Cohort Study of Hospital Acquired Lower Respiratory Tract Infections in Shanghai

Abstract:Objective: To understand the morbidity, risk factors and cost of hospital acquired lower respiratory tract infections (HALRTI) in Shanghai. Method:Retrospective cohort study. Eighteen hospitals were selected from 55 general hospitals with more than 400 beds in Shanghai. All charts of patients discharged in October of 1999 were reviewed by 38 trained nosocomial infection control workers. Results:402 HALRTI cases were indentified in 19?235 discharges. The average incidence was 2.1% or 1.0 case per 1?000 patient-days. Department of neurology ranked highest (5.4%), followed by department of hematology (4.8%), while it was less than 1% in department of gynecology and obstetrics, department of paediatrics, ophthalmology and otolaryngology. Logistic regression analysis suggested that artificial ventilation≥3?d, elderly, neure surgery are significant risk factors, their ORs being 16.7 (95%CI=8.1-34.4, P=0.000), 4.1-6.0 (P=0.000), 2.26 (95%CI=1.2-5.7, P=0.016), respectively. Patients complicated with HALRTI after neurosurgery were two fold that of control group. Average increased medical cost was 18?386.1 RMB per HALRTI patient (95%CI=15?665.0-21?107.3, P=0.000). Mortality of HALRTI was 25.8%, much higher than that of control group ( 2.3%). Conclusions:HALRTI was of high morbidity, mortality and cost in Shanghai. It is imperative to make every effort to lower down the morlidity of HALRTI.
