Hemodynamic Changes and NOS2/VEGF Expression in Patients with Portal Hypertension

Abstract:Objective To investigate the relationship between the bleeding caused by varicosis of the coronary vein and hemodynamic changes in portal system and determine the NOS2/VEGF expression in patients with portal hypertension. Methods A total of 43 patients were divided into 2 groups. Patients (n=26) in group A were without history of bleeding or bled just once and those in group B (n=17) bled twice or over 2 times. Doppler ultrasonography was used to determine the portal hemodynamic changes. The portal pressure was checked during operation. The expression of NOS2 and VEGF in coronary and portal vein was determined with immunohistochemistry. Results The PVD was 14.56±1.69 mm, the PVmax 19.47±5.35 cm/s, the PVBF 840.66±211.81 L/min and PVP 33.2±7.07 cmH 2O. There were no significant differences in PV max and PVP between the two groups. PVD and PVBF were much higher in group A than in group B. Coronary hepatoalfugal blood current happened in 65.1% of the patients. The spleen vein blood volume (SVBV) was 422.5±139.4 L/min. The ratio of mean SVBV to PVBF was 0.503. SVBV was lower in group B than in group A. The expression of NOS2 and VEGF was lower in group A than in group B. Both NOS2 and VEGF significantly expressed in the endothelium of the portal vein. Conclusions The high hemodynamic level in portal system is the elementary condition of the cause of upper gastric bleeding. The levels of PVBF and SVBV are obviously decreased in the patients bled for over 2 times. The effects of spleen's positive hyperemia are important reasons for portal hyper dynamics. The changes of vein structure are the most important or direct factors affecting the bleeding possibility of portal hypertension. The expression of NOS2 in the endothelium is related to the hyper hemodynamic of the portal system. The expression of VEGF is correlated to changes of the vein structure and may affect the bleeding possibility.
