Enhanced Gallbladder Bile Coagulation Associated with Cholesterol Gallstone Formation

Abstract:AIM: To elucidate the mechanism of fibrin in cholesterol gallstone disease. METHODS: Hemostatic indices were systemically examined in the gallbladder bile obtained from 26 patients with gallstones and 17 without gallstones. RESULTS: Significant increases were found in tissue factor (TF) concentration( P 0.01), prothrombin fragment F_ 1+2 ( P 0.05), factor Ⅷ (FⅧ) antigen ( P 0.01), fibrinogen ( P 0.01), and antithronbin Ⅲ antigen (AT-Ⅲ: Ag) concentration ( P 0.01), and the ratio of AT-Ⅲ:Ag to AT-Ⅲ:Ac ( P 0.05)in patients with gallstones, compared with those without gallstones, but there was no significant difference in the antigen of tissue factor pathway inhibitor (TFPI) and the antigen of protein C (PC) between the two groups, although the stone patients exhibited a significant elevation of the ratio of TF to TFPI, compared to those without gallstones ( P 0.05). In addition, the activity of antithrombin Ⅲ (AT-Ⅲ:Ac) was much lower than that in controls ( P 0.05). CONCLUSIONS: The coagulatic activities in the bile of patients with gallstones was enhanced, but the anticoagulatic activities was not concomitantly enhanced. The enhancement of coagulatic activities was mainly initiated by tissue factor.
