Parathyroidectomy through Small Incision under Local Anesthesia: Another Approach for primary Hyperparathyroidism

Abstract:Objective:To assess the efficacy and safety of parathyroidectomy through small incision under local anesthesia(PSILA).Methods:Among600consecutive primary hyperparathyroidism patients,those identified to be solitary parathyroid adenoma and without contra-indications were chosen for PSILA.B-US and 99m Tc-sestamibi(MIBI)were used for preoperative localization,and parathyroid hormone levels were monitored during the operation.For patients with multiple lesions and encountering difficulty during operation,PSILA was converted to conventional exploration immediately.Results:A tota of310patients underwent PSILA.There were56males and254females,with an average age of(60.8±16.1)and an average history of(34.34±50.53)months.Preoperative serum calcium and PTH levels were(3.0±0.33)mmol/L and(228.7±360.1)ng/L.The adenomas in the PSILA group were(1.90±0.96)cm in diameter and weighed(2.19±4.44)g.Fifty-one cases were converted to conventional exploration,including22patients with multiple glandulear lesions,14cases of mis-localization,12difficult cases and3patients intolerant of local anesthesia.The mean operating time of PSILA was(32.68±6.96)min.Forty-nine patients were discharged2hours after PSILA,and the other210patients were discharged the next morning.The complicatcons observed were persistent hypercalcemia(2.87mmol/L)1case,transient recurrent laryngeal nerve palsy in2cases and hematoma,1case.The overall accuracy rate of B-US with MIBI was88.39%,and it was95.14%in solitary adenomas.Conclusions:PSILA is an efficacious and safe approach for the treatment of solitary parathyroid adenoma.The key points of this procedure are:mastering the indications in selecting the camdidates,and timely conversion to conventional surgery in difficult cases.
