Abstract:BACKGROUND:In light of the critical shortage of donor livers, xenogeneic sources offer the best alternative to human hepatocytes for the treatment of acute liver failure. This study investigated whether a combination of simian virus 40 large T antigen (SV40 LT) and human telomerase catalytic subunit (hTERT) genes could immortalize primary porcine hepatocytes that could reverse acute liver failure (ALF) in rats.METHODS:We cotransfected SV40 LT and hTERT genes into primary porcine hepatocytes to examine the features of the transfected cell lines. We characterized the potentially therapeutic effect of immortalized porcine hepatocytes in a rat model of ALF induced by 90% hepatectomy.RESULTS:An immortalized porcine hepatocyte cell line, HepLi, was expanded by >250 passages. HepLi cells maintained the defining characteristics of primary porcine hepatocytes, including porcine albumin secretion, urea production, and diazepam metabolism. Intrasplenic transplantation of HepLi cells significantly improved liver function, and significantly prolonging the survival of rats with ALF.CONCLUSIONS:Cotransfection of SV40 LT and hTERT immortalized primary porcine hepatocytes without tumorigenicity in vitro. The Immortalized porcine hepatocytes served as a potential cell resource for xenotransplantation.
Microencapsulated Porcine Hepatocyte Transplantation to Treat Acute Hepatic Failure of Rats Induced by Drug

Abstract:Objective To investigate the therapeutic effect of microencapsulated porcine hepatocytes transplanted intraperitoneally on toxic hepatic failure in rats and observe the survival rate and the change of hepatic function of the recipient rats. Methods The collagenase perfusing method was used to separate hepatocytes from pup pig liver. And then the hepatocytes were encapsulated by the alginate method and transplanted into abdominal cavity of D gal-induced acute hepatic failure (AHF) rats. The survival rate and hepatic function in the rats were determined. Results Hepatocytes transplanted intraperitoneally could improve greatly hepatic function of rats, and the ALT, AST and TB were all improved significantly (P0.05) as compared with NS injection group.The 1- and 2-week survival rate in microencapsulated porcine hepatocyte transplantation group was longer than in free hepatocyte transplantation group (78.6% vs 66.7 % , 42.9 % vs 25.0 % respevtively, P 0.01) . Conclusion Microencapsulated xenogeneic hepatocyte transplantation is effective in the treatment of AHF.

Abstract:The treatment of acute liver failure, a condition with high mortality, comprises optimal clinical care, and in severe cases liver transplantation. However, there are limitations in availability of organ donors. Hepatocyte transplantation is a promising alternative that could fill the medical need, in particular as the bridge to liver transplantation. Encapsulated porcine hepatocytes represent an unlimited source that could function as a bioreactor requiring minimal immunosuppression. Besides patients with acute liver failure, patients with alcoholic hepatitis who are unresponsive to a short course of corticosteroids are a target for hepatocyte transplantation. In this review we present an overview of the innate immune barriers in hepatocyte xenotransplantation, including the role of complement and natural antibodies; the role of phagocytic cells and ligands like CD47 in the regulation of phagocytic cells; and the role of Natural Killer cells. We present also some illustrations of physiological species incompatibilities in hepatocyte xenotransplantation, such as incompatibilities in the coagulation system. An overview of the methodology for cell microencapsulation is presented, followed by proof-of-concept studies in rodent and nonhuman primate models of fulminant liver failure: these studies document the efficacy of microencapsulated porcine hepatocytes which warrants progress towards clinical application. Lastly, we present an outline of a provisional clinical trial, that upon completion of preclinical work could start within the upcoming 2-3 years.
THE SHORT-TERM EFFICACY AND SURGICAL EXPERIENCE OF ORTHOTOPIC LIVER TRANSPLANTAION FOR PORCINE ACUTE HEPATIC FAILURE

Abstract:Objective To investigate the short-trem efficacy and surgical experience of orthotopic liver transplantation for porcine acute hepatic failure.Methods Porcine acute liver failure was induced by intraporta administration of 0.5g/kg D-galactosamine and 1μg/kg Lipopolysaccharide.Pigs were randomly divided into two groups:group A was without orthotopic liver transplantation(n=6),group B received orthotopic liver transplantation(n=5).The clinical manifestations,biochemical,pathological changes and 7-day survival rate of these pigs were investigated.Results In group A,the level of aspartate transaminase,total bilirubin,ammonia and prothrombin time reached to peak at 48h,and the 7-day survival rate was only 17%.The postmortem liver specimens showed a severe hepatocellular necrosis with hemorrhage.In group B,the level of aspartate transaminasw,total bilirubin,ammonia and prothrombin time returned near to normal level on the 7~(th) postoperative day(POD)in all recipients,and the 7-day survival rate was 100%.The graft liver specimens on POD8 revealed normal liver architecture,mild cloudy swelling degeneration of hepatocytes and minimal inflammatory infiltration in the periportal tracts.Conclusions Orthotopic liver transplantation is an effective method in the treatment of porcine acute hepatic failure.Preoperative correction of coagulation disturbance,utilization of venous-venous bypass and reliable methods of vascular reconstruction are the keys for successful operation.
Transplantation of Monocyte-Derived Hepatocyte-Like Cells (neoheps) Improves Survival in A Model of Acute Liver Failure

Abstract:Objective:Investigation of the efficacy of implantation of monocyte-derived hepatocyte-like cells (NeoHeps) in acute liver failure. Summary Background Data:Extended liver resection or split liver transplantation is still associated with high morbidity and mortality because of postoperative liver insufficiency. In view of liver support systems, implantation of isolated hepatocytes or hepatocyte-like cells such as NeoHeps is increasingly under discussion. Methods:Twenty-four hours before subtotal hepatectomy, cells of different origin [A: human mononuclear cells (24 × 106); B: NeoHeps (16 × 106); C: NeoHeps (24 × 106); D: rat hepatocytes (24 × 106)] were injected into the spleen of Wistar rats. After an observation period of 5 days, animal survival, postoperative weight, and signs of encephalopathy were recorded. At the end of the observation period, blood was collected for laboratory analysis. Results:Transplantation of both rat hepatocytes and NeoHeps significantly improved animal survival when compared with control animals (group A: 21%), reaching 72% in group D (P = 0.001), 50% in group C (P = 0.04), and 36% in group B (P = 0.22). Moreover, animals in these groups postoperatively experienced less frequently signs of encephalopathy, as well as earlier weight increase when compared with group A. Discussion:Hepatocyte transplantation is a practicable and successful treatment option in case of liver insufficiency because implantation of NeoHeps or primary rat hepatocytes had an improving effect on survival. The promising data of the present study warrants further analysis to elucidate the role of NeoHeps in treatment of acute postoperative liver failure to a greater extent.

Abstract:Various liver diseases result in liver failure, and liver transplantation as a definite treatment is limited by the shortage of organs available for transplantation. The use of isolated primary hepatocytes in cell-based therapies including hepatocyte transplantation, tissue engineering liver transplantation, and bioartificial liver support systems has gained increasing interest during the past years. Human hepatocytes are the preferred source of cells. Aside from the organ shortage, the isolation of human liver cells is usually limited by obtaining a sufficient quantity of high-quality, metabolically active cells. Furthermore, livers from which hepatocytes are typically harvested are not suitable for transplantation, with the variability in quantity and quality. Porcine hepatocytes, on the other hand, have the ability to perform complex biological functions and show modifiable behavior. Primary porcine hepatocytes are currently widely used in the investigation of drug metabolism, hepatotoxicity, protein biosynthesis, and gene expression. Primary hepatocytes do not proliferate in vitro and are sensitive to freeze-thaw damage in cryopreservation and thus need to be freshly isolated for each experiment. Consequently, the methods of porcine hepatocyte isolation are being actively sought after. Our laboratories have been involved in various applications of liver cells, and we have long-lasting experiences in liver cell isolation and their application in R&D. We here summarize the present protocol of our laboratories for primary hepatocyte isolation from pig and their liver tissue engineering for xenotransplantation.
EFFECT OF DEEP HYPOTHERMIA REDUCING IMMUNOLOGIC REJECTION REACTION ON HEPATOCYTE TRANSPLANTATION

Abstract:Objective: The experiment aimed at studying effect of deep hypothermia reducing immunologic rejection on hepatocyte transplantation. Methods:Dissolution rate of porcine hepatocytes was assayed by antibodydependent cellular cytotoxicity (ADCC). The conformation of their membrane proteins was calculated from their circular dichroism spectra. The fresh or cryopreserved pig hepatocytes were respectively transplanted into the abdominal cavities of Wistar rats with acute hepatic failure (AHF), and allogeneic transplantation of SD rats' hepatocytes cryopreserved was performed. Then, survival rates of the rats were observed. Results:The ADCC showed that amount of the fresh hepatocytes survived was 0.24×105 and that of the frozen cells survived was 0.68×105. The αhelix content in the membrane proteins of fresh cells was 47.5%. After the cells were frozen within liquid nitrogen, the content increased to 56.5%. The survival rate of rats with AHF was 33.3%, and that in the fresh hepatocyte transplant group was 66.7%. The survival rate of rats in the cryopreserved cell transplant group increased significantly to 94.1%. But, there was no significant difference between the xenotransplant group of the cryopreserved pig hepatocytes and the allogeneic transplant group of the SD rats' hepatocytes cryopreserved, in which the survival rate of the SD rats was 71.4%. Conclusion:The results suggested that the deep hypothermia is able to raise the content of the αhelix in the membrane proteins and to reduce the immunoreactivity of transplant cells. The transplantation of the cryopreserved hepatocytes is able to raise the cure rate of the AHF.

The apoptosis of transplanted hepatocytes during porcine hepatocytes xenotransplantation

Abstract:Objective To evaluate the apoptosis of transplanted hepatocytes during porcine hepatocytes intraperitoneal xenotransplantation. Methods Recirculating collagenase perfusion method was used to isolate hepatocytes of Chinese experimentmini-pigs and the isolated porcine hepocytes suspensions were cultured for 24h. 2ml free porcine hepatocytes suspensions (5.0×107cells) were transplanted into abdominal cavity of SD rats at 48h after D-gal introperitonear injection (1.2g/kg). The two-week survival rate and the hepatic functions were examined. The pathological changes of transplanted hepatocytes were observed. The rate of apoptosis, viability and necrosis of the transplanted hepatocytes was measured by Annexin V-FITC apoptosis detecting kit and flow cytometry. ResultsThere was a significant difference in two-week survival rate between acute liver failure(ALF) group (56.25%) and transplantation group (18.75%) (P<0.05). The hepatic functions were damaged significantly and recovered after hepatocyte transplantation. The survival time of the transplanted porcine hepatocytes was from 3 to 5 days after D-gal injection. The apoptosis of hepatocytes occurred after culture for 24h in vitro. Compared with culture of 24h, the apoptosis rate of the transplanted hepatocytes increased and the viability of the transplanted hepatocytes decreased significantly (P<0.05). The necrosis rate of the transplanted hepatocytes changed unsignificantly (P>0.05). Compared with transplantation for 24h, the rate of apoptosis increased and the viability of the transplanted hepatocytes decreased continuously at 48h after transplantation (P<0.05). The necrosis rate of the transplanted hepatocytes changed unsignificantly (P>0.05). The apoptosis rate of the transplanted hepatocytes decreased at 72h after transplantation. Compared with transplantation for 48h, the viability of the transplanted hepatocytes changed unsignificantly (P>0.05) and the necrosis rate of the transplanted hepatocytes increased significantly (P<0.05). Conclusion The transplanted hepatocytes can only survive for 3 to 5 days by using free porcine hepatocytes intraperitoneal xenotransplantation. The apoptosis is the main way in early immune rejection of the transplanted hepatocytes,and necrosis three days later.
The Therapeutic Effect of Porcine Fepatocyte Transplantation Using Polylactic Acid-O-carboxymethylated Chitosan Nanoparticles on Rats with Acute Liver Failure

Abstract:Objective:To evaluate the therapeutic effect of intraperitoneal transplantation of porcine hepatocytes cultured with polylactic acid-O-carboxymethylated chitosan(PLA-O-CMC)nanoparticles and embedded in collagen gel on acute liver failure(ALF)rats induced by D-galactosamine(D-Gal).Methods:At 48h after D-gal introperitoneal injection,free porcine hepatocytes suspension(5.0×107 hepatocytes,groupⅠ),porcine hepatocytes embedded in collagen gel(5.0×107 hepatocytes,group Ⅱ)and porcine hepatocytes cultured with PLA-O-CMC nanoparticles and embedded in collagen gel(5.0×107 hepatocytes,group Ⅲ)were transplanted into peritoneal cavity of SD rats,respectively.RPMI 1640 medium was injected into peritoneal cavity of SD rats as control group(group Ⅳ).The 14d survival rate,liver functions such as ALB,ALT,TB,NH3 level and the pathological change of transplanted hepatocytes were observed.Results:The 14d survival rate in groups I,II,III and IV was 56.25%,62.5%,75% and 18.75%,respectively.It was higher in groups I,II,III than group IV(P0.05).The results showed that liver functions were quickly restored in groupsⅠ,Ⅱand Ⅲ compared with those in group Ⅳ during 5 days post-transplantation.The improvement of the liver functions in group Ⅲ was the most quick.The second was group Ⅱand the third was groupⅠ.The slowest one was group Ⅳ in all groups.The survival time of transplanted porcine hepatocytes was longest and the level of infiltration of inflammatory cells was lightest in group Ⅲ.Conclusion:Intraperitoneal transplantation of porcine hepatocytes cultured with PLA-O-CMC nanoparticles and embedded in collagen gel can increase the survival rate of ALF rats induced by D-Gal and improve liver functions of the damaged recipient significantly.PLA-O-CMC nanoparticles have the better bio-compatibility in hepatocytes xenotransplantation and can prolong the survival time of the transplanted hepatovcytes.
[Treatment of Acute Liver Injury by Intrasplenic Transplantation of Hepatic Stem Cells Combined with Heparin in Rats].

Abstract:OBJECTIVE:To determine the treatment effects of transplanted hepatic progenitor cells (WB-F344 cells) combined with heparin on the acute liver injury in SD rats.METHODS:A total of 2*10(7) hepatic stem cells (WB-F344) infected with GFP lentivirus and 8 μL heparin were transplanted through the spleen in SD rats with acute liver injury, which was induced by an intraperitoneal injection of CCl4. The liver and spleen tissues underwent fluorescence examination 1 day after the transplantation. The liver functions were tested, and the liver tissues were histopathologically examined on the 3rd, 7th, 14th, and 28th day of the cell transplantation.RESULTS:The transfected WB-F344 cells expressed GFP 3 days after the lentivirus infection and were found in the rat liver 1 day after the WB-F344 transplantation. The liver function and histopathological recovery of the liver tissues in the group of WB-F344 transplantation were better than those of the control group (P<0.05).CONCLUSION:Transplantation of hepatic stem cells combined with heparin can promote the liver recovery in rats with acute liver injury induced by CCl4.
Cryopreservation and Gel Collagen Culture of Porcine Hepatocytes

Abstract:AIM: To study the method of cryopreserving porcine hepatocytes and gel collagen culture measure after its cryopreservation.METHODS: Hepatocytes, isolated from Chinese experimental suckling mini-pigs by two-step perfusion with collagenase using an extra corporeal perfusion apparatus, were cryopreserved with 50 mL/L to 200 mL/L DMSO in liquid nitrogen for 4 mo, then thawed and seeded in 1 or between 2 layers of gel collagen. The expression of porcine albumin message RNA, cellular morphology and content of aspartate aminotransferase (AST) and urea nitrogen (UN) were examined during culture in gel.RESULTS: Viability of 150 mL/L DMSO group thawed hepatocytes was (83 +/- 4)%,but after purification, its viability was (90 +/- 5)%, attachment efficiency was (86 +/- 7)%, the viability of thawed hepatocytes was near to fresh cells. When the thawed hepatocytes were cultivated in gel collagen with culture medium adding epidermal growth factor, the hepatocytes grew in various administrative levels in mixed collagen gel, and bunchy in the sandwich configuration cultures. For up to 10 days' culture, the typical cellular morphological characteristics of cultivated hepatocytes could be observed. The leakage of AST was lower during culture in gel than that in common culture. At the same time, the UN synthesized by cells cultivated in mixed gel collagen was higher than that in other groups.CONCLUSION: Storage in liquid nitrogen can long keep hepatocytes' activities, the concentration of 150 mL/L DMSO is fit for porcine hepatocytes' cryopreservation. Thawed hepatocytes can be cultivated with collagenous matrix, which provides an environment that more closely resembles that in vivo and maintain the expression of certain liver-specific function of hepatocytes.
Treatment of Acute Hepatic Failure in Mice by Transplantation of Mixed Microencapsulation of Rat Hepatocytes and Transgenic Human Fetal Liver Stromal Cells.

Abstract:Microencapsulation-mediated cell therapy overcomes the immune incompatibility between donor and recipient in transplantation. The aim of this study was to investigate the effects of transplantation of microcapsules containing a mixture of rat hepatocytes and human fetal liver stromal cells (hFLSCs), engineered to produce basic fibroblast growth factor (bFGF), on acute liver failure (ALF) in mice. In vitro experiments showed that different combinations of microencapsulated rat's hepatocytes and stromal cells survive, grow, and function better in three-dimensional conditions. The metabolic activity of rat hepatocytes co-microencapsulated with hFLSCs, particularly when engineered to produce bFGF (FLSCs/bFGF), is significantly higher than that of microcapsules with rat hepatocytes alone. Intraperitoneal transplantation of the encapsulated hepatocytes with FLSCs/bFGF increased the survival rate and improved liver function of an ALF mouse model induced by a 70% partial hepatectomy in BALB/C mice. Moreover, dramatic liver regeneration was observed 2 days after transplantation in the group that received intraperitoneal transplantations of encapsulated hepatocytes with FLSCs/bFGF. Therefore, transplantation of encapsulated hepatocytes and hFLSCs/bFGF may be a promising strategy to treat ALF or related liver diseases.
Transplantation of Co-Microencapsulated Hepatocytes and HUVECs for Treatment of Fulminant Hepatic Failure

Abstract:Purpose: Microencapsulated hepatocytes might solve immunological rejection, broadening a new perspective for the treatment of fulminant hepatic failure (FHF). However, the transplantation of microcapsulated hepatocytes is limited by low cell viability Nevertheless, the co-microencapsulation of hepatocytes and human umbilical vein endothelial cells (HUVECs) may make the treatment of FHF more promising. Methods: We prepared the microcapsules using the high-voltage electrostatic droplet spray method, transplanted the empty microcapsules, isolated hepatocytes, microcapsulated hepatocytes, and co-microencapsulated hepatocytes and HUVEC intraperitoneally into rat models of FHF induced by D-aminogalactose (D-gal). After 1, 3, and 7 days, and 2, 3, and 4 weeks posttransplantation, we calculated the mortality and assessed alanine aminotransferase (ALT), aspartate aminotransferase (AST), and albumin (ALB) levels in the serum of the model; evaluated the integrality and recovery of microcapsules; and stained with hematoxylin and eosin (H&E) the recovered microcapsules as well as the liver of the FHF rats. Results: Hepatocyte-specific functions, including the levels of ALT, AST, and ALB in the serum of the co-microencapsulation group, were significantly better than those in the other groups (p<0.05) from 2 to 4 weeks after transplantation. Moreover, cotransplantation of the microencapsulated hepatocytes and HUVECs decreased the mortality rate of the FHF rats. The recovered microcapsules were intact, and recovery was up to 90%. H&E staining showed that the microencapsulated cells were still alive, and the liver tissues had started to recover after 4 weeks posttransplantation. Conclusion: The microcapsules have good biocompatibility and immunoprotection to protect the hepatocytes from immunological rejection. Cotransplantation of the microencapsulated hepatocytes and HUVECs could decrease mortality rates and improve liver function in FHF.
Establishment and Modification of Normothermic Machine Perfusion System for Porcine Liver

Abstract:Objective To investigate the operability,security and availability of an improved normothermic machine perfusion system in the process of extracorporal porcine liver preservation.Methods Livers from Chinese male Landraces were perfused using the improved normothermic machine perfusion system for 6 hours.Haemodynamics,blood gas,biochemical markers and histology were detected.Results After 6-hour normothermic machine perfusion,the flow of hepatic artery and portal vein were (293 ± 23) ml/min and 885 ± 65 ml/min,respectively.The hepatic arterial pressure was (66.9 ± 3.6) mmHg and the portal pressure was (6.1 ±0.6) mmHg.Bile generation was (10.5 ± 1.1) ml/h.pH was 7.5 ±0.1.Glucose level decreased to (6.9 ± 0.8) mmol/L.ALT was (47.4 ± 9.1) U/L and AST was (765.1 ± 137.6)U/L.Histological architecture remained intact without any unrecoverable injury and the Suzuki score was 3.2 ± 0.2 after perfusion.Conclusions The physiological condition of the extracorporal porcine livers can be maintained by the improved normothermic machine perfusion system with excellent operability and security.This system can provide a promising platform for repairing and resuscitating marginal livers,which could further expand the donor pool.

Abstract:Hepatocyte transplantation may be a viable alternative treatment to liver transplantation for acute/chronic liver failure and metabolic liver disorders.Hepatocyte transplantation is an effective treatment to support liver function around liver transplantation due to its relatively easy manipulation and mild wound.In recent two decades,hepatocyte transplantation have been applied in clinical treatment and showed some effect in acute/chronic liver failure and metabolic liver disorders.Here,we sum up the status of clinical hepatocyte transplantation,discuss its value in clinical application and some challenges need to resolve.
Evaluation of Small-for-size Liver Transplantation and Auxiliary Partial Orthotopic Liver Transplantation with Small-for-size Graft for Porcine Acute Hepatic Failure

Abstract:Objective To evaluate the short-term efficacy of small-for-size liver transplantation and auxiliary partial orthotopic liver transplantation(APOLT) with small-for-size graft for porcine acute hepatic failure. MethodsFifteen healthy female domesticated pigs received liver transplantaion 24 h after being induced acute hepatic failure by intraportally administration of 0.5 g/kg D-Gal plus 1 μg/kg LPS.Recipients were randomly divided into three groups: group A,60% APOLT,n=5;groupB,30% small-forsize liver transplantaiton,n=5;group C,30% small-for-size APOLT,n=5.The portal vein pressure of the recipients was measured at laparatomy,immediately after splenectomy and 30 min after reperfusion.The clinical manifestations,biochemical and pathological changes and one-week survival rate were observed. Results One-week survival rate was 100%,20% and 80%,respectively.Group A and C demonstrated significantly better survival than group B(P0.05).Serum aspartate transaminase,lactate,total bilirubin and ammonia in group A and C were much lower than those of group B on the second day after operation.The portal pressure after reperfusion in group B was significantly higher within 30 min than those in group A and C.After reperfusion,group B showed severe sinusoidal congestion,collapse of the space of Disse,vacuolar changes in cytoplasm of hepatocytes and rounded,detached sinusoidal cells. Conclusion In acute hepatic failure,APOLT can significantly increase the short-term survival of small-for-size graft.This function may be related to the reduction of portal pressure of the graft.
Experimental study in the treatment of acute liver failure with intraperitoneal microencapsulated hepatocytes in rats

Abstract:The aim of this study was to evaluate the feasibility of this method by transplanting the microencapsulated hepatocytes intraperitoneally, as while to examine the function of the bio-membrane. All the model rats of acute liver failure were induced by D-galactosamine and were divided into 4 groups at random:Ⅰ. NS group ( n =15);Ⅱ. free capsule group ( n =15);Ⅲ. HGF group( n =15); Ⅳ. microencapsulated hepatocyte group. Two weeks later, observed the survival rate, and changes of liver biochemical markers; collected the microcapsules from the survival rats and observed the histological changes and its function. The survival rate of group Ⅳ was higher than that of Ⅲ?Ⅱ?Ⅰgroup(respectively 66.7%, 33.3%. 20.0%,13.3%,20.0%, P 0.05). ALB,TBIL, PT of group Ⅳ had improved markedly than other groups from the time 24 hours after transplantation. Two weeks later, the microcapsules collected from the survival rats remained smooth and intact, the hepatocytes inside remained alive partially. [Conclusions] Microencapsulated hepatocyte transplantation can improve the survival rate of acute liver failure rats, also can improve the liver biochemical function as well. During the course of transplantation, microcapsules remained stable perfectly, and has showed no inpacts on the body, which can prevent the hepatocyte from the immune responses and can also help for their function better.

Abstract:BACKGROUND:Adipose-derived stem cells(ADSCs) are particularly attractive in future clinical applications of stem cell-based therapy for acute-on-chronic liver failure(ACLF) This study was undertaken to evaluate the therapeutic potential of ADSCs on ACLF.METHODS:ADSCs isolated from porcine fat tissue were expanded and labeled with BrdU.Rabbit models of ACLF were created by administration of D-Gal following CCl 4-induced cirrhosis.One day after administration of D-Gal,rabbits of the ACLF/ADSCs group(n=15) were received ADSCs transplantation,while those in the ACLF/saline group(n=15) were treated with the same volume of saline.Biochemical parameters and histomorphological scoring were evaluated;the distribution and characteristics of transplanted ADSCs as well as the pathology of the liver were examined.RESULTS:ADSCs transplantation improved the survival rate and the liver function of rabbits with ACLF.Biochemical parameters of the ACLF/ADSCs group were improved compared with those of the ACLF/saline group,and histomorphological scoring of the ACLF/ADSCs group was significantly lower than that of the ACLF/saline group.ADSCs were identified in the periportal region of the liver after cell transplantation.CONCLUSION:Xenogenic ADSCs have therapeutic efficacy in the ACLF rabbit model.

Abstract:A critical element restricting the application of liver transplantation is the shortage of human deceased donor organs. Xenotransplantation using pig organs might be a solution to this shortage. Although the problems that still require resolution include the immunologic barrier, the potential risk of transferring infectious agents with the transplanted organ, and uncertainty about whether the transplanted organ will function satisfactorily in the human environment, recent progress in the genetic manipulation of pigs has led to the prospect that clinical xenografting, at least as a bridge to allotransplantation, may be possible in the foreseeable future. Experience with clinical auxiliary and orthotopic liver xenotransplantation and experimental liver xenotransplantation in nonhuman primate and other large animal models is reviewed, and the remaining immunologic problems are discussed. Evidence suggests that, in patients with hepatic failure, the pig liver may be less susceptible to antibody-mediated injury than other pig organs, such as the heart or kidney. Pig Kupffer cells and other macrophages will recognize and phagocytose primate red blood cells, but this problem should be overcome by pretransplant depletion of macrophages from the organ-source pig. From the evidence currently available, it does not seem unduly optimistic to anticipate that a liver from an alpha1,3-galactosyltransferase gene-knockout pig would survive at least long enough to function as a successful bridge to allotransplantation.
