Link-pattern Neuro-Veno-fasciocutaneous Flap:Vascular Basis and Clinical Implications

Abstract:Objective: To propose a new type of flap ascularization (link-pattern flap) and discuss its clinical implications. Methods: Summary of authors' anatomic and clinical data and review of literature concerning the study of integument vascularization and neuro-veno-fasciocutaneous flap application in the limbs. Results: The perforators from the deep source artery form three layers of longitudeinal directional vascular plexuses (i.e.fascial, paraneural and perivenous) in the integument, Link-pattern flap's territory is made up of a series interlinking small individual perforators that are aligned one after another.Because the flap takes the advantages of the lower resistance chain-linked longitudeinally distributed arterial systems, a given arterial inflow pressure can be easily translated into high blood flow and resulted in a greater length-to-breadth survival ratio,even 5∶1. Conclusion: The concept of link-pattern flap can resolve the conflicts of clinical long flap survival (e.g.neuro-veno-fasciocutaneous flap) with no anatoical sizeable pedicle artery.Link-pattern flap can be considered as an intermediate one between random and axial flaps.
