Chronic Heroin Toxic Encephalopathy

Abstract:Objective: To understand the clinical manifestations and MRI features of chronic heroin toxic encephalopathy, we analyzed two cases and made discussion based on reviewing relevant literatures. Both of 3 patients have definite heroin-taking history(duration 3~5 years), and their clinical presentations were characterized by a number of signs and symptoms which indicate chrojic brain impairments, including progressive disturbance of intelligence, dysarthria, dystonia and cerebellar ataxia. 1 patient(female)progressed to disturbance of consciousness. MRI scan of brain revealed diffuse, symmetrical lesions which are low signals on T 1 image and high signals on T 2 image, primarily in the white matter of cerebrum, cerebellum, posterior limbs of the internal capsule and corpus callosum, and they are not contrast enhancing as demyelination shows on MRI. By treated with glucocorticoid, the two patient(male)improved remarkably, and the other one had no changes. Early diagnosis is of great importance to the management of chronic heroin toxic encephalopathy.
