A Novel Approach to Overcome Non-Small-cell Lung Cancer: Co-Inhibition of Autophagy and Hedgehog Pathway

Abstract:Background: Amongst the various types of lung caners, non-small-cell lung cancer (NSCLC) comprises ∼80% of all cases, while over 70% of NSCLCs could be diagnosed as unresectable ones. Although a series of efforts, including platinum-based chemotherapy and tyrosine kinase inhibitors, have been applied to optimize the treatment of NSCLC, the prognosis for NSCLC patients still remains poor, with the 5-year overall survival rate of ∼15% only. Herein, we report a potential approach for NSCLC therapy by co-inhibition of autophagy and Hedgehog pathway. Methods: We used MTT assay to determine the growth inhibitory effect of Vismodegib with/without autophagy inhibitors. Confocal microscope, transmission electron microscope (TEM) and western blots were applied to detect the autophagy in A549 and H1975 NSCLC cells after Vismodegib exposure. Flow cytometry and confocal microscope were employed to test apoptosis and reactive oxygen species (ROS) in NSCLC cells after Vismodegib and/or autophagy inhibitors treatment. Results: Western blots analysis showed the conversion of LC3-I to LC3-II in Vismodegib-treated NSCLC cells. The results of TEM indicated the double-membrane autophagic vacuoles formation and accumulation, and intensive specific punctuate fluorescence could be observed under the confocal microscope. These results indicated the autophagy was induced by vismodegib in NSCLC cells. Furthermore, the growth of NSCLC cells faced severe disadvantage after cells were exposed to vismodegib and autophagy inhibitors, with accelerated apoptotic cells and increased ROS. Conclusions: We demonstrated that autophagy was triggered in NSCLC cells after vismodegib treatment, while inhibition of autophagy could significantly enhance the anti-tumor efficacy of vismodegib in NSCLCs. Our data elucidated the critical role of autophagy in vismodegib-based NSCLS therapy and highlighted the potential approach for the treatment of NSCLC by co-inhibition of Hedgehog pathway and autophagy.
