Clinical characteristics and classification of obese diabetic patients with unprovoked ketosis

Abstract:Objective To investigate the clinical characteristics and classification of obese diabetic patients with unprovoked ketosis or ketoacidosis. Methods According to body mass index (BMI), 66 patients were divided into 3 groups; underweight group (BMI≤18.5kg/m2, n =21), normal body weight group ( BMI 18.6-24.9 kg/m2,n=30) and obese group ( BMI≥25 kg/m2,n=15).Clinical characteristics, including age, sex, mode of presentation, glutamic acid decarboxylase antibody (GAD-Ab) , protein tyrosine phosphate antibody (IA2-Ab) and islet beta-cell function, were compared among 3 groups. Sequence-based genotyping was used to determine the alleles of HLA-DQA1 and DQB1 in obese patients, and the obese patients were followed up. Results There were no significant differences regarding some clinical features (glycemia, weight loss and ketosis status) at presentation among 3 groups. Compared with the underweight group, obese group had higher serum triglyceride level and lower GAD-Ab/IA2-Ab prevalence. When ketosis or ketoacidosis occurred, less severe insulin deficiency was observed in obese group. During the follow-up, 9 out of 15 obese patients could stop insulin therapy. Among 12 patients who were obese and negative for GAD-Ab and IA2-Ab, 6 patients did not carry any type 1 diabetes susceptible alleles. Conclusion The clinical and immunologic characteristics of obese and underweight patients are quite different, some of the obese patients may be classified as idiopathic type 1 diabetes.
