A family with CADASIL caused by NOTCH3 gene splicing mutation

Abstract:Objective To report the clinical,MRI,and molecular genetic features of a Chinese family with cerebral autosomal dominant arteriopathy with subcortical infarcts and leukoencephalopathy(CADASIL).Methods The clinical and radiological features of a CADASIL family were summarized.The sequence of NOTCH3gene was analyzed in the DNA from the proband and his family members.Reverse transcription PCR was performed to observe the effect of the mutation on mRNA splicing.Results The symptomatic members of the CADASIL family mainly manifested as cerebral infarction and dementia without migraine or psychiatric disorders.Brain MRI of the proband revealed multifocal lacunar infarctions and diffuse white matter lesions.A heterozygous mutation of c.341-2 AG in NOTCH3gene has been identified in the proband and other symptomatic family members,while this mutation was not detected in his normal sister or 100normal controls.Reverse PCR identified that this splicing mutation resulted in the initial 7amino acid of exon 4skipping,producing a truncated protein.Conclusions The Chinese family with CADASIL were caused by a splicing mutation,c.341-2 AG of NOTCH3gene,which resulted in abnormal gene splicing that changed the structure and function of its encoding protein.Compared with the European CADASIL patients carrying the same mutation,the patients in this reported family lacked migraine.
