Abstract:Objective To study the linkage disequilibrium and haplo views among single nucleotide polymorphism (SNP),which includes genotypes (hENT1-201A＞G,hCNT3 565G＞A,hCNT3 4T＞C,hCNT3 1647+376G＞C,CDA 79A＞ C,CDA-435C＞ T,dCK-1205C＞ T,dCK+ 9846A＞ G,DCTD-47T＞ C,RRM1-3 7C＞A and RRM1-756T＞ C) associated with gemcitabine metabolic pathways,to observe the relationship between polymorphism and the adverse reactions of gemcitabine.Methods A total of 123 cases of consecutive cancer patients using gemcitabine were enrolled.The venous peripheral blood of patients was collected.DNA was extracted by DNA extraction kit for genotype detection test.The genotypes of CDA-435C＞ T and hENT1-201A＞ G were successfully detected using LDR method,and the genotypes of remaining nine SNP sites were also successfully detected using SnaPshot method.Hardy-Weinberg equilibrium test was conducted by x2 test.Haploview software was used for the analysis of linkage disequilibrium,and the strongest interaction of SNP for haplotype analysis was selected.The correlation of SNP genotypes with adverse reactions were analyzed using Pearson's chi-squared test (x2) or Fisher for statistical analysis.Results All of SNP sites were in Hardy-Weinberg equilibrium.The dCK-1205C＞T and +9846A＞G,RRM1-756T＞C and-37C＞A were blocks of high linkage disequilibrium (D'=0.98 and D'=0.95).The hCNT3 1647,+ 376G＞ C and 565G＞A were blocks of completed linkage (D'=1).The GT in haplotypes dCK gene was major haplotype accounted for 62.50％,and the CG in hCNT3 haplotypes was major haplotype accounted for 44.70％,and the TC in RRM1 haplotypes was major haplotype accounted for 72.50％.In the study on influence of gene polymorphism on adverse reactions,the transporter gene polymorphism was significantly unrelated to severe hematological toxicity (P＞0.05).In metabolic enzyme gene polymorphism study,the incidence rate of severe neutrophilopenia in CDA 79A＞C mutation patients was significantly higher than that in wild type patients (33.33％ vs 16.13％,P＜0.05),and the CDA-435C＞T mutation may also be higher with severe Hb reduction risk (22.45％ vs 9.59％,P＜0.05).Other metabolic enzyme gene polymorphism was significantly irrelevant with severe hematologic toxicity (P＞0.05).In target gene polymorphism,the incidence of severe neutrophilopenia with RRM1-37C＞A mutation patients was lower than that in wildtype patients (21.31％ vs 24.19％,P＜0.05).The RRM1-756T＞C mutation patients had less severe thrombocytopenia decrease compared with wild type (0.00％ vs 4.84％,P＜0.05).In the haplotype study,RRM1 haplotype mutation patients had lower severe thrombocytopenia compared with wild type patients patients (0.00％ vs 4.84％,P＜0.05).The severe hematological toxicity was no correlation with gene haplotype polymorphism of dCK and hCNT3 (P＞0.05).Conclusion CDA 79A＞C and CDA-435C＞ T mutation will increase the risk of severe hematological toxicity,whereas RRM1-37C＞A and RRM1-756T＞ C mutation will reduce the risk of some severe hematoloqical adverse reactions.
