Effect of the Air Pollution on Acute Exacerbation of Copd Patients in Shanghai

Abstract:SESSION TITLE: COPD SESSION TYPE: Original Investigation Slide PRESENTED ON: Sunday, April 17, 2016 at 09:45 AM - 11:15 AM PURPOSE: Many epidemiologic studies have documented variable relationships between air pollution (particulate matter (PM) and NO2) and chronic obstructive pulmonary disease (COPD) exacerbation and mortality. The aim of our study is to determine the effect of PM2.5/NO2 on acute exacerbation of COPD (AECOPD) and provide clinical evidence for preventing AECOPD in different ambient air quality. METHODS: The concentrations of PM2.5 and NO2 were measured by real-time monitor by Bureau of Meteorology. (1) 101 stable COPD patients were examined. Self-evaluations of symptoms were recorded by the patients in the diary cards. Acute exacerbations of COPD from November 2010 to August 2011 were also recorded. (2) 80 AECOPD patients were examined from January 2015 to May 2015. Their living environmental factors were collected. RESULTS: (1) The worse the air pollution, the higher the exacerbations (PM2.5:r=0.419, p<0.05; NO2:r=0.523, p<0.05). The average concentration of PM2.5 before 7 days of AECOPD was mostly among >75μg/m3, whereas the average concentration of NO2 was >40μg/m3. (2) Patients living less than 100m from a busy road had a significantly increase exacerbations than for those living farther away (p<0.05). CONCLUSIONS: Exposure to PM2.5/NO2 and living near a major road might increase the risk of developing AECOPD. The concentration of PM2.5 >75μg/m3 and NO2 >40μg/m3 might play an important role in warning AECOPD in Shanghai. CLINICAL IMPLICATIONS: PM2.5/NO2 might play an important role in warning AECOPD.
