Research of Treating Premature Ventrucular Complex by Radiofrequeqncy Catheter Ablation

Abstract:Objective: To investigate the methods, effects and ECG characters of treating refractory premature ventricular complexes (PVCs) by radiofrequency catheter ablation (RFCA) through analyzing 23 patients with refractory PVCs. Methods: 23 patients with frequent, drug-resistant, symptomatic and refractory monomorphologic PVCs were chosen in this study, aged of (44 4± 9 6)?years, 16 males and 7 females. Pacing mapping and activation sequence mapping were adopted, the former employing the site whose morphology was the same as that of the premature ventricular QRS when pacing as ablation target site, the latter exerting the earliest ventricular excited site when premature pacing as ablation target site. Results: 17 patients PVCs originated from right ventricle, the other 4 from left ventricle. The immediately successful ablation rate was 86 9%(20/23). The accumulating discharge was (930 4±725)?s. Dynamic ECG was performed in curative patients to record the quantity of PVCs before and after the ablation, which was (5671±2?436)?t/24?h and (39±52)?t/24?h, respectively (P0 01). There was no recurrence or complication and the patients did not take any anti-irrhythmia medicine during the (23 2±12 3) months follow-up. Conclusion: RFCA can eliminate the monomorphologic PVCs effectively and safely. If using rough mapping activation sequence, precise mapping pacing image and long time high power, and extending the ablation area around the target site, successful rate could increase and recurrence could decrease. This method can be one of the therapeutic choices for the patients with duplicated symptoms or those in no effect and no tolerance of medicines.
