Abstract:Objective:To evaluate the effect and safety of endoscopic metal biliary endoprosthesis(EMBE) in management of malignant biliary obstruction.Methods:Forty-two patients with unresectable malignant biliary obstruction were treated with EMBE.The level of serum bilirubin,postoperative complications,survival and the unobstructed time of biliary stents were observed.Results: All 42 patients with metal biliary endoprosthesis were placed successfully.The success rate of EMBE was 100.0%.Serum bilirubin in 30 patients(71.4%) in two weeks after the treatment recuperated normal level;12 cases(28.6%) were significantly decreased.The cholangitis occurred in 2 cases(4.8%),and the mild acute pancreatitis occurred in 3 cases(7.1%).The average unobstructed time of biliary stents was 9.6 months.The survival rates of six months and one year were 64.3% and 19.0%,respectively.Conclusion:The study showed that the therapy of endoscopic retrograde biliary drainage was effective and safety for malignant biliary obstruction.

Abstract:Objective To evaluate the efficiency of percutaneous drainage and stenting for treatment of malignant obstructive jaundice.Methods Twenty-eight patients with malignant obstructive jaundice were treated with drainage and stenting by the way of percutaneous transhepatic insertion.Results Twenty-seven of the 28 patients were performed successfully.Only one patient,due to the guide wire couldn't go straight through the obstructive position,was treated by external drainage for a week before inserting a metal stent.Thirty-one stents were inserted into the 28 patients.One patient suffered little pleural effusion after operation.Two cases had to maintain their drainage catheter because of much bile extra-drainage.There was not any other complications occured.Conclusion Percutaneous drainage and stenting is micro-invasive and effective in the treatment of malignant obstructive jaundice.The patients had a good tolerance and fewer complications.The simple procedure for resolving biliary obstruction could effectively improve the patient's quality of life.
[Long-term retrospective analysis of metallic stents in malignant biliary obstruction]

Abstract:Objective: To analyze and evaluate of the long-term follow-up data of 152 patients with inoperable malignant biliary obstruction who underwent percutaneous placement of metallic stents. Methods: From 2000 through 2006, 152 consecutive patients with malignant biliary obstruction were treated with PTCD or placement of metallic stents. The causes of obstruction were bile duct carcinoma (n=35)ìpancreatic carcinoma (n=32), hepatocellular carcinoma and metastatic (n=61), lymphadenopathy in the hepatoduodenal ligament (n=24). We used 86 stents: 31 Cook Zilver Stentsì25 Sinus Superflex Stentsì12 Wall Stentsì3 Sinus-s Iliaca Stentsì3 ZA-S Bliary Design Stentsì5 Smart Stents and 7 other stents. Every three months we followed up all patients except those who died. Patient survival and stent patency rates were estimated by life-table analysis. The Kaplan-Meier method (log-rank test) was used to compare the patency and survival rates among different groups. And the COX regression model was established to compare the relative risks. Results: The overall median length of patency of all stents was 314 days. The median length of survival for the entire patient group was 215 days. The survival rate was 79.1%ì51.7% and 26.8% after 3, 6, and 9 months, respectively. Six patients developed recurrent jaundice or cholangitis. The patients with hepatic hilar obstruction group indicated worse prognosis (P< or =0.05). Conclusion: Metallic stents showed a favorable patency rate with regard to patient survival. In patients with hilar obstruction, the clinical prognosis was worse than that in patients with common bile duct (CBD) obstruction. We believe that the placement of metallic stents is the procedure of choice for palliation of malignant biliary obstruction.

Abstract:【Objective】To explore the methods of implanting biliary metallic stent via endoscopic retrograde cholangiopancreatography (ERCP) or percutaneous transhepatic cholangiography (PTC),then implanting duodenal stent,to evaluate the efficacy of palliative treatment of malignant biliary and duodenal obstruction with metallic stents. 【Methods】Of the 16 patients with malignant biliary and duodenal obstruction,11 biliary metallic stents were implanted by ERCP,4 biliary metallic stents were implanted by PTC,16 duodenal metallic stents were implanted through the mouth,1 outer draining was done. 【Results】14 patients were performed simultaneously in biliary and duodenal stents and 4 patients biliary stents were implanted by PTC,in another patient duodenal stent was placed after biliary stenting when duodenal obstruction symptoms had developed two months later. The outer biliary draining was performed in 1 patient because the guidewire could not advance through the obstruction. During follow-up,all patients except one were able to take semi-liquid or solid food,the jaundice was alleviated,and the result was satisfactory,no major complication occurred in all cases.【Conclusion】The implantation of metallic stent by endoscopic and interventional procedure is a simple and effective method which may provide a palliative treatment of malignant biliary and duodenal obstruction for patients who are poor candidates for surgery.
Comparison of endoscopic bilateral metal stent drainage with plastic stents in the palliation of unresectable hilar biliary malignant strictures: Large multicenter study

Abstract:Background Endoscopic stenting to manage malignant hilar biliary obstruction has no consensus regarding the optimal stenting strategy. In this multicenter study, we compared transpapillary parallel-style bilateral metal stenting with bilateral plastic stenting, and evaluated short- and long-term outcomes. Methods We recruited 262 consecutive patients (Bismuth classification types II-IV) who underwent either bilateral metal or plastic stenting as primary therapy at four tertiary centers. To overcome selection bias, we performed 1:1 propensity score matching. Our primary outcome was overall survival. Results After propensity score matching, each group comprised 96 patients, with no significant differences in any baseline characteristics. The median survival was significantly longer in the metal stenting group than in the plastic stenting group (7.2 months [95% CI 6.0-8.5] vs. 4.1 months [95% CI 2.9-5.3]; P = 0.015). The clinical success rates were significantly higher in the metal stenting group than in the plastic stenting group (99.0% vs. 71.9%, respectively; P < 0.001), and lower post-procedure cholangitis incidence (7.3% vs. 26.0%; P < 0.001), longer median symptom-free stent patency (9.2 months [95% CI 7.6-10.6] vs. 4.8 months [95% CI 4.2-5.3]; P < 0.001), and fewer total interventions (1.3 +/- 0.6 vs. 2.0 +/- 1.4; P < 0.001). In multivariate Cox analysis of the overall survival, metal stenting (HR 0.589, P = 0.002), hilar cholangiocarcinoma (HR 0.419, P = 0.009), and adjuvant treatment (HR 0.596, P = 0.006) were independent predictors of death. Conclusions Endoscopic therapy using bilateral metal stenting is superior to bilateral plastic stenting, with prolonged overall survival, higher clinical success, and longer stent patency in patients with advanced hilar biliary malignancies.
Percutaneous Transpapillary Biliary Stent Placement for Distal Malignant Biliary Obstruction: Outcomes and Survival Analysis

Abstract:BACKGROUND/AIMS:For distal malignant biliary obstruction (MBO), a percutaneous metal stent is usually inserted by the transpapillary method. However, stent-related complications and recurrent biliary obstruction following transpapillary stent placement are concerns, and survival analysis of patients with distal MBO has rarely been done.MATERIALS AND METHODS:From January 2012 to March 2016, 104 patients underwent transpapillary uncovered metal stent placement for distal MBO at our institution. Clinical success, complications, recurrent biliary obstruction rates, and predictors of survival were analyzed.RESULTS:Of the total 104 patients, clinical success after stent insertion was achieved in 93 patients (90.3%). Major complications were observed in 24 patients (23.1%), which were as follows: cholangitis in 19 patients; pancreatitis in four patients; and biloma in one patient. Recurrent biliary occlusion was observed in 28 patients (26.9%). The median overall survival period was 162 days. The 3-, 6-, and 12-month overall survival rates after stent insertion were 64.4%, 41.3%, and 10.6%, respectively. Results of multivariate analysis indicated that metastatic carcinoma compared with ampullary carcinoma (HR=3.82; 95% CI, 1.30-11.24; p=0.015) and longer biliary stricture (HR=1.04; 95% CI, 1.02-1.06; p<0.001) were independent risk factors for worse survival after metal stent insertion.CONCLUSION:Transpapillary stent placement was found to be effective with acceptable complication rates for treating distal MBO. Primary tumor and length of biliary stricture were found to be statistically significant independent prognostic factors for survival.

Abstract:Objective:To investigate the effect of percutaneous transhepatic biliary stent placement for malignant bile duct obstruction.Methods:Total of 92 patients with malignant bile duct obstruction were treated with percutaneous transhepatic biliary stent placement under DSA.Results:In 92 cases,91 cases' stents were implanted successfully,and double stents were implanted in 12 cases.The sents expand finely and draining smoothly,serum bilirubin decreased obviously,2 cases died after the operation.3 hemobilia cases,6 hydroperitoneum percolated cases,2 bile percolated cases,1 sents shifting case,9 cholangitis cases,and 11 biliary tract blockage cases happened.Conclusion:It is a safe and effective method for inoperable malignant bile duct obstruction cases,that percutaneous transhepatic biliary stent placement has little wound,and can improve the living quality and prolong living time.
Palliative treatment for obstructive jaundice caused by unresectable malignancies by percutaneous transhepatic insertion of self-expandable metallic stents

Abstract:Objective: To determine the technical feasibility and clinical efficacy of self-expandable metallic stents for palliation of obstructive jaundice caused by unresectable malignancies. Methods: A total of 105 patients with unresectable malignant tumors involving the bile duct who presented with obstructive jaundice underwent transhepatic implantation of self-expandable metallic stents. The follow-up period of the patients was 3-755 days. Technical and clinical success, adverse events, patient survival, and duration of stent patency were analyzed. Results: Percutaneous biliary stenting was performed on 105 malignant biliary obstructive patients with 129 self-expandable metallic stents. Technical successful rate was 94.3%, and clinical success rate was 78%. Thirty-day mortality after stenting was 7. 6%, not procedure-related. Median survival time and stent patency of all patients were 189 days and 246 days. Primary patency rates were 92.1% and 64.5% at 3 and 6 months, respectively. Conclusion: Self-expandable metal wall stenting for malignant obstructive jaundice provides good palliation with low, procedure-related morbidity and mortality in the long term. Further investigation to identify the subgroup of patients in whom stenting has no beneficial effect would be required.

Abstract:The purpose of this study is to assess the relative clinical efficacy of treating malignant hilar biliary obstruction (MHBO) via percutaneous unilateral or bilateral metal stenting. Relevant articles up to December 2019 were identified within the Web of science, Pubmed, Embase, and Cochrane Library databases. Stent dysfunction served as the primary endpoint, while we assessed technical success, clinical success, early and late complication incidence, and overall survival as secondary outcomes. Pooled odds ratios (ORs) and corresponding 95% confidence intervals (CIs) were calculated for dichotomous variables. Hazard ratio (HR) with 95% CI were determined for overall survival. This meta-analysis included seven studies. Six studies were non-randomized controlled trials (RCTs) and one study was a RCT. A total of 888 MHBO patients underwent either percutaneous unilateral (n = 376) or bilateral (n = 512) metal stenting in these seven studies. We detected no significant differences in stent dysfunction rates (OR 0.97; 95% CI 0.67, 1.41,P = 0.89), technical success rates (OR 1.10; 95% CI 0.53, 2.29,P = 0.81), clinical success rates (OR 0.72; 95% CI 0.43, 1.22,P = 0.22), early complication rates (OR 0.82; 95% CI 0.34, 1.98,P = 0.66), late complication rates (OR 0.87; 95% CI 0.29, 2.63,P = 0.81), or overall survival (HR: 0.99; 95% CI 0.83, 1.17,P = 0.88) between unilateral and bilateral groups. Funnel plots demonstrated no obvious publication bias of these primary and secondary endpoints. From a clinical perspective, percutaneous unilateral and bilateral metal stenting are similarly effective for treatment of patients with MHBO.

Abstract:Objective To explore methods of remote implanting gastointesinal inner stent in digestive tract and evaluate the value of metal inner-stent implantation in the treatment of malignant gastro-duodenal jejunal stenosis. Methods There were 67 cases with malignant gastro-duodenal and proximal jejunal stenosis, age ranged from 20 to 86 years. Under X-ray monitoring super-slip guide wire was introduced in by conveying catheter or gastroscope, passing through gastro-duodenal stenosis or obstruction. For those of complete obstruction or failure for the guide-wire to pass, we used microwave or heat-node to build fine path first, and then we inserted guidewire to pass through with stent implanted. Results 64 cases of the whole group were operated for 95 times, 83 metal stents were implanted successfully. Among them, 18 times in gastric corpus and pyloric part,17 times in gastro-intestinal anastomosis.44 times in duodenal descending and horizontal segment,4 times in proximal jejunum, 59 cases had clinical symptomatic remission ,and 5 cases had much improvement in general condition. One month postoperative examination showed stent passing rate as 97.5%. Five stents in 4 cases revealed migration from 16 to 61 days after implantation. One stent was taken out through oral cavity,other three stents through anus and another one stayed at ileocecal region without any complication.Conclusion Peroral implantation of metal stents alleviated malignant gastro-duodenal and jejunal stenosis providing palliative treatment, raising quality of life and prolonging survival time in advanced cancer patients.
Assessment of Endoscopic Drainage with Biliary Double Stents for Advanced Malignant Hilar Biliary Obstruction

Abstract:Objective To evaluate the therapeutic effects of endoscopic biliary double stents for advanced malignant hilar biliary obstruction.Methods From January 2007 to December 2010,double stents was attempted in 28 patients (15 men and 13 women,median age 66.4 years (44-88 years),including 9 with Bismuth Ⅱ,8 with type Ⅲa,5 with type Ⅲb and 6 with type Ⅳ.A total of 23 consecutive patients ( 11 men and 12 women,median age 65.8 years (42-83 years) with malignant hilar obstruction undergoing a therapy with single stent were recruited as the control group,including 7 with Bismuth Ⅱ,5 with Ⅲa,6 with Ⅲlb and 5 with Ⅳ.The rates of successful drainage,complications,mean survival time of patients and the average duration of biliary stent patency were compared between the two groups.Results Successful rate of cannulation was both 100％ in the two groups.Successful rate of drainage and complications of double stent group were 96.4％ (27/28) and 17.9％ (5/28),and these two variables of single stent group were 87.0％ (20/23) and 13.0％ (3/23),which were not significantly different (P ＞0.05).23 patients (82.1％ ) in double stent and 19 ( 82.6％ ) in single stent group were followed up.The average duration of stent patency and mean survival time of double stent group were ( 129 ±48.5) d and ( 187 ±94.5) d,which were superior to those of the single stent group,i.e.( 102 ±37.8) d and ( 103 ±98.5) d.Conclusion Double stenting is an effective therapy for malignant hilar obstruction of Bismuth Ⅱ and above.It is superior to single stent method in the mean duration of patency and mean survival time.
Optimal stent placement strategy for malignant hilar biliary obstruction: a large multicenter parallel study

Abstract:Background and Aims: The endoscopic management of malignant hilar biliary obstruction (MHBO) remains extremely challenging without universal consensus. For the first time, we compared 4 major modalities aiming to determine the optimal strategy. Methods: We reviewed 1239 patients with advanced MHBO who underwent endoscopic stent placement as the primary treatment in 4 tertiary centers. Among them, 633 eligible patients were identified and classified into 4 groups: bilateral metal stent placement (BMS), unilateral metal stent placement (UMS), bilateral plastic stent placement (BPS), and unilateral plastic stent placement (UPS). The outcomes were compared before and after propensity score matching (PSM). Results: After PSM, 87, 97, 91, and 81 patients in the BMS, UMS, BPS, and UPS groups, respectively, were matched. The clinical success rates were 98.9%, 83.5%, 71.4%, and 65.4% in the BMS, UMS, BPS, and UPS groups (P < .001), respectively. The postprocedural cholangitis rates were 8.0%, 17.5%, 26.4%, and 29.6% (P = .002), respectively. The median symptom-free stent patency was 9.6, 6.8, 4.6, and 4.2 months (P < .001), respectively. The mean number of interventions required was 1.2 +/- 0.5, 1.7 +/- 0.8, 2.0 +/- 1.4, and 1.9 +/- 1.3 (P < .001), respectively. The median (95% confidence interval) overall survival (OS) was 7.1 (6.0-8.2), 4.4 (3.8-4.9), 4.1 (2.9-5.2), and 2.7 (1.8-3.7) months (P = .001), respectively. Compared with plastic stent placement, metal stent placement achieved higher success in all outcome parameters (P < .001). Bilateral stent placement was superior to unilateral stent placement in terms of clinical success (P = .024), stent patency (P = .018), and OS (P = .040). Conclusions: If technically possible, dual metal stent placement is a preferred palliation for inoperable MHBO, and unilateral metal stent placement is the second option.

Abstract:Objective To explore the effects and safety of endoscopic parallel placement of double metal stents on unresectable hilar malignant obstruction.Methods The clinical data of 11 patients with malignant hilar obstructive jaundice due to advanced carcinoma who were treated with parallel placement of double biliary stents from January 2011 to September 2012 were retrospectively analyzed.Results Out of 11 patients,10(90.9％) were successfully embedded with double biliary stents and 4 were dead during the follow-up.There was no sign of stent occlusion during the follow-up period.The survival time ranges from 128 to 185 days.One case was lost during the follow-up and 5 others are still alive.Conclusion The endoscopic parallel placement of double biliary stents is effective and safe for patients with unresectable malignant hilar obstruction.

Abstract:Since the late 1970s, endoscopic biliary stenting has become a standard palliative treatment for obstructive jaundice due to malignancies of the pancreas and the hepatobiliary system. Despite the high initial success rate in achieving biliary drainage, endoscopic stenting therapy has been limited by the clogging of biliary stents, usually after four to five months, due to formation of adherent bacterial biofilm and accumulation of biliary sludge. Various methods for the prevention of bacterial adhesion and prolongation of stent patency have been investigated, including prophylactic antimicrobial agents and bile salts, new stent materials, and new stent designs. Recently, the introduction of self-expandable metal stents has significantly improved the duration of stent patency but the cost is considerably higher. Each method has its own merits as well as specific problems. This article reviews the pathogenesis of biofilm formation on the biliary stents and the latest status of research in avoiding stent occlusion.

This randomized controlled multicenter trial is of great significance for clinical practice, yet we would like to share our reservations for further research. Percutaneous Stenting for Malignant Hilar Biliary Obstruction: a Randomized Controlled Trial of Unilateral Versus Bilateral Stenting.

Abstract:Purpose To compare the clinical outcomes between unilateral and bilateral metal stenting for patients with malignant hilar biliary obstruction (MHO). Methods This is a single-center, open-label, prospective, randomized study. Between January 2016 and March 2018, patients with MHO who were treated by percutaneous unilateral or bilateral metal stenting were enrolled. The primary endpoint was stent dysfunction. The secondary endpoints included technical success, clinical success, adverse events, and death. The protocol is registered at ClinicalTrials.gov (identifier: NCT02649712). Results A total of 72 patients were randomly grouped for the unilateral ( n = 36) or bilateral ( n = 36) stenting. The bilateral stenting was performed through the side-by-side technique. While technically, the rates of success of unilateral and bilateral stenting were 83.3% (30/36) in both the cases ( P = 1.000), the clinical rates of success in unilateral and bilateral stenting were 90.0% (27/30) and 96.7% (29/30), respectively ( P = 0.605). Based on the per-protocol analysis, stent dysfunction was found in 5 and 3 patients in unilateral and bilateral groups, respectively (16.7% vs. 10.0%, P = 0.704). No predictor was observed to influence stent dysfunction. The median cumulative survival in the unilateral group was 122 days and in the bilateral group was 125 days ( P = 0.844). We also observed higher levels of post-operative total bilirubin and pre-operative alanine aminotransferase, and the absence of post-operative anticancer treatment as predictors of worse survival. Conclusion When compared, the bilateral and unilateral stentings provide a similar clinical effectiveness in patients with MHO.

Abstract:Objective To evaluate the clinical value of metal stenting under endoscopy for malignant gastric outlet, duodenal or proximal small bowel obstruction. Methods The clinical data of 21 patients with malignant gastric outlet, duodenal or proximal small bowel obstruction treated with endo-scopic metal stenting from March 1999 to March 2005 were analysed retrospectively. Results Stent insertion was technically successful in 20 patients (95. 2%), 4 stents were placed through the scope channel(TTS), 16 stents were non-TTS placement. After stenting, 19 patients showed relief of obstructive symptoms within 1 to 3 days, the clinical success rate was 90. 5% , the mean survival date was 4. 5 months. During the follow-up period, one patient had gastrointestinal bleeding, and was successfully treated by internal medication. Recurrent obstruction was observed in 2 cases, one with distal stent migration one month after stent implantation, the other with tumor ingrowth at 2 months. The obstruction symptoms were relieved after second stent placement in these two cases. Conclusions Endoscopic placement of metal stent is a simple, safe and effective method for palliation in patients with malignant gastric outlet, duodenal or proximal small bowel obstruction.
