Assessment of possible malignancy of the thyroid nodules based on clinical manifestations and pre-operative examinations.

Abstract:Objective To assess the incidence of malignancy of thyroid nodules on the basis of clinical manifestation and other parameters of pre operative examination. Methods We had retrospectively analyzed 1369 cases of benign thyroid nodule and 97 cases of malignant nodule, in correlation to the clinical manifestations and the pre operative examinations of thyroid scintigraphy and ultrasonographic scanning.Results The incidence of malignancy of the thyroid nodules had no statistical correlation with the gender and age of the patients ( P = 0.3701), nor with the number of nodules ( P = 0.2138). However, the malignancy was found to be significantly related to the size of the nodules, the incidence was increased in correlation to the size ( P = 0.01356); the possibility of malignancy was also related to the tenderness of the nodule, with higher incidence among the hard ones. Pre operative examinations demonstrated that neither cold nodules found by scintigraphy nor the results of thyroid ultrasonographic scanning were useful in the diagnosis of thyroid cancer.Conclusion The above findings emphasize the importance of careful reviewing the clinical information of each patient, in addition to the cytology examination by fine needle aspiration (FNA). Surgery is highly suggested in younger patients with nodules over 1 centimeter in diameter, regardless the numbers of the nodules. For patients with nodule size below 1 cm in diameter, follow up with ultrasonic scanning is highly suggested, FNA guided by ultrasonic scanning should be performed in order to exclude the possibility of early stage of malignancy.
