Clinical characteristics and radiological appearance in patients with combined pulmonary fibrosis and emphy-sema syndrome

Abstract:Objective To investigate the clinical and radiological features and pulmonary function changes of patients with combined pulmonary fibrosis and emphysema ( CPFE) syndrome. Methods The clinical features, im-aging and lung function indexes were retrospectively analyzed in 21 patients diagnosed with CPFE according to the findings by high resolution computerized tomography ( HRCT) from 2009 to 2013 at Xinhua Hospital. Results 21 patients were all male and heavy smokers, with an average age of 77 (60~88). 14 of them had dyspnea on exertion. Basal crackles were heard in 11 patients and finger clubbing was observed in 8 patients. Pulmonary function demon-strated that forced expiratory volume in one second/forced vital capacity ( FEV1/FVC) was slightly lower with a medi-an of 76. 4%. Forced vital capacity ( FVC) was 68. 7%, and the total lung capacity ( TLC) was 70%. The predic-tion value of carbon monoxide diffusion capacity ( DLCO) was 35%. HRCT findings included emphysema predomi-nantly at the upper zone, while reticular opacity, honeycombing and traction bronchiectasis were in the lower lobes. Pulmonary hypertension was found in 15 patients. Conclusion Upper lobe emphysema combined with lower lobe fi-brosis are the main HRCT features of patients with CPFE syndrome. Remarkable impairment in gas exchange com-bined with normal or nearly normal lung volume can be observed in CPFE patients. A high prevalence of pulmonary hypertension is found in CPFE syndrome, and some cases with pulmonary cancer. The pathogenesis and the treatment and prognosis need to be further studied.
