Clinical analysis of glucocorticoids in completely cured pemphigus vulgaris

Abstract:Objective: To summarize the regimen for treating pemphigus vulgaris(PV) with systemic glucocorticoids. Methods:We retrospectively analyzed PV patients who were cured and followed up for at least 2 years in our department from January2001 to December 2010. Results: 128 cases were completely cured by systemic glucocorticoids(maintained by prednisone or the equivalent ≤10 mg/d, or no medication; lesion free for ≥6 months). The maintenance dose of daily glucocorticoid for mild, moderate, severe and very severe patients were(38.9±18.1)mg,(51.3±16.2)mg,(68.8±15.6)mg and(88.3±16.0)mg,respectively(P0.001). When the maintenance dose of daily glucocorticoid were ≤40 mg, 45~60 mg, 65~80 mg and 80 mg,the respective treatment time were(85.0±85.9)d,(47.7±36.0)d,(36.3±16.0)d and(27.0±18.1)d(P0.001) before the reduction of dose. All patients reached clinical cure within 6 years. 80.2%(101/126) of patients were cured within 3 years, and98.4%(124/126) within 5 years. The dose was annually reduced by 50% of the prior year. Conclusions: PV can be cured within 3~6 years, and effectively controlled by annual reduction of 50% of glucocorticoids. Too much and too fast reduction of glucocorticoids are the major causes of PV relapse.
