Effect of Clonidine on Heart Rate Variability During Laparoscopic Cholecystectomy

Abstract:OBJECTIVE: To study the effect of α _2-adrenergic agonist clonidine on cardiac autonomic nervous as evaluated by heart rate variability during laparoscopic cholecystectomy. METHODS: In this randomized, double-blind controlled clinical study, sixty healthy patients undergoing elective laparoscopic cholecystectomy orally took either placebo or clonidine 150 μ g 90 minites before surgery. All the patients were anesthetized with balanced general anesthesia. Pneumoperitoneum was introduced by inflation of CO_2. Intra-abdominal pressure was maintained at 1 l~13mmHg. Measurements of heart rate variability were taken before medication, before the induction of anesthesia and 10 minutes after intubation . Measurements were repeated at 10 and 20 minutes during laparoscopy. RESULTS: Decreases in low frequence (LF), high frequence (HF) and total frequence power (TP) were noted during the maintenance of anesthesia in both groups. The power of LF, normalized unit of LF (nuLF) and LF/HF ratio increased during inflation period in both groups. In clonidine group, the increases of LF, LF/HF and nuLF were much less than that in placebo group. The differences were statistically significant between the two groups. CONCLUSION: Oral clonidine preanesthetic medication significantly reduces the increase of sympathetic cardiac activity due to pneumoperitoneum during laparoscopic cholecystectomy.
