Abstract:From the aspects of strengths,weaknesses,opportunities and threats,this paper analyzed external factors of the reorganizations and group building reform of public hospitals in Shanghai.Points made are as follows:Characteristics of individual hospitals should he respected instead of a generalized pattern for all;the role of the government to play in this regard should he guidance of an orderly competition,Strengthen Supervision,and promotion of the reform in property rights,These efforts will create a desirable environment for hospital groups;mid- and long-term goals for the reform should consolidate the independent legal entity status of such groups.

Abstract:Shanghai is facing three underlying problems in the health reform. First is that the relationship between government and market in the health sector is unclear, second is that the environment for the health settings , owners of all kinds to deliver medical care needs to be optimized, and third is that the system and mechanism of public hospitals needs to be improved. Targeting the above three problems, the paper proposes ideas and policy suggestions to deepen Shanghai health sector reform.
Study on the Patient Satisfaction and its Influencing Factors in Public Hospitals of Shanghai

Abstract:The satisfaction of inpatients and outpatients in Public Hospitals of Shanghai was analyzed after questionnaire survey of patients. The results showed that the patients were quite satisfied with medical care services in public hospitals of Shanghai and had higher satisfaction compared with previous year. But the quality of outpatient care services in tertiary specialty hospitals was relatively low. The authors suggested that outpatient service process should be improved, that privacy protection in medical care should be paid attention to, and that a hierarchical system for medical care should be set up in Shanghai.

Abstract:Based on the previous survey and data analyses,the principals,mechanism and scheme of governmental subsidies to public hospitals of one Shanghai district were suggested. Abstract:Objective To probe into reform outcomes of county-level public hospitals in the province,for the purpose of improving reform measures and ensuring validity and sustainability of the reform.Methods 38 general hospitals with ongoing reform since December 2012 were rounded up in the research.From these hospitals the performance indicators of January-June,2012 and January-June,2013 were collected and analyzed for comparison before and after the reform.Results The reform found a significant rise of medical services,an increase in the revenue of both outpatients and inpatients,and a proportional drop of drug sales(from 54.41％ to 49.30％); an increase of outpatient costs per visit (0.75％average annual increase); a drop of hospitalization fees per person(1.41％ average annual decrease); an increase of medical workers' income (6.04％ increase); and a budget surplus for the hospitals.Conclusion The comprehensive reform has reduced medicine cost burden for patients,yet its role in regulating physicians' prescription behavior and alleviating the general out-of-pocket burden of patients in general remains to be confirmed.

Abstract:With the intensification of aging in Shanghai, the hospitalization for elderly patients with chronicle diseases is arousing public concern increasingly.Based on the research of the situations on this issue in several second and third level hospitals, the main features of elderly people with chronicle diseases is analyzed, and a preliminary demand of the amount of hospital-bed for chronicle diseases in Shanghai is conducted, which will provide some evidences for helath reform and decision making on resources integration.

Abstract:Summaries the opinions of hospital directors who have attended the training course in the UK-China Partners in Health Innovation Programme organized by China Ministry of Health on hospital reform,including aspects of objectives of the reform,health system,healthcare financing,hospital payment and health service delivery in order to provide information for relevant health decision makers.The hospital reform not only includes the reform on hospital internal system and management,but also includes aspects related to whole social system reform.Thus,all relevant partners should work together.It should be the one of government's main tasks,not the sole task of health sectors.The governments should take main roles in the internal market resulting in a purchaser/provider split.

Abstract:Following an analysis of the problems and challenges of hospitals regulation in China and a comparative study of situations at home and abroad,this paper presented six policy proposals for the reform of public hospitals regulatory system in China.The policy suggestions included definite regulatory objectives,defined relationship between public hospitals and government,strengthening both economic regulation and social regulation, establishing multivariate regulatory system, introducing flexible regulatory methods and full utilization of the third-party impact,which met the requirements of public hospital in healthcare reform in China and propelled the reform forward.

Abstract:To fully understand the current situation of community health reform and its development in Shanghai in 2015,this paper sums up the comprehensive evaluation results of community health services in Shanghai from the level of districts and community health service institutions respectively with requirements of relevant national policy and local documents of Shanghai. In districts of Shanghai,the policy implementation,resource input and construction of health management system were running smoothly,while there was still room for improvement in the aspects of financial input,institutional layout,human resource input and application of informatization. In community health service institutions of Shanghai,the work of public health and family planning was progressing well,while efficiency - related indicators of primary medical care had a relatively low up - to- standard rate. Progress on comprehensive reform of community health services was at different stages in pilot communities, especially the separation between the implementation level and the policy - making level. Based on the comprehensive evaluation results,it is necessary to realize system building and resources allocation in five aspects of service system construction, comprehensive reform,human resources input,information system construction and patient experience in the community health reform in Shanghai in order to improve the quality of community health services.

Abstract:Objective To adjust the governmental compensation for public hospitals owned by one district of Shanghai.Methods Study data were collected from the statistical materials and questionnaire survey of all 17 public hospitals owned by one district of Shanghai.Through the interview of focus group,the opinion of government compensation schedule of hospital director and related government managers was understood.Results Based on data analysis and expert consultation,the objectives,principles,mechanism and scheme of governmental compensation for public hospitals owned by one district of Shanghai is made based on "zero pharmaceutical mark-up policy".Conclusion Overall,the governmental compensation for public hospitals owned by one district of Shanghai meets the requirements of further public hospital reform in China.This will facilitate the healthy and steady development of the public hospitals.

Abstract:Reforms in public hospitals are among the most important improvements in China’s health care system over the last two decades. However, the reforms that should be implemented in public hospitals are unclear. Thus, a feasible direction of reforms in Chinese public hospitals is suggested and reliable policy suggestions are provided for the government to reform public hospitals. The data used in this study were mainly derived from a qualitative study. Focus group discussions and in-depth interviews were conducted in Shanghai, Guangdong, and Gansu between May and December 2014. Government funding accounted for approximately eight percent of the total annual revenue of public hospitals in China, and the insufficient government subsidy considerably affects the operation mechanism of public hospitals. However, solely increasing this subsidy cannot address the inappropriate incentives of public hospitals in China. The most crucial step in setting the direction of reforms in public hospitals in China is transforming inappropriate incentives by implementing a new evaluation index system for directors and physicians in public hospitals.
