Abstract:Purpose: To investigate the current state of medication adherence among patients with inflammatory bowel disease (IBD) in China and analyze the influencing factors. Patients and Methods: We recruited as many eligible subjects as possible and a total of 105 patients with IBD finished this cross-sectional survey. The General Information Questionnaire and Morisky Medication Adherence Scale were used as measurement instruments. The factors influencing the Morisky score were studied by multiple linear regression analysis. Results: The average Morisky score was 6.11 +/- 1.93. Univariate analysis showed that there were statistically significant differences in Morisky scores among patients with different ages, education levels, disease types, medication administration methods, medication frequencies, disease understanding and knowledge learning frequencies (P < 0.05). Multiple stepwise linear regression analyses showed that medication frequency, medication administration method and disease understanding were important factors influencing the Morisky score (P < 0.001). Conclusion: Our data confirmed that the medication adherence of IBD patients was poor, especially among patients taking oral medication, those taking medications with a high frequency and those with a low understanding of the disease. To reduce symptom recurrence and control the disease, medical staff should pay attention to the individual patient factors, simplify the medication regimens, formulate effective coping strategies for drug management support.

Abstract:Background: Poor medication adherence in inflammatory bowel disease (IBD) had a negative impact on disease outcomes. In this study, we aimed to determine predictors of low adherence in the Chinese IBD populations and also aimed to compare a self-reported scale to a pharmacy refill index in assessing adherence of 5-ASA and azathioprine taken by Chinese IBD patients. Patients and Methods: Adult patients with IBD who had been taking 5-ASA or azathioprine for at least 3 months were recruited from hospital outpatient clinics. The MPR was calculated from previous six-month pharmacy refill data and the self-reported Morisky Medication Adherence Scale (MMAS-8) was issued through QR code questionnaires. Intentional and unintentional adherence scores were calculated according to specific items. Non-adherence was defined as MMAS-8 scores <6 or MPR < 0.8. Results: The response rate in the IBD patients was as high as 97%. 5-ASA non-adherence rate assessed by MPR was 30% and 37% by MMAS-8, and azathioprine non-adherence rate assessed was 33% by both MPR and MMAS-8. In a linear regression analysis, MPR value was significantly correlated with MMAS-8 score in 5-ASA group (r=0.4, p=0.003), and significantly correlated with unintentional adherence score (r=0.47, p<0.001). No significant correlation was observed between MPR value and MMAS-8 score in azathioprine group. Multivariate analysis demonstrated that age (OR: 1.08; 95% CI: 1.02-1.13; P=0.0015) and previous abdominal surgery (OR: 3.18; 95% CI: 2.09-4.27; P=0.04) were associated with high medication adherence. While patients who had small intestine lesion (OR: 0.09; 95% CI: 0.01-0.17; P=0.006) were associated with low adherence. Conclusion: Predictors of low adherence were young age, lesions on small intestine, whereas previous abdominal surgery was a protective factor. This study also demonstrated that the MMAS-8 scale was a valid instrument for assessing 5-ASA adherence in IBD patients. Unintentional non-adherence was significantly related to the total non-adherence, which would allow to use the tool to seek ways for adherence improvement.

Abstract:Many studies have demonstrated that non-adherence to oral anticancer drugs (OACDs) has challenged treatment efficacy. Otherwise, few validated tools exist to measure patients' adherence to medication regimen in clinical practice. To synthesize previous studies on adherence by cancer patients taking OACDs, especially in targeted therapy, a systematic search of several electronic databases was conducted. We analyzed existing scales' contents for various cancer patients and outcomes of studies assessing adherence. However, a well-validated scale designed particularly for OACD adherence is still lacking. Most adherence scales used in the studies reviewed contain items focused on measuring patients' medication-taking behavior more than their barriers to medication compliance and beliefs. However, non-adherence to OACDs is a complex phenomenon, and drug-taking barriers and patient beliefs significantly affect patients' non-adherence. To understand the key drivers and predisposing factors for non-adherence, we need to develop a well-validated, multidimensional scale.

Abstract:Abstract Purpose The aim of this study was to explore the beliefs, perceptions and representations of patients in order to identify the determinants of oral anticancer drugs adherence and to take action in current practice to improve patient support in digestive oncology. Methods We constructed a semi-directed interview guide which aimed to explore the patient’s relationship with medication, their health history, their experiences at the time of the announcement of treatment, their confidence, their fears, their motivations to adhere to their treatment and the constraints linked to their treatment. The data were analysed and discussed using a thematic approach. Results Seventeen patients agreed to participate in the study. The median age was 60 years. Ten patients had colorectal cancer, 3 patients had hepatocellular carcinoma, 3 patients had gastrointestinal stromal tumour and 1 patient had neuroendocrine pancreatic tumour. We identified five categories of factors influencing adherence: demographic and socioeconomic, disease-related, treatment-related, care system-related, and patient representation and pathways’ factors. A majority of patients emphasised the importance of family support in the adherence process and the convenience of per os treatment compared to other intravenous treatments. However, several negative determinants emerged such as the toxicity of the treatment, fears of forgetting to take the medication, difficulties with the galenic formulation and negative beliefs of the family. Conclusion This study demonstrates the need to address the different dimensions of the patient in order to understand his or her behaviour with regard to adherence and to identify the levers for improvement.

Abstract:Objective To explore the influence factors of medication adherence of hormonal therapy for breast cancer patients .Methods In-depth interviews were conducted to 15 healthcare professionals . Results The results of in-depth interviews for breast cancer experts showed that medication adherence of hormonal therapy of breast cancer patients was influenced by multiple factors , including patient-related, condition-related, therapy-related, family related and health system related factors .The balance between side effects and efficacy were the key point of medication adherence for breast cancer patients .The awareness on the importance of hormonal therapy was insufficient among breast cancer patients .Lack of dynamic monitoring indicators and channels to communicate with health professionals were the most significant influence on the adherence to hormonal therapy .Conclusions It is recommended that individualized and effective interventions should be implored to improve the adherence to hormonal therapy for breast cancer patients .

Abstract:Clinical Nursing Research, Ahead of Print. This study aims to synthesize the barriers and facilitators of oral nutritional supplement adherence in people with cancer and assess levels of adherence. Databases including Embase, PubMed, Web of Science, Cochrane Library, Chinese National Knowledge Infrastructure, Wanfang Database, and Chinese Scientific Journal Database, were used to search articles before January 4, 2021. Eighteen studies with 1,267 participants met the inclusion criteria. All studies reported rates of adherence, ranging from 6.0% to 96.9%. Gastrointestinal intolerances and disliked tastes were considered common barriers to cancer patients' adherence, whereas the ONS management strategy was considered the most crucial facilitator of adherence. Health professionals can improve adherence through regular monitoring and guidance and incentive strategies, and paying attention to the management and prevention of adverse reactions when prescribing nutritional products.

Abstract:Objective To investigate the symptoms,management,and influencing factors of constipation in cancer patients.Methods From 21 April to 27 April 2010,67 cancer patients were admitted in our wards and received questionnaire investigation about constipation.Results Constipation was confirmed in 30 patients(44.77%).The influencing factors included use of antiemetic drugs,long bed time,less physical activity,use of pain killers,chemotherapy, and other drug-related risks.Conclusion Constipation frequently occurs in cancer patients due to a variety of factors, and therefore should be carefully managed.
Medication compliance and influencing factors of oral chemotherapy among patients with colorectal cancer: a longitudinal investigation

Abstract:Objective To explore medication compliance and influencing factors at different stages of chemotherapy for orally taking capecitabine patients with colorectal cancer.Methods Totally 119 colorectal cancer patients orally taking capecitabine were investigated using Morisky Medication Adherence Scale 8(MMAS-8),quality of life questionnaire cancer30(EORTC QLQC30) and Beliefs about Medicines Questionnaire Specific(BMQ Specific) at the early stage (at the end of the first cycle),the middle stage (at the end of the 4 cycle) and the terminal stage (at the end of the 8 cycle) of medication.Results A total of 15 patients gave up taking medication,and 12 of them gave up at the first 3 cycles of chemotherapy.The scores of patients' medication compliance at the early stage,the middle stage and the terminal stage was 7.43±1.00,6.98±1.13 and 6.83±1.30 respectively,and there were significant differences among the scores at different stage (P＜0.05).Regression analysis showed that,the predictors of medication compliance were loss of appetite,educational background and beliefs in medication at the early stage,fatigue and stoma at the middle stage,and living in Shanghai or other provinces and cities at the terminal stage.Conclusion Colorectal cancer patients undergoing oral chemotherapy are prone to give up medication in the first 3 cycles,and the patients who didn't give up totally have good medication compliance,which are influenced by different factors at different stage,thus nursing staff should formulate targeted nursing interventions.

Abstract:Adherence to chemotherapy treatment is recognized as a crucial health concern, especially in managing cancer patients. Chemotherapy presents challenges for patients, as it can lead to potential side effects that may adversely affect their mobility and overall function. Patients may sometimes neglect to communicate these side effects to health professionals, which can impact treatment management and leave their unresolved needs unaddressed. However, there is limited understanding of how patients' experiences contribute to improving adherence to chemotherapy treatment and the provision of appropriate support. Therefore, gaining insights into patients' experiences is crucial for enhancing the accompaniment and support provided during chemotherapy.

Abstract:Oral chemotherapy is an important topic in the 21st century medicine, which may radically change the current regimen of chemotherapy and greatly improve the quality of life of the patients. Unfortunately, most anticancer drugs, especially those of high therapeutic efficacy such as paclitaxel and docetaxel, are not orally bioavailable due to the gastrointestinal (GI) drug barrier. The molecular basis of the GI barrier has been found mainly due to the multidrug efflux proteins, i.e. P-type glycoproteins (P-gp), which are rich in the epithelial cell membranes in the GI tract. Medical solution for oral chemotherapy is to apply P-gp inhibitors such as cyclosporine A, which, however, suppress the body's immune system either, thus causing medical complication. Pharmaceutical nanotechnology, which is to apply and further develop nanotechnology to solve the problems in drug delivery, may provide a better solution and thus change the way we make drug and the way we take drug. This review is focused on the problems encountered in oral chemotherapy and the pharmaceutical nanotechnology solutions such as prodrugs, nanoemulsions, dendrimers, micelles, liposomes, solid lipid nanoparticles and nanoparticles of biodegradable polymers. Proof-of-concept in vitro and in vivo results for oral delivery of anticancer drugs by the various nanocarriers, which can be found so far from the literature, are provided.
Adherence Status to Adjuvant Endocrine Therapy in Chinese Women with Early Breast Cancer and Its Influencing Factors: A Cross‐sectional Survey

Abstract:Abstract Objective Despite the proven benefits of adjuvant endocrine therapy, adherence to oral endocrine therapy in breast cancer treatment is a substantial problem. The aim of this study was to assess adherence to adjuvant endocrine therapy by women in China for the first 5 years, and to identify its influencing factors. Methods Stratified sampling method was adopted to select 1875 cases of breast cancer patients for cross‐sectional telephone follow‐up. Compliance to medications was assessed using the Morisky Medication Adherence Scale. Status of endocrine therapy was assessed using nine additional questions. Binomial regression was used when assessing the factors associated with persistence, multinomial regression models were used to assess factors associated with compliance. Results Of 888 patients who started adjuvant endocrine therapy, 769(86.6%) persisted and 119 (13.4%) discontinued. 760 patients who completed Morisky Medication Adherence Scale, the compliance was 7.4% low, 42% medium, and 50.6% high. The type of medication, duration of medication and side effects had an impact both on persistence and compliance. Age, history of radiotherapy and caregivers only had an impact on persistence. Conclusions Medication adherence was affected by many factors. Special attention and interventions should be given to women taking tamoxifen in the 2nd to 3rd year of medication, and aromatase inhibitors in the 1st to 2nd year. Further prospective design studies are needed to explore effective measures to improve medication adherence of women with breast cancer treated by endocrine therapy.

A Systematic Review on Influencing Factors of Nutritional Status in Patients with Head and Neck Cancer

Abstract:Objective To systematically review the influencing factors of nutritional status in patients with head and neck cancer(HNC). Methods Researches on influencing factors of nutritional status in HNC patients were retrieved in databases,such as PubMed,EBSCO,Web of Science,CNKI,WanFang Data and Chinese Biological Medicine(CBM)by computer from January 2010 to July 2018. Quality assessment, content extract and analysis was carried out for literatures met inclusive criteria. Results Finally,a total of 19 researches were included.Qualitative analysis and simple correlation showed that those researches involved 47 factors,the influencing factors of nutritional status included the reduced ingestion,high levels of weight and body mass index(BMI)before treatment,low levels of hemoglobin and serum albumin,swallowing problem,thirst,sense problem,constipation and oral mucositis;23 factors of them were with varying degrees of influence,such as the ages,males,education levels and drinking. Multivariate analysis showed that those researches involved 35 factors;the influencing factors of nutritional status of HNC patients included the radiotherapy types/technology and swallowing problem;17 factors of them were with varying degrees of influence,such as the ages,habitation and smoking. In simple correlation and multivariate analysis,swallowing problem was the only factor with statistical differences in two kinds of analysis;the ages,tumor stage,tumor location/kinds of disease,grades of T and N,complications after radiotherapy and adverse reactions were only with statistical differences in simple correlation but without statistical differences in multivariate analysis. Conclusions Nutritional status of HNC patients is influenced by many factors and influence of most factors is uncertain with great individual differences. Influencing factors of nutritional status of HNC patients and mechanisms need to be explored and expounded respectively by further studies,especially pay attention to swallowing problem of HNC patients.

Abstract:Purpose: To provide evidence support for the development of clinical practice guidelines regarding patient adherence to medication protocols used in highly active antiretroviral therapy (HAART) in China.Methods: We analyzed information contained in recent systematic reviews and meta-analyses regarding patient compliance with medication protocols used in HAART.Results: Nine systematic reviews and one meta-analysis were included in our study which involved three different aspects of patient compliance: influencing factors, assessment methods, and interventions.Conclusions: The high quality data obtained from our study was suitable for use in developing clinically useful guidelines for patent compliance with HAART medication protocols. Adherence with oral nutritional supplements and influencing factors in postoperative patients with digestive tract tumors: a cross-sectional study

Abstract:Objective: This study aims to use structural equation modeling to explore the pathways and effect sizes of factors influencing the adherence of postoperative patients with digestive tract tumor to oral nutritional supplements, providing a theoretical basis for future nursing intervention measures. Methods: A total of 300 postoperative patients with digestive tract tumor within 30 days after surgery were conveniently sampled. Surveys were conducted using a General Information Questionnaire, Morisky Medication Adherence Scale, Digestive System Tumor Patient Nutrition Knowledge-Attitude-Behavior Questionnaire, Multidimensional Social Perception Scale, Beliefs about Medical Questionnaire, and General Self-Efficacy Scale. Structural equation modeling was employed to analyze the factors and pathways affecting adherence with oral nutritional supplements. Results: The adherence score of postoperative patients with digestive tract tumor to oral nutritional supplements was 1.61 ± 1.38. The structural equation model had a good fit (χ2/df = 2.685, GFI = 0.930, CFI = 0.913, AGFI = 0.887, IFI = 0.915, and RMSEA = 0.075). Nutrition knowledge, social support, medication beliefs, and self-efficacy were found to be factors influencing adherence with oral nutritional supplements in postoperative patients with digestive tract tumor, with total effects of 0.539, 0.264, 0.215, and 0.180, respectively. Nutrition knowledge indirectly affected adherence through self-efficacy and medication beliefs, while social support indirectly affected adherence through self-efficacy. Conclusion: Adherence with oral nutritional supplements in postoperative patients with digestive tract tumor is at a low level. Improving social support, enhancing patients nutrition knowledge, increasing self-efficacy, and strengthening medication beliefs are effective ways to improve patient adherence.

Abstract:Oral cancer is one of the most common tumours in the world threatening human life and health. The 5-years survival rate of patients with oral cancer has not been improved significantly for many years. The existing clinical diagnostic methods rarely achieve early diagnosis due to deficiencies such as lack of sensitivity. Most of the patients have progressed to the advanced stages when oral cancer is detected. Unfortunately, the traditional treatment methods are usually ineffective at this stage. Therefore, there is an urgent need for more effective and precise techniques for early diagnosis and effective treatment of oral cancer. In recent decades, nanomedicine has been a novel diagnostic and therapeutic platform for various diseases, especially cancer. The synthesis and application of various nanoagents have emerged at the right moment. Among them, polymer nanoagents have unique advantages, such as good stability, high biosafety and high drug loading, showing great potential in the early accurate diagnosis and treatment of tumours. In this review, we focus on the application of advanced polymeric nanoagents in both the diagnosis and treatment of oral cancer. Then, the future therapy strategies and trends for polymeric nanoagents applied to oral cancer are discussed, with the hope that more advanced nanomedical technology will be applied to oral cancer research and promote the development of stomatology.

Abstract:Abstract Although notable progress has been made on novel cancer treatments, the overall survival rate and therapeutic effects are still unsatisfactory for cancer patients. Chemoimmunotherapy, combining chemotherapeutics and immunotherapeutic drugs, has emerged as a promising approach for cancer treatment, with the advantages of cooperating two kinds of treatment mechanism, reducing the dosage of the drug and enhancing therapeutic effect. Moreover, nano-based drug delivery system (NDDS) was applied to encapsulate chemotherapeutic agents and exhibited outstanding properties such as targeted delivery, tumor microenvironment response and site-specific release. Several nanocarriers have been approved in clinical cancer chemotherapy and showed significant improvement in therapeutic efficiency compared with traditional formulations, such as liposomes (Doxil ® , Lipusu ® ), nanoparticles (Abraxane ® ) and micelles (Genexol-PM ® ). The applications of NDDS to chemoimmunotherapy would be a powerful strategy for future cancer treatment, which could greatly enhance the therapeutic efficacy, reduce the side effects and optimize the clinical outcomes of cancer patients. Herein, the current approaches of cancer immunotherapy and chemoimmunotherapy were discussed, and recent advances of NDDS applied for chemoimmunotherapy were further reviewed.

Abstract:Chemotherapy remains a prevalent treatment for a wide range of tumors; however, the majority of patients undergoing conventional chemotherapy experience varying levels of chemoresistance, ultimately leading to suboptimal outcomes. The present article provided an in‐depth review of chemotherapy resistance in tumors, emphasizing the underlying factors contributing to this resistance in tumor cells. It also explored recent advancements in the identification of key molecules and molecular mechanisms within the primary chemoresistant pathways.

Abstract:Cancer remains a leading cause of death globally, with patients frequently experiencing malnutrition due to both the disease and its treatment, which negatively affects their quality of life and treatment outcomes. Oral nutritional supplements (ONS) provide a noninvasive solution to improve nutritional status, but poor patient adherence limits their effectiveness. Studies on ONS adherence vary in their definitions and assessment tools, creating inconsistencies. A scoping review is essential to synthesize these studies and establish a foundation for future research and clinical practice.

Abstract:Objectives: Our objective was to explore the effect of a pharmaceutical care program on adherence toadjuvant endocrine therapy (AET) in breast cancer patients. Methods: Female breast cancer patients takingAET (N=429) were enrolled in the study; the pharmaceutical care program was implemented and the effect ofthis intervention was evaluated. The primary outcomes were scores on an 8-item medication adherence scaleChinese version (MMAS), beliefs about medicines questionnaire specific (BMQ), 5-level EuroQol 5 dimensionsquestionnaire (EQ-5D-5L) and a patient satisfaction questionnaire (PSQ-18). Results: We found statisticallysignificant difference in scores on MMAS and BMQ-concern (p < .05) before and after intervention. TheMMAS score was 6.12 (±1.21), 7.15 (±0.92) and 7.18 (±0.96) at baseline, 6 months and 12 months, respectively.The BMQ-concern score was 15.64 (±2.14), 12.26 (±2.85), 11.56 (±2.27) at baseline, 6 months and 12 monthsseparately. Conclusion: The pharmaceutical care program significantly improved medication adherenceand reduced patients’ medication concerns to adjuvant endocrine therapy in Chinese breast cancer patients.
