Abstract:Diffuse large B-cell lymphoma (DLBCL) is stratified into prognostically favorable germinal center B-cell (GCB)-like and unfavorable activated B-cell (ABC)-like subtypes according to gene expression signatures. ABC-DLBCL is characterized by its constitutive NF-κB activation and upregulation of a variety of target genes. However, it is not known which proteins contribute to the poor prognosis of ABC-DLBCL. Double-hit lymphoma with concurrent MYC and BCL2 rearrangements is a subset of aggressive lymphoma with a predominance of GCB immunophenotype.

We conclude that MYC/BCL2 coexpression in DLBCL is associated with an aggressive clinical course, is more common in the ABC subtype, and contributes to the overall inferior prognosis of patients with ABC-DLBCL. In conclusion, the data suggest that MYC/BCL2 coexpression, rather than cell-of-origin classification, is a better predictor of prognosis in patients with DLBCL treated with R-CHOP.

Abstract:OBJECTIVE:To investigate the mutation of 5' non-coding region of bcl-6 gene in germinal center B-cell (GCB) subtype of diffuse large B-cell lymphoma (DLBCL).METHODS:t(14;18) detection and immunohistochemical staining (EnVision method) were performed in 60 cases of DLBCL, which were divided into GCB and non-GCB subtypes. Polymerase chain reaction (PCR), single-strand conformation polymorphism and direct DNA sequencing were used to identify mutations in the 5' non-coding region of the bcl-6 gene.RESULTS:Seven of 60 cases showed t(14;18) translocation in the major breakpoint region. Using minimally acceptable criteria, 18 of 60 cases were probably to be germinal centre derived. Bcl-6 mutations were detected in 12 of 60 cases (20.0%) of DLBCL, with a significantly higher frequency in the GCB subgroups (7/18) than in the non-GCB subgroups (11.9%, 5/42). Bcl-6 mutations occurred most frequently in +363 and +469 sites. An association of bcl-6 mutation and GCB subgroup was obtained.CONCLUSIONS:The 5' regulatory region of the bcl-6 gene underwent less frequent somatic hypermutation during lymphomagenesis than the results of previous reports. Bcl-6 mutation occurred mostly in the GCB subtype and detection of t(14;18) seems helpful in the classification of DLBCL.

Abstract:To investigate the role of BCL6 in the pathogenesis of gastric lymphoma, we analyzed the BCL6 promoter region for BCL6 translocations, somatic hypermutations, and deregulating mutations in 43 gastric lymphomas, including 4 extranodal marginal-zone B-cell lymphomas of mucosa-associated lymphoid tissues (MALT lymphomas), 33 diffuse large B-cell lymphomas (DLBCLs), and 6 composite DLBCLs with residual MALT lymphoma (DLCLMLs). BCL6 promoter substitutions by immunoglobulin (Ig) and non-Ig translocation partners, resulting in its deregulation, were frequently involved in DLBCL (36.4%) and DLCLML (50%). Two novel BCL6 translocation partner genes, 28S rRNA and DMRT1, and a new BCL6 translocation breakpoint in intron 2 were also identified. Deregulating mutations were found only in DLBCL (24.2%), which correlated significantly with high BCL6 protein expression. Significantly, high BCL6 expression correlated strongly with longer overall survival (OS), independent of mechanism in gastric DLBCL and DLCLML. Gastric DLBCLs were further subclassified into germinal center B-cell-like (GCB) and non-GCB subgroups immunohistochemically. High BCL6 expression was detected in all GCB cases, irrespective of BCL6 genetic alterations. In the non-GCB subgroup, BCL6-deregulating mutations correlated significantly with high BCL6 expression level. No significant correlation was found between the BCL6 expression level and OS in the non-GCB subgroup, which had significantly poorer prognosis than the GCB subgroup.

Abstract:OBJECTIVE To investigate bcl-6 protein expression and gene rearrangement patterns in diffuse large B-cell lymphoma (DLBCL) and their clinicopathologic significance. METHODS Immunohistochemical studies for bcl-6 and CD10 proteins were performed on 51 cases of DLBCL paraffin-embedded tissues (including 22 nodal samples and 29 extranodal samples) and 10 cases of reactive lymphoid hyperplasia (RLH) paraffin-embedded tissues. Interphase fluorescence in-situ hybridization (FISH) with dual color breakapart probe was also used to identify rearrangement of bcl-6 gene in 32 cases of nodal DLBCL tissues (including 22 paraffin-embedded samples and 10 fresh samples) and 5 cases of RLH paraffin-embedded tissues. RESULTS (1) The rates of bcl-6 protein expression in nodal DLBCL, extranodal DLBCL and RLH were 72.7% (16/22), 75.9% (22/29) and 100.0% (10/10) respectively. The rates of CD10 expression were 40.9% (9/22), 41.4% (12/29) and 100.0% (10/10) respectively. All lymphoma samples which expressed CD10 also showed co-expression of bcl-6 protein. (2) The co-expression of bcl-6 and CD10 was observed in 40.9% (9/22) nodal DLBCL and 41.4% (12/29) extranodal DLBCL. Low clinical stage (stage I and II) was more frequently observed in cases with co-expression of bcl-6 and CD10 (P < 0.05). (3) The rates of bcl-6 gene rearrangement in nodal DLBCL was 28.1% (9/32), with 27.3% (6/22) in paraffin-embedded tissues and 30.0% (3/10) in fresh tissues. There was no statistically significant difference found between the two groups (P > 0.05). Bcl-6 gene rearrangement was not found in all the 5 cases of RLH, and there was a significant difference between RLH and DLBCL (P < 0.05). CONCLUSIONS The rate of bcl-6 protein expression is high in DLBCL cases, and the detection of bcl-6 and CD10 protein co-expression may help in the diagnosis and differential diagnosis of DLBCL. Those DLBCL cases with co-expression of bcl-6 and CD10 may also have a better prognostic implication. On the other hand, bcl-6 gene rearrangement can be identified by interphase FISH with dual color breakapart probe in both paraffin-embedded and fresh lymphoma tissues.

Abstract:[Objective] To investigate the ralationship between bcl-6,bcl-2 and ki-67 as well as the clinical features and prognosis of diffuse large B cell lymphoma(DLBCL).[Methods] Immunohistochemistry stain was used to determin the expression of bcl-6,bcl-2 and ki-67.[Results] Positive rates of bcl-6 and bcl-2 were 65.6% and 71.9%,the high expression rate of ki-67 was 65.6%.there was a significant correlationship between the bcl-6 and Ann Arbor stage,bcl-2 had a relationship with LDH and Ann Arbor stage;ki-67 had a relationship with age,LDH and Ann Arborstage;The average survival time of bcl-6 positive group and negative group were 54.8 months and 22.2 months,bcl-2 positive group and negative group were 26.5 months and 68.1 months,ki-67 high-expression group and low-expression group were 26.5 months and 59.6 months;bcl-6 positive,bcl-2 negative,Ki-67 low-expression showed a trend to better the overall survival.[Conclusion] The results suggest a signficant correlationship among bcl-6,bcl-2,ki-67 and prognosis.Moreover bcl-6,bcl-2 and ki-67 may be the useful prognostic indicator in diffuse large B cell lymphoma.

Abstract:OBJECTIVE To investigate BCL-6 gene mutations in B-cell non-Hodgkin lymphomas (B-NHL) and their implications in lymphoma pathogenesis. METHODS Polymerase chain reaction (PCR) and direct DNA sequencing methods were used to identify mutations in the 5'-noncoding region of BCL-6 gene in 135 cases of B-NHL, 5 cases of T-NHL, 5 cases of nodular lymphocyte predominance Hodgkin's lymphoma (NLPHL) and 10 cases of reactive hyperplasia of lymph node. RESULTS Mutations were identified in 6 cases of nodal DLBCL (27.3%), 4 cases of FL (22.2%), 4 cases of MALT lymphoma (22.2%), 4 cases of extranodal DLBCL (20.7%) and 2 cases of LRH (20%). No mutations were detected in T-NHL and NLPHL (P < 0.05). There were no significant differences in incidences of BCL-6 gene mutations between nodal and extranodal DLBCL (P > 0.05). All mutations were base substitutions and the frequency of single-base change was 0.14 x 10(-2)/bp approximately 0.68 x 10(-2)/bp. CONCLUSIONS Mutations of the 5'non-coding region of BCL-6 gene may be involved in the pathogenesis and progression of B-NHL. Molecular demonstration of such mutations may provide a marker of lymphomas derived from the germinal center-related B cells.

Abstract:Background and Purpose:Both Bcl-6 and CD10 are markers of germinal center(GC) B cells in the lymph node.Their experssion in nodal diffuse large B cell lympoma(DLBCL),extranodal DLBCL and other types of lymphoma and the sigifcance is worthy to be investigated.This paper will investigate the protein expression of BCL-6,CD10,BCL-2 gene in B-cell Non-Hodgkin's lymphomas(B-NHL) in Chinese populations and its clinic-pathological significance.Methods:Immunohistochemical analyses of BCL-6,CD10 and BCL-2 protein expression were performed on 135 cases of B-NHL including the common types occurring in Chinese populations and 5 cases of T-NHL,5 cases of nodular lymphocyte predominance Hodgkin's lymphoma(NLPHL) and 10 cases of Reactive lymphoid hyperplasia(RLH) with paraffin-embedded tissues.Results:① BCL-6 and CD10 protein were expressed almost exclusively in RLH,nodal DLBCL,extranodal DLBCL,follicular lymphoma(FL) and Burkitt's lymphoma(BL),the rates of positivity were 100% and 100%,72.7% and 40.9%, 75.9% and 41.4%,(88.9%) and 72.2% as well as 100% and 100%,respectively.BCL-6 and CD10 protein were not expressed in other B-NHL such as small lymphocytic lymphoma(B-SLL),mantle cell lymphoma(MCL),extranodal marginal zone B-cell lymphoma of mucosa-associated lymphoid tissue(MALT-L),Lymphoplasmacytic lymphoma(LPL) and lymphoblastic lymphoma(LBL) as well as T-NHL and NLPHL.BCL2 protein were expressed in nodal DLBCL,extranodal DLBCL,FL,B-SLL,MCL,MALT-L and LBL,the rates of positivity were 36.4%,27.6%,83.3%,(88.9%,)86.7%,72.7% and 33.3%,respectively.BCL-2 protein were not expressed in LPL,BL,T-NHL and NLPHL.② BCL-6 protein expression in DLBCL could be classified into four patterns: GC/FL pattern,intermediate pattern,sporadic pattern and rare or absent pattern.③ The coexpression of BCL-6 and CD10 was observed in 40.9% nodal DLBCL,41.4% extranodal DLBCL,72.2% FL and 100% BL.Among them the BCL-6 protein expressions were GC/FL pattern.(4)Low clinic stage(stageⅠandⅡ) was more frequent in nodal DLBCL cases with coexpression of BCL-6 and CD10 than those without(P0.05).Conclusions:Detection of BCL-6,CD10 and BCL-2 protein expression in combination may contribute to pathological diagnosis and differential diagnosis of some B-NHL.Nodal DLBCL with both BCL-6 and CD10 experssion pattern patients may have much better prognosis.

Abstract:B-cell lymphoma 6 (BCL6) is a transcriptional repressor that regulates the differentiation of B lymphocytes and mediates the formation of germinal centers (GCs) by recruiting corepressors through the BTB domain of BCL6. Physiological processes regulated by BCL6 involve cell activation, differentiation, DNA damage, and apoptosis. BCL6 is highly expressed when the gene is mutated, leading to the malignant proliferation of cells and drives tumorigenesis. BCL6 overexpression is closely correlated with tumorigenesis in diffuse large B-cell lymphoma (DLBCL) and other lymphomas, and BCL6 inhibitors can effectively inhibit some lymphomas and overcome resistance. Therefore, targeting BCL6 might be a promising therapeutic strategy for treating lymphomas. Herein, we comprehensively review the latest development of BCL6 inhibitors in diffuse large B-cell lymphoma and discuss the overview of the pharmacophores of BCL6 inhibitors and their efficacies in vitro and in vivo. Additionally, the current advances in BCL6 degraders are provided.

Abstract:Objective To investigate BCL-6 gene mutations in Chinese populations with B-cell non-Hodgkin's lymphoma.Methods Polymerase chain reaction (PCR), denaturing gradient gel electrophoresis (DGGE) and direct DNA sequencing were used to identify mutations in the 5' noncoding region of the BCL-6 gene in a total of 40 cases of diffuse large-cell lymphoma (DLCL) and follicular lymphoma (FL).Results Nine cases were found to have base substitutions. The incidence of BCL-6 gene mutation and the frequency of single-base changes were approximately 25.7% and (0.56 - 1.10) x 10(-2)/bp, respectively.Conclusions The 5' regulatory region of the BCL-6 gene undergoes frequent somatic hypermutation during lymphomagenesis and the identification of BCL-6 gene hypermutations provides a molecular marker for confirmatory diagnosis of B-NHL.

Abstract:Structural alterations of the promoter region of the BCL-6 proto-oncogene represent the most frequent genetic alteration associated with non-Hodgkin lymphoma, a malignancy often deriving from germinal-centre B cells. The BCL-6 gene encodes a zinc-finger transcriptional represser normally expressed in both B cells and CD4+ T cells within germinal centres, but its precise function is unknown. We show that mice deficient in BCL-6 displayed normal B-cell, T-cell and lymphoid-organ development but have a selective defect in T-cell-dependent antibody responses. This defect included a complete lack of affinity maturation and was due to the inability of follicular B cells to proliferate and form germinal centres. In addition, BCL-6-deficient mice developed an inflammatory response in multiple organs characterized by infiltrations of eosinophils and IgE-bearing B lymphocytes typical of a Th2-mediated hyperimmune response. Thus, BCL-6 functions as a transcriptional switch that controls germinal centre formation and may also modulate specific T-cell-mediated responses. Altered expression of BCL-6 in lymphoma represents a deregulation of the pathway normally leading to B cell proliferation and germinal centre formation.

Abstract:OBJECTIVE:To observe the mutation of 5'noncoding region of bcl-6 gene in diffuse large B cell lymphoma (DLBCL) and its effect on lymphoma pathogenesis.METHODS:38 DLBCL, 2 reactive hyperplasias, 5 follicular lymphomas and 5 T cell lymphomas were chosen for PCR direct sequence analysis using two sets of primers in 5'noncoding region of the bcl-6 gene.RESULTS:No mutation was found in the marginal region of reactive hyperplasias, T cell lymphomas, and follicular lymphomas but detected in 1/2 of the follicular center cells, and 7/38 cases of DLBCL. The incidence is less than that seen in other reports. Basepairs substitution and point insertion were the main mutation types.CONCLUSIONS:The positive rate of mutation of 5'noncoding region of bcl-6 gene in DLBCL is 18.7%, less frequent than the published data of DLBCL reported in other countries. It may, in some extent, participate in the pathogenesis and progression of DLBCL.
The Relationship between RUVBL1 (Pontin, TIP49, NMP238) and BCL6 in Benign and Malignant Human Lymphoid Tissues

Abstract:The human BCL6 gene, which is involved in the pathogenesis of certain human lymphomas, encodes a transcriptional repressor that is needed for germinal center B cell development and T follicular helper cell differentiation. Our goal was to identify BCL6 target genes using a cell system in which BCL6 repressive effects are inhibited followed by subtractive hybridization, and we detected the RUVBL1 (Pontin, TIP49) gene as a potential target of BCL6 repression. Here we show that the BCL6 protein significantly represses RUVBL1 transcription (6.8-fold). Knockdown of endogenous BCL6 in a human B cell lymphoma line leads to significant upregulation of RUVBL1, and there is an inverse expression pattern between the BCL6 and RUVBL1 proteins in certain human lymphomas. RUVBL1 is part of the AAA+ superfamily and participates in multiple processes, including gene transcription regulation, chromatin remodeling, and DNA repair, which, if dysregulated, may promote lymphoma development. A further understanding of the relationship between RUVBL1 and BCL6 should improve our understanding of the pathogenesis of human lymphomas.

Abstract:Cell division cycle 6 (CDC6) is a key licensing factor in the assembly of pre-replicative complexes at origins of replication. The role of CDC6 in the pathogenesis of in diffuse larger B-cell lymphoma (DLBCL) remains unknown. We aim to investigate the effects of CDC6 on the proliferation, apoptosis and cell cycle regulation in DLBCL cells, delineate its underlying mechanism, and to correlate CDC6 expression with clinical characteristics and prognosis of patients with DLBCL.

Abstract:BACKGROUND: Diffuse large B-cell lymphoma (DLBCL) is the most common type of non-Hodgkin's lymphoma, and hence, a comprehensive understanding based on the gene expression profile is imperative. Although several studies have identified some critical mutant genes of DLBCL, the disease in the central nervous system has not been investigated clearly. This study is aimed to identify some novel and important mutant genes of DLBCL in central nervous system.METHODS: A total of 156 cases of central nervous tumors were collected from 2016 to 2018, in which the DLBCL cases were confirmed by H&E staining and immunohistochemistry. With the whole-genome high-throughput sequencing, the mutations of samples were identified. By matching with TCGA database, the common mutations of DLBCL were further confirmed.RESULTS: Twelve cases were designated as DLBCL, of which 1 case was classified into germinal center B cell (GCB) subtype, and 11 cases were non-GCB subtypes. The gene mutation spectrum demonstrated that the most common substitutions of six single bases were C>T/G>A, wherein the mutation frequency of C(C>T) G was the highest. The most common type of mutation is missense, and the most frequently mutated genes included MYD88, LRP1B, CD79B, GNA13 and PIM1. Based on the TCGA database, finally, the 4 significantly mutated genes (SMG), including MYD88, PIM1, CD79B, and BTG1 common in the above groups, were identified.CONCLUSIONS: Taken together, the analysis of the TCGA database and the results of the sequencing experiment displayed four mutations that might provide novel targets for the treatment of DLBCL.

Abstract:Background B7-H6 is a novel co-stimulatory protein exclusively expressed on a variety of cancer cells and associated with poor prognosis. T-cell lymphoblastic lymphoma (T-LBL) is a highly aggressive hematological malignancy whose treatment requires reliable prognostic biomarkers and therapeutic targets. However, the rare nature and delayed progression of T-LBL have limited its clinical management. Methods The expression of B7-H6 was analyzed by immunohistochemistry (IHC) in 65 T-LBL samples; the association with the clinicopathological characteristics and prognosis was also investigated. B7-H6-depleted Jurkat cells were also generated to investigate the effect of B7-H6 on cell proliferation, migration, and invasion. RNA sequencing was used to explore differentially expressed genes. Results B7-H6 was expressed in 61.5% (40/65) of T-LBL patients; of note, 38.5% (25/65) of patients showed membrane/cytoplasmic expression of B7-H6. Although the expression of B7-H6 varied across samples and did not correlate with patient survival, it was significantly associated with B symptoms, high ECOG scores (3 to 4), elevated serum lactate dehydrogenase level, and reduced complete remission at interim evaluation. B7-H6 underwent translocation into the nucleus of T-LBL cells, showing a specific nuclear localization sequence in the C-terminus. Moreover, the depletion of B7-H6 in Jurkat cells impaired cell proliferation, migration, and invasion. RNAseq showed the differential expression of RAG-1, which may be involved in the tumorigenesis of T-LBL. Conclusions B7-H6 may serve as a novel prognostic biomarker and therapeutic target of T-LBL.

Abstract:Human malignancies displaying a mature B cell phenotype include non-Hodgkin lymphoma, (NHL), chronic lymphocytic leukemia (CLL), and multiple myeloma (MM). Analogous to most cancer types, the pathogenesis of these malignancies represents a multistep process involving the progressive and clonal accumulation of multiple genetic lesions affecting proto-oncogenes and tumor suppressor genes. However, several important features distinguish the mechanism and type of genetic alterations associated with NHL, CLL, and MM (Table 1).

Abstract:BCL6 was initially discovered as an oncogene in B-cell lymphomas, where it drives the malignant phenotype by repressing proliferation and DNA damage checkpoints and blocking B-cell terminal differentiation. BCL6 mediates its effects by binding to hundreds of target genes and then repressing these genes by recruiting several different chromatin-modifying corepressor complexes. Structural characterization of BCL6–corepressor complexes suggested that BCL6 might be a druggable target. Accordingly, a number of compounds have been designed to bind to BCL6 and block corepressor recruitment. These compounds, based on peptide or small-molecule scaffolds, can potently block BCL6 repression of target genes and kill lymphoma cells. In the case of diffuse large B-cell lymphomas (DLBCL), BCL6 inhibitors are equally effective in suppressing both the germinal center B-cell (GCB)- and the more aggressive activated B-cell (ABC)-DLBCL subtypes, both of which require BCL6 to maintain their survival. In addition, BCL6 is implicated in an expanding scope of hematologic and solid tumors. These include, but are not limited to, B-acute lymphoblastic leukemia, chronic myeloid leukemia, breast cancer, and non–small cell lung cancer. BCL6 inhibitors have been shown to exert potent effects against these tumor types. Moreover, mechanism-based combinations of BCL6 inhibitors with other agents have yielded synergistic and often quite dramatic activity.
