Clinical analysis of Graves' ophthalmopathy in the elderly

Abstract:Objective To summarize the manifestations and to analyze the contributing factors of Graves' ophthalmopathy(GO) in the elderly. Methods Elderly GO patients(67 cases) were collected from 1994 to 2001, and their clinical manifestations were analyzed. Their clinical features were compared with the young GO patients in hospitalization at the same time. Results It was demonstrated that GO in elderly was commonly bilateral. The serious deficiency of vision acuity was more common in the elderly than in the young patients(P0.001), but intraocular pression, intraorbital pression, and ocular protrusion in elderly GO patients were higher than that in young(P0.005). Hepatic dysfunction, thyroidism dysfunction, hypertention, hyperlipidemia, diabetes, or heart diseases were more frequently found in the elderly than in the young patients(P0.05). In the elderly GO patients, the Vth and VIth grade occurred more frequently in male than in female(P0.05). Patients complicated with hepatic dysfunction, thyroid dysfunction, hypertention, hyperlipidemia, diabetes, or heart diseases was more commonly seen in the grade V and VI patients than in the under grade Ⅳ patients(P0.05). Methyl prednisolone pulse therapy was prescribed in all 67 elderly GO patients and it was effective in 25 cases(37.3%), and it was not satisfactory in 42 patients who then were operated on orbital decompression. Three months after decompression, vision acuity was improved in 39 cases(92.9%), no progression in 3 cases (7.1%) . Conclusions GO occurres commonly in male elderly patients. Hepatic dysfunction, thyroid dysfunction, and some common diseases were found in most of the elderly patients. These complications may exert influences on the treatment and prognosis. Orbital decompression is an effective measure to the V and VI grades of GO in elderly patients.
