of the novel N6-methyladenine (m6A) DNA implicates this epigenetic mark in human malignant disease, but its role in atherosclerosis (AS) is largely unknown.Methods: We used ELISA to determine leukocyte global 5mC and m6A levels in 207 AS patients and 142 sex- and age-matched controls. Gene expression was examined by qRT-qPCR, western blot analysis and immunohistochemistry in mice with AS induced by a western diet, and oxidized low-density lipoprotein (ox-LDL)-treated human umbilical vein endothelial cells (HUVECs) and human monocytic cells (THP-1). Chromatin immunoprecipitation (ChIP) assay and dual-luciferase reporter assay were used to measure the transcriptional regulation of the lncRNA myocardial infarction-associated transcript (MIAT). Linear regression analysis was used to assess predictors of progression of carotid plaque size and carotid intima media thickness (CIMT).Findings: Leukocyte level of m6A but not 5mC was decreased with increased carotid plaque size and CIMT in AS patients. Serum LDL and leukocyte m6A levels were associated with progression of carotid plaque size and CIMT. LDL level and CIMT were also independently related to m6A level. Reduced m6A level was further confirmed in leukocytes and endothelium in WD-induced AS mice and ox-LDL treated HUVECs and THP-1 cells. Decreased m6A level was related to upregulated ALKBH1, the demethylase of m6A. Silencing of ALKBH1 or hypoxia-inducible factor 1α (HIF1α) could rescue the ox-LDL-increased level of MIAT, a hypoxia-response gene. Mechanically, ox-LDL induced HIF1α for transfer into the nucleus. Nuclear HIF1α bound to the ALKBH1-demethylated MIAT promoter and transcriptionally upregulated its expression.Interpretation: Elevated ALKBH1 level in endothelium and leukocytes reduced m6A level, which is associated with plaque progression.
