Preservation of the Hypothalamus Function in Resecting Giant Pituitary Adenoma Via a Pterional Approach

Abstract:Objective To investigate the methods for preserving the hypothalamus function in resecting a giant pituitary adenoma via a pterional approach. Methods 120 cases of giant pituitary adenoma undergone surgical procedures were reported. Results Thirty-seven patients suffered from hypothalamus injury, which was closely associated with the tumor invasion to the bottom of the third ventricle of the brain or hardness of the tumor mass. Conclusion The use of proper surgical instruments and reduction of tension are crucial for protecting function of the hypothalamus. When dissecting carotid cistern and terminal cistern, attention should be paid to protection of the vessels supplying the hypothalamus. Some brunches driving from basal arteries can supply not only hypothalamus, but also tumor tissues; we should differentiate them as clearly as possible during ablating tumors.
