Abstract:Objective To summarize our surgical experience and evaluate therapeutic outcomes of atrioventricular valve regurgitation in patients with single ventricle.Methods The medical records of 61 consecutive patients with functional single ventricle who underwent surgical treatment in Xinhua Hospital,School of Medicine of Shanghai Jiaotong University from July 2006 to January 2012 were retrospectively reviewed.There were 36 males and 25 females with their age ranging from 2 months to 20 years and body weight of 2-58 kg.Dominant ventricle morphology was right in 41 patients,left in 13 patients and indeterminate in 7 patients.All the patients were divided into 3 groups according to the degree of valvular regurgitation:none/trace valvular regurgitation group(28 patients whose atrioventricular valve was not repaired),mild/moderate valvular regurgitation group(21 patients whose atrioventricular valve was not repaired),and severe valvular regurgitation group(12 patients who underwent concomitant valvuloplasty).Hospitalization and follow-up data of all the patients were collected.Long-term outcomes of atrioventricular valve regurgitation in the mild/moderate valvular regurgitation group and severe valvular regurgitation group were analyzed,as well as risk factors that affected atrioventricular valve regurgitation.Results There were 5 in-hospital deaths,and the overall hospital mortality was 8.2%(5/61).The valvular regurgitation degree of the severe valvular regurgitation group was significantly reduced after valvuloplasty(from preoperative 4.00 degree to postoperative 2.08 degree).Fifty-six patients were followed up for 6-38 months.Ten patients in the severe valvular regurgitation group were followed up,and 2 patients died during follow-up.Among the remaining 8 alive patients,there were 2 patients with severe valvular regurgitation,3 patients with moderate valvular regurgitation,2 patients with mild valvular regurgitation,and 1 patient with trace valvular regurgitation.The valvular regurgitation degree increased significantly during follow-up(from postoperative average 2.08 degree to average 2.75 degree during follow-up).Nineteen patients in the mild/moderate valvular regurgitation group were followed up with no death during follow-up.Among them,there were 3 patients whose valvular regurgitation degree increased to severe(postoperatively 1 patient with mild valvular regurgitation and 2 patients with moderate valvular regurgitation),and 3 patients whose postoperative mild valvular regurgitation increased to moderate valvular regurgitation during follow-up.The average valvular regurgitation degree increased from postoperative 2.33 degree to 2.58 degree during follow-up in this group.There was no statistical difference between the mild/moderate regurgitation group and none/trace regurgitation group in the increasing rate of valvular regurgitation during follow-up(the valvular regurgitation increasing rates were 31.5% and 19.2% respectively,χ 2 =0.36,P=0.55).Univariate analysis showed that heart function of patients whose valvular regurgitation increased during follow-up was significantly worse than that of patients whose valvular regurgitation did not change or decreased during follow-up(postoperative LVEF 53.11%±5.61% vs.59.65%±3.32%,t =-5.49,P=0.00),while the left ventricular end-diastolic volume of patients whose valvular regurgitation increased during follow-up was significantly larger than that of patients whose valvular regurgitation did not change or decreased during follow-up(t =2.58,P=0.01).Conclusions Short-term clinical outcomes of valvuloplasty for patients with severe atrioventricular valve regurgitation and single ventricle are satisfactory,but their valvular regurgitation may increase significantly with ventricular dilation and decreased heart function over time.Patients with mild/moderate valvular regurgitation may not receive concomitant atrioventricular valve repair,but valvular regurgitation of some patients is likely to increase,suggesting that more attentive postoperative follow-up is needed for them.
