Abstract:Background: Esophageal visceral hypersensitivity plays an important role in the pathogenesis of non-erosive reflux disease (NERD), but the mechanisms responsible for esophageal hypersensitivity in NERD are poorly understood. Aims: To evaluate the correlation between the visceral perception threshold and the expression of calcitonin gene-related peptide (CGRP) in NERD patients by detecting the hypersensitivity response to esophageal mucosal acid exposure and distention, and observing the expression and characterization of CGRP in lower esophageal sphincter (LES) mucosa in patients with NERD. Methods: Esophageal sensitivity to mechanical stimuli was measured by Synectics visceral stimulator and esophageal barostat. The Bernstein test was used to evaluate esophageal sensitivity to intraesophageal acid perfusion. The expression of CGRP in mucosal layer of LES was detected by immonohistochemical staining techniques, and relative quantitative processing was performed by using micro-image analysis system. Results: According to the response to mechanical distension and acid exposure, thirty-one NERD patients were divided into esophageal hypersensitivity group (21) and non-esophageal hypersensitivity group (10). In comparison with control group and non-esophageal hypersensitivity group, the initial perception threshold and maximally tolerated pain threshold of patients with esophageal hypersensitivity were significantly reduced (P<0.05). The CGRP positive nerve fiber in esophageal mucosa and the A values (the average value of integral light density) of CGRP immunoreactivity of patients with esophageal hypersensitivity was significantly higher compared with that of NERD patients without esophageal hypersensitivity and that of healthy volunteers (P<0.05). The level of CGRP was negatively correlated with initial perception threshold and maximally tolerated pain threshold in NERD patients with esophageal hypersensitivity (r=-0.68 and -0.79, P<0.03). Conclusions: A majority of NERD patients showed esophageal hypersensitivity to mucosal acid exposure and/or mechanical distention. The expression of CGRP in esophageal mucosa was significantly higher in patients with esophageal hypersensitivity, and the level of CGRP was negatively correlated with esophageal perception thresholds, suggesting a relationship between esophageal visceral hypersensitivity and the expression of CGRP in NERD patients.
