Transurethral electroscission fenestration for the treatment of ureterocele in children (report of 88 cases)

Abstract:Objective To summarize the experience and to evaluate the efficacy of electroscission fenestration through cystoscope for treating ureterocele in children. Methods From 1988 to 2003, transurethral electroscission fenestration was perfo med in 88 children (37 males and 51 females) with 134 ureteroceles. Their age ranged from 2 months to 1; years. Bilateral ureterocele occurred in 46 cases and unilateral, in 42 cases. The preoperative symptoms inc luded voiding difficulty (42 cases) ,urinary infection (13 cases) , abdominal pain (22 cases) ,hematuria (6 cases) , external urethral meatus cyst (9 cases) and vesi-coureteral reflux (VUR) (3 cases). Of the 88 cases,25 had concomitant bilateral renal and ureteric duplication ;20,unilateral renal and ureteric duplication; 18 ,hydronephrosis/partial hydronephrosis;4,stones in the cyst;l ,ectopic orifice of the ureter. Under direct vision by cystoscopy,a crosswise incision on the cyst base was made when the cyst was≤2 cm in diameter; while a lengthwise incision on the cyst top was made when the cyst was > 2 cm in diameter. For bilatera cysts, the incisions were made concurrently. Among these eases,2 underwent electroscission fenestration twice,the rest underwent a single session of the procedure. Results All the patients were followed up for 6 months to 15 years. Thirteen cases developed VUR;of them, 4 with VUR >Ⅳ°were cured by anti-reflux ureteral reimplantation after 1 year;in the other 9 cases, VUR disappeared within 4 years. In the 18 cases ac ;ompanied with hydronephrosis/partial hydronephrosis, hydrops disappeared in 3 years in 15 cases,and it cisappeared after 10 years in 3 cases. All the ureteroceles in 88 children disappeared 1 year after the procedue. Conclusions Transurethral electroscission fenestration of ureterocele through cystoscope is a simple,effective and minimal invasive procedure for children with ureterocele.
