Irreducible Atlantoaxial Dislocation and Ventral Decompression of Medulla and Spinal Cord

Abstract:Objective:To review the imaging study of the irreducible atlantoaxial dislocations and explore the feasibility of transoral or posterolateral approach for ventral decompression of the atlantoaxial dislocation. Method:The X-ray film,MRI and/or CT of craniocervical junction of 36 patients with irreducible atlantoaxial dislocation were reviewed and analyzed,of which 27 patients were operated by ventral decompression via posterolateral approach.Result:There were 23 cases of axis-intact atlantoaxial dislocation,3 cases of the atlas were anteroinferiorly rotated,4 cases were laterally displaced and 3 cases of the unilateral joint were dislocated severely.There were 21 cases with assimilated atlas in the axis-intact dislocation,of which 2 cases were with irregular stenosis of vertebral foramen of occipitated atlas and 1 case was with vertebrated odontoid. There were 13 cases of odontoid-detached atlantoaxial dislocation,of which the atlas was posteriorly displaced in 1 case,and the atlas were anteriorly suluxated in 8 cases (of which 1 case was with os odontoideum) and the atlas were anteriorly dislocated completely in 4 cases.Conclusion:In so far as the ventral decompression,the posterolateral approach is favorable for the treatment of atlantoaxial dislocation with"rotated"atlas,or with malformed odontoid (such as vertebrated odontoid,os odontoideum,et al) or with irregular-stenosed vertebral foramen of occipitated atlas,and the transoral lysis and reduction by traction is favorable and reasonable for the complete dislocation with detached odontoid.
