The Influences of Enteral, Parenteral Nutrition and Major Gastrointestinal Operation on Intestine Mucosal Immunity

Abstract:Objectives:To evaluate the influences of enteral nutrition (EN), parenteral nutrition (PN) and major operation (MO) on intestine mucosal immunity in abdominal surgical patients. Methods: A prospective, successive and controlled trial was taken. Fifty-eight non-infection patients with intestinal fistula and severe acute pancreatitis were divided into EN group(n=29) and PN group(n=29). Eleven patients, 48 hours after major gastrointestinal operation, were taken as MO group. 10 patients admitted with the diagnoses of cholecyst polyp and inguinal hernia were taken as control group. The blood level of albumin, IgA, SC, D-lactic acid and C reaction protein (CRP) were measured and compared in all groups. Results: The albumin level in four groups had no statistical difference. The IgA level in PN group was lower than that in EN, MO and control groups (P0.01). The D-lactic acid level in PN group was significantly higher than that in EN, MO(P0.01), and contol grorp (P0.05).The CRP level in MO group was the highest in four groups. There was no difference in SC level between EN,MO and control groups(P 0.05). Conclusions:Compared with control group, Large gastrointestimal operation may weaken the intestinal mucosal immune barrier function and increase possibility of infection from the intestine. EN can properly maintain the intestine mucosal immune barrier function, while PN weakens it.
