Abstract:The idea that problems in early pregnancy commonly precede cerebral palsy (CP) dates back to its first descriptions as a clinical syndrome, just prior to the 20th century. Yet it was probably only after a severe rubella epidemic in 1941 in Australia that pregnancy infection was recognized to have severe neurodevelopmental consequences. The initial observation was that cataracts were more frequently identified in infants born to mothers who had rubella infection in early gestation. Microcephaly and hearing loss were also described as features of a ‘so-called’ rubella syndrome. Following another extensive rubella epidemic in 1964 in the United States, it was further reported that a chronic, disseminated infection could ensue, causing damage to multiple organs, including the developing brain. Similar features, including hearing loss and CP, were also recognized in children born to mothers infected by cytomegalovirus (CMV) or, to a lesser degree, other less prevalent infections. But not all infected pregnancies had infected fetuses, and not all infected fetuses who survived had neurodevelopmental impairment. The concept of a syndrome was initially invoked to explain or characterize such variability. Soon, a multifactorial view came into vogue, effectively ‘mathematizing’ causation. Great caution has since been advised to all who accept single-cause attribution of perinatal brain damage – and rightfully so. However, while such attribution still seems out of fashion, the neo-pragmatic view essentially argues that terminologies should be judged by whether they are helpful. In other words, a probabilistic view of causation is adopted, yet the features of the predictive model are less important than the fruits brought to bear by its use. The report by Smithers-Sheedy et al. of a ‘known cause’ field being used by the Australian Cerebral Palsy Register (ACPR) to classify some children as having CP attributed to congenital CMV infection is perhaps best appreciated in this light. I know of no study that shows meaningful agreement among investigators and/or chart reviewers in assigning a single known cause to a child’s CP. Thus, it seems that the ACPR’s classification of CP attributed to CMV infection is inherently flawed. However, vaccines for the prevention of CMV infection, antiviral drugs, and other therapeutic options targeting neuroinflammation for the prevention of perinatal brain damage are indeed seemingly on the horizon. Hence, population-based estimates of burden associated with specific infections (and/or inflammation unrelated to infection) are needed to inform the design, funding (i.e. prioritization), evaluation, and monitoring of preventive interventions. Moreover, lacking widespread screening and follow-up, estimates of CMV-associated burden in CP reported by the ACPR might just be the best currently available – warts and all. Yet, from a neo-pragmatic perspective, whether the limitations of attributing CP to congenital CMV infection as done by the ACPR is permissible, or even advised, depends solely on whether it truly motivates increased attention, funding, and/or scholarship towards the prevention of perinatal brain damage. Let us all strive collectively to make this a reality.
