Existence of a differentiation blockage at the stage of a megakaryocyte precursor in systemic lupus erythematosus associated with thrombocytopenia

Abstract:Objective To study the differentiation of megakaryocyte progenitor s in systemic lupus erythematosus (SLE) associated with thrombocytopenia, and th e serum effects of these patients on the proliferation and differentiation of megakaryocyte progenitors. Methods Seven patients with SLE-associated thrombocytopenia ,four SLE patients who had no thrombocytopenia and eleven normal controls were included in this study. Bone marrow mononuclear cells from all subjects were cultured in vitro in serum-fre e fibrin clot system in the presence of combination of thrombopoietin (TPO), int erleukin-3(IL-3) and interleukin-6(IL-6). Colony-forming units-megakaryocy t e (CFU-MK) were quantified by indirect immunophosphatase alkaline labeling tech nique using anti-GPⅡb/Ⅲa MoAb. The effects of serum from SLE associated with thrombocytopenia on the formation of CFU-MK were also studied in the same cultu re system for all subjects. Results A significantly lowering of megakaryocyte progenitors (CFU-MK) was observed in patients with SLE associated with thrombocytopenia (27.33±9.30 CFU-MK per 2.2 ×106 bone marrow mononuclear cells), compared to the normal control (61.22±2 9.7 1 CFU-MK) and the patient with no thrombocytopenia control (60.00±29.71 CFU-M K, P0.05). An inhibi tion of CFU-MK in both SLE-associated thrombocytopenia group and normal contro l group was found when the serum from SLE-associated thrombocytopenia was added to their culture system (27.33±9.30 vs 14.29±6.73 CFU-MK and 61.22± 29.71 vs 29.44±23.35 CFU-MK, respectively, P0.05). Conclusion The growth potential of megakaryocyte progenit ors is lowered in SLE-associated thrombocytopenia group, indicating existence o f a differentiation blockage at the stage of a megakaryocyte precursor, possibly due to the mechanism of autoantibodies against stem cell in serum.
