Abstract:Pancreatic cancer has the worst prognosis of any gastrointestinal cancer, with the mortality approaching the incidence. Early detection is crucial for improving patient prognosis. We therefore performed a meta-analysis to evaluate and compare the sensitivity and specificity of CA19-9 and CA242 in pancreatic cancer. We searched PubMed, EMBASE, and the Cochrane Library for studies that evaluated the diagnostic validity of CA19-9 and CA242 between January 1966 and March 2011. Meta-analysis methods were used to pool sensitivity and specificity and to construct a summary receiver-operating characteristic (SROC) curve. A total of 11 studies that included 2,316 patients who fulfilled all of the inclusion criteria were considered for analysis. The pooled sensitivities for CA242 and CA19-9 were 0.719 (95 % confidence interval [CI] 0.690–0.746) and 0.803 (95 % CI 0.777–0.826), respectively. The pooled specificities of CA242 and CA19-9 were 0.868 (95 % CI 0.849–0.885) and 0.802 (95 % CI 0.780–0.823), respectively. The diagnostic odds ratio (DOR) estimate was significantly higher for CA242 (16.261) than for CA19-9 (15.637). Our meta-analysis showed that CA242 and CA19-9 could play different roles in the diagnosis of pancreatic cancer. Although the sensitivity of CA242 is lower than that of CA19-9, its specificity is greater.

Abstract:Objective: To evaluate the significance of tumor markers of CA242, CA19-9 and CEA in diagnosis of pancreatic and bilious cancer.Methods: CA242, CA19-9 AND CEA were detected in 74 cases of pancreatic and bilious disease.Results: The serum levels of CA242, CA19-9 and CEA were significantly increased in patients with pancreatic and bilious cancer than those of normal control group. Among 44 cases of pancreatic cancer, there were 34 cases with positive CA19-9, 32 cases with positive CA242 and 5 cases with positive CEA, the sensitivities were 77.3%, 72.7% and 22.7% respectively, the specificities were 73.9%, 89% and 80.5%respectively. There was not statistical difference between CA242 and CA19-9 in sensitivity, but the specificity of CA242 was significantly higher than that of CA19-9 (P0.05). CA19-9 was positive in 12 cases and CA242 was positive in 1 case among 30 patients with benign obstructive jaundice, the rates of false positive were 40% and 3.3% respectively. Conclusion:The serum levels of CA242 and CA19-9 can be used as a supplementary laboratory index for diagnosis of pancreatic cancer, CA242 was superior to CA19-9 because of its higher specificity and lower false positive rate.

Abstract:CA 19-9 and carcinoembryonic antigen (CEA) are widely used for the diagnosis of pancreatic cancer. The purpose of the present study was to compare the diagnostic value of CA 19-9 with CEA for pancreatic cancer.The studies were obtained from electronic searches conducted in PubMed, Embase, and Cochrane Library databases until December 2017. The keywords included diagnosis of pancreatic cancer, CA 19-9, and CEA. The ratio of sensitivity, the specificity, the diagnostic odds ratio (DOR), and the summary of the receiver operating characteristic (SROC) with regard to CA 19-9 and CEA were measured using the random effects model. The current study included 13 studies that comprised 4,537 participants and 1,277 patients with pancreatic cancer.The levels of CA 19-9 for use for the detection of pancreatic cancer were associated with higher sensitivity (ratio of sensitivity: 1.54; 1.31-1.81; P < 0.001), DOR (DOR: 3.50; 95% CI: 2.24-5.45; P < 0.001), and AUC (ratio of AUC: 1.24; 95% CI: 1.15-1.33; P < 0.001) compared with the variable CEA, while no significant difference between CA 19-9 and CEA was noted with regard to specificity (ratio of specificity: 0.97; 95% CI: 0.89-1.06; P = 0.517). The findings of the subgroup analyses suggested that different cutoff values of CA 19-9 and CEA might affect the diagnostic value.The findings of the present study indicated that CA 19-9 levels were associated with higher sensitivity, DOR, and AUC compared with the corresponding levels of CEA with regard to the diagnosis of pancreatic cancer.

Abstract:Objective:The use of a combination of tumor markers may be an important tool in the early diagnosis of pancreatic cancer,which is the key to improving prognosis. The study aim was to investigate the diagnostic value of carbohydrate antigen 19-9（CA 19-9） and carcinoembryonic antigen（CEA） levels in patients with pancreatic cancer. Methods:An immunoradiometric assay was used to homochronously measure the serum CA19-9 and CEA levels in 78 pancreatic cancer cases and 64 healthy examinees in hospital. The normal reference values were CA19-9（0-39 U/ml） and CEA（0-3.4 ng/ml）. Results:Mean serum CA19-9 and CEA levels in patients with pancreatic cancer（406.55±60.18 U/ml,12.43±1.25 ng/ml） were significantly higher（P ＜ 0.01） than those in healthy examinees（16.54±1.95 U/ml,2.37±0.17 ng/ml）. The sensitivity of the combined detection of CA19-9 and CEA （92.31%） was significantly higher（P ＜ 0.05） than that of either marker alone （79.49%,71.79%,respectively）. In addition,the sensitivity to diagnose pancreatic cancer by detecting the serum CA19-9 and CEA levels was higher（P ＜ 0.05） in stage Ⅱ B + Ⅲ + Ⅳ（87.04%,79.23%） than stageⅠ + Ⅱ A（62.50%,54.17%）. Conclusion:The combined detection of CA19-9 and CEA could overcome the deficiency of using single marker detection by improving the sensitivity to diagnose pancreatic cancer. At the same time,CA19-9 and CEA detection could be used to assess mesenteric artery invasion and the metastasis of lymphatics and distant organs in pancreatic cancer.

Abstract:Objective To investigate the clinical significance of CA19-9 and CEA detection in the diagnosis of pancreatic carcinoma.Methods From January 2003 to November 2005,serum tumor markers(CA19-9 and CEA)in 44 patients with pancreatic carcinoma and 64 healthy volunteers were detected by radioimmunoassay.Results The levels of CA19-9 and CEA were markedly higher in the patients with pancreatic carcinoma than in healthy volunteers(P＜0.01).The sensitivity of combined detection of CA19-9 and CEA was much higher than any single detection.While the specificity of combined detection of CA19-9 and CEA had no statistical difference when compared to a single detection.The levels of CA19-9 and CEA in the Ⅰ+ⅡA stage patients were of no significant difference as compared with those in the patients of ⅡB+Ⅲ+Ⅳ stage,whereas the sensitivity of CA19-9 and CEA in the patients of ⅡB+Ⅲ+Ⅳ stage but much higher than in those of Ⅰ+ⅡA stage.Conclusion The serum concentrations of CA19-9 and CEA are closely related to pancreatic carcinoma with the invasion of mesenteric artery and lymphatic and distant organic metastasis.Combined detection of CA19-9 and CEA can improve the diagnosis of pancreatic carcinoma.
Diagnostic and Prognostic Value of Carcinoembryonic Antigen in Pancreatic Cancer: a Systematic Review and Meta-Analysis

Abstract:Background Carcinoembryonic antigen (CEA) is one of the most widely used tumor markers and is increased in 30%–60% of patients with pancreatic cancer. Although carbohydrate antigen 19-9 (CA19-9) is the most important serum biomarker in pancreatic cancer, the diagnostic and prognostic value of CEA is gradually being recognized. Materials and methods The MEDLINE, EMBASE, and Web of Science databases were searched for related literature published until January 2017. Diagnostic accuracy variables were pooled using the Meta-Disc software. The pooled hazard ratios (HRs) for prognostic data were calculated and analyzed using Stata software. Results A total of 3,650 participants enrolled in 19 studies met our inclusion criteria. The pooled sensitivity, specificity, positive likelihood ratio, and negative likelihood ratio of a CEA-based panel were 0.45 (95% confidence interval [CI], 0.41–0.50), 0.89 (95% CI, 0.86–0.91), 5.39 (95% CI, 3.16–9.18), and 0.55 (95% CI, 0.41–0.72), respectively. The area under the curve (AUC, 0.90) and Q-value (0.84) of the CEA-based panel indicated a significantly higher diagnostic accuracy compared with CEA or CA19-9 alone. Moreover, there was also a significant association between high levels of CEA and worse overall survival (HR, 1.43; 95% CI, 1.31–1.56). Conclusion Our meta-analysis indicated that elevated serum CEA level, as a vital supplementary to CA19-9, can play an important role in the clinical diagnosis of pancreatic cancer patients and predict poor prognosis.

Abstract:AIM:To establish a method to detect the expression of the tumor specific growth factor TSGF, CA242 and CA19-9 in serum and evaluate their value in diagnosis of pancreatic cancer.METHODS:ELISA and Biochemical colorimetric assay were used to detect the serum content of TSGF, CA242 and CA19-9 in 200 normal cases, 52 pancreatitis patients and 96 pancreatic cancer patients.RESULTS:The positive likelihood ratios of TSGF, CA242 and CA19-9 were 5.4, 12.6 and 6.3, respectively, and their negative likelihood ratios were 0.10, 0.19 and 0.17, respectively. With single tumor marker diagnosed pancreatic cancer, the highest sensitivity and specificity of TSGF were 91.6% and 93.5%. In combined test with 3 markers, when all of them were positive, the sensitivity changed to 77.0% and the specificity and the positive predictive value were 100%. The levels of TSGF and CA242 were significantly higher in the patients with pancreatic cancer of head than those in the patients with pancreatic cancer of body, tail and whole pancreas, but the expression of CA19-9 had no correlation with the positions of the pancreatic cancer. The sensitivity of TSGF, CA242 and CA19-9 was increased with the progress in stages of pancreatic cancer. In stage I, the sensitivity of TSGF was markedly higher than CA242 and CA19-9.CONCLUSION:The combined use of TSGF, CA242 and CA19-9 expressions can elevate the specificity for pancreatic cancer diagnosis. And it shows that it plays an important role to differentiate positions and tissue typing. It is a forepart diagnosis for the pancreatic cancer by combination checking. There is very important correlation between the three markers and the pancreatic cancer.
Clinical Significance of CA242 and CA19-9 in Colorectal Cancer

Abstract:Objective:To investigate the clinical value of CA242 and CA19-9(tumor marker) in the diagnosis,treatment and prognosis of colorectal cancer. Methods: Serum levels of CA242 andCA19-9 in 35 cases with colorectal cancer and 204 cases without cancer were examined by enzyme linked immunosorbent assay (ELISA) . Results: Serum levels of CA242 and CA19-9 were significantly higher in the patients with colorectal cancer than that without (99.51±112.41U/mL vs 9.63± 15.50U/mL and 46.23±70.82U/ mL vs 8.42 ± 22.02U/mL) , respectively(P 0.01) . The diagnostic sensitivity,specificity and accuracy of serum CA242 and CA19-9 were 60%, 96.57% and 91.21% vs 31.43%,95.59% and 86.19% .respectively. CA242 was more sensitive than CA19-9 in the diagnosis of colorectal cancer, but the difference in specificity and accuracy was not marked. The sensitivity of examination of CA242 and CA19-9 jointly was higher than singly (68.57% vs 31.43%). The sensitivity of CA242 and CA19-9 was higher in the diagnosis of colon cancer (64.71% and 41. 18%)than rectal cancer (55.55%and 22.22%). For the patients accompanied by serum CA242 level elevated or not , the therapeutic responses were 14.29% and 57.14% respectively ( P 0.05),and the median survival was 6.5 months and 9 months. For the patients accompanied by serum CA19-9 level elevated or not, the therapeutic responses were 27.27% and 33.33% respectively (P 0.05), and the median survival was 6.5 months and 7 months. Conclusion: CA242 and CA19-9 are useful serum tumor markers for the diagnosis of colorectal cancer. Abstract:Objective To observe the expression of carcinoembryonic antigen (CEA),carbohydrate antigen 19-9 (CA19-9),and carbohydrate antigen 242(CA242) in pancreatic cancer patients and to search the valuable serum tumor markers that can identify patients with unresectable pancreatic cancer despite radiologic staging demonstrating resectable disease.Methods Preoperative and postoperative serum CEA,CA19-9,and CA242 were measured in 23 resectable and 45 unresectable pancreatic cancers by immunoassay and immunoradiometric assay.Results Among the three tumor markers,CA 19-9 and CA 242 levels were significantly lower in resectable cases than those of unresectable cases( P=0.01,P=0.02) while the CEA levels were not different in the two groups(P=0.08).Conclusion Obviously increased serum levels of CA19-9 and CA242 maybe regarded as the ancillary parameter for the unresectable pancreatic cancer.

Abstract:Objective:To evaluate serum tumor marker carbobydrate antigen 19-9(CA19-9)for the diagnosis and prognosis of pancreatic cancer.Methods:Serum CA19-9 levels were determined gerially electrochemilu minescene in 132 patients with pancreatic cancer coming from West China Hospital and Sichuan province people's hospital from jan 2001 to jan 2004.Routine preoperative examinations in all patients consist of ultrasonography,CT-scan and MRI of the abdomen;clinical courses with respect to the pathological data,surgical procedures employed and the prognosis of these patients were evaluate prospectively.Results:Serum CA19-9 level of 589±502.21 and the positive rate 87.12% in 132 patients with pancreatic cancer were significantly higher than that of healthy people.With higher-serum CA19-9(CA19-9>200 u/mL)level group for the metastasis rate of pancreatic carcinoma and operation resection rate were no significant relation with the lower-serum(CA19-9≤200 u/mL)CA19-9 level group.There were no significant relations between the serum CA19-9 level and other item such as the age,sex,smoking history of the patients,differentiation and location of the tumor.The serum CA19-9 level had significant relations with the stage,size and histological classification of tumor.Conclusion:Serum CA19-9 may be of useful tumor maker for diagnosis of pancreatic carcinoma,prediction of the prognosis of the patients after resection.The serum CA19-9 is unablel for guiding the surgical operation and were be influenced by the stage,size and histological classification of tumor.

Abstract:OBJECTIVE:To investigate the clinical value of tumor markers CEA, CA19-9, CA72-4 and CA242 in the diagnosis and prognosis of patients with gastric cancer.METHODS:One hundred and sixty gastric cancer patients who had received treatment from 2002 to 2007 at the Beijing Cancer Hospital were retrospectively analyzed. Blood samples were taken from patients upon admission to the hospital, and CEA, CA19-9, CA72-4, CA242 levels were detected. Statistical analysis was performed to identify the clinical value of these tumor markers in diagnosis and prognosis.RESULTS:On initial diagnosis, the positive rates of CEA, CA19-9, CA72-4 and CA242 were 37.7%, 26.7%, 37.6% and 21.3%, respectively, and the positive rate of combined detection was 62.9%. CEA was more frequently positive in patients with lymph node metastasis (P=0.029); CA72-4 was more frequently positive in patients with vascular involvement and advanced stage (P=0.039, P=0.011). Multivaraite analysis showed that CA72-4 was an independent prognostic factor (P=0.012). Patients with positive CA72-4 carried a 2.147-fold increased risk of death than those with negative CA72-4. Kaplan-Meier analysis showed that patients with positive CA19-9 or positive CA72-4 had worse survival than those with negative CA19-9 or CA72-4 (P=0.006, P=0.002).CONCLUSIONS:Tumor markers including CEA, CA19-9, CA72-4 and CA242 have clinical significance and prognostic value in patients with gastric cancer. Combined detection of four tumor markers can increase the positive rate. CA72-4 is an independent prognostic factor. CA19-9 and CA72-4 are associated with the prognosis of patients with gastric cancer.

Abstract:Pancreatic ductal adenocarcinoma (PDAC) is a highly malignant tumor with few biomarkers to guide treatment options. Carbohydrate antigen 19.9 (CA19.9), the most frequently used biomarker for PDAC, is not sensitive and specific enough for the detection of the disease. This study aimed to evaluate serum periostin (POSTN) and CA242 as potential diagnostic biomarkers complementing CA19.9 in detecting pancreatic cancer. Blood samples were from 362 participants, including 213 patients with different stages of PDAC, 75 patients with benign pancreatic disease, and 74 healthy individuals. All samples were randomly divided into a training set and a validation set. Carbohydrate antigen 19.9, CA242, POSTN, as well as carcinoembryonic antigen, were measured by ELISA or automated immunoassay. The receiver operating characteristic curve analysis revealed that the performance of CA19.9 in the validation group were improved by the marker panel composed of CA19.9, POSTN, and CA242, to discriminate early stage PDAC not only from healthy controls (area under the curve [AUC]CA19.9 = 0.94 vs AUCCA19.9 + POSTN + CA242 = 0.98, P < .05) but also from benign conditions (AUCCA19.9 = 0.87 vs AUCCA19.9 + POSTN + CA242 = 0.90, P < .05). In addition, POSTN retained significant diagnostic capabilities to distinguish PDAC CA19.9-negative from healthy controls (AUCPOSTN = 0.87) as well as from benign conditions (AUCPOSTN = 0.84) in the whole set. This study suggested that POSTN and CA242 are potential diagnostic serum biomarkers complementing CA19.9 in detecting early pancreatic cancer.

Abstract:To evaluate the clinical significance of combined analysis of seven biomarkers in the diagnosis of pancreatic cancer,the serum CA19-9,CA242,CEA,β2-MG,PALB,Fib and D-D levels were detected in 125 patients with pancreatic cancer,48 patients with liver metastasis of pancreatic cancer,40 patients with benign pancreatic tumor and 60 healthy controls by CLIA,ELISA,coagulation point detection method and biochemistry assay.The results showed that the combined measurement of CA19-9,CA242,CEA,β2-MG,PALB,Fib and D-D could increase the diagnostic sensitivity of pancreatic carcinoma to 96.0%.There was no statistics significance in Fib levels with tissue grading number increasing(P>0.05),but the CA19-9,CA242,CEA,β2-MG,PALB and D-D levels had statistics significance(P<0.05).The serum levels of CA19-9,CA242,CEA,β2-MG,Fib and D-D in patients with pancreatic cancer and liver metastasis of pancreatic cancer were increased,while the serum PALB level was decreased.The combined measurement of these biomarkers could increase the sensitivity in the diagnosis of pancreatic carcinoma.

Abstract:Pancreatic cancer has the worst prognosis of any gastrointestinal cancer, with the mortality approaching the incidence. Early detection is crucial for improving patients’ prognosis. CA242 has been widely reported to play a role in diagnosis of pancreatic cancer. However, published data on this subject are inconclusive. Therefore, we performed a meta-analysis to evaluate the diagnostic value of CA242 in pancreatic cancer. We searched all the eligible studies through PubMed, Embase, and the Cochrane Library databases without language limitation. Studies were assessed for quality using the quality assessment of studies of diagnostic accuracy (QUADAS). Positive likelihood ratio (PLR) and negative likelihood ratio (NLR) were pooled separately and compared with overall accuracy measures diagnostic odds ratio (DOR) and symmetric summary receiver operating characteristic (SROC). The PLR and NLR and their 95 % confidence interval (CI) were calculated using a fixed effects model according to the Mantel-Haensed method and random effects model based on the work of DerSimonian and Laird, respectively. A total of eight studies were included for analysis. The pooled sensitivity was 0.719 (95 % CI 0.690–0.746). The pooled specificity was 0.868 (95 % CI 0.849–0.885). The DOR estimate was performed and the result was (16.261). Our meta-analysis showed that CA242 could play an important role in the diagnosis of pancreatic cancer.

Abstract:Objective To study the neutrophil-lymphocyte ratio (NLR),CA19-9 and CEA in the diagnosis of pancreatic cancer (PC).Methods From January 2013 to June 2016,the data of 723 consecutive patients with pancreatic diseases were reviewed.Of these 723 patients,632 patients had PC (stage Ⅰ,Ⅱ 324 patients;stage Ⅲ,Ⅳ 289 patients),66 patients had pancreatic cystic tumors,and 26 patients had tumor-forming pancreatitis.The Receiver Operating Curve (ROC) and the logistic regression model were used to assess the blood biomarkers in predicting PC.Results Using the ROC,CA19-9 CEA,NLR,PLR,ALP,GGT,LDH,GLU and MONO# were useful in diagnosing malignant pancreatic diseases.Logistic regression analysis showed that CA19-9 (95％ CI:12.928 ～ 103.330;P ＜ 0.05),CEA (95％ CI:1.041 ～3.472,P＜0.05) and NLR (95％ CI:1.020～ 3.525,P =0.043) were independent variables in predicting PC.The concentrations of CA19-9,CEA,and NLR had the highest values in predicting PC.When CA19-9 was ＜37 kU/L,the concentrations of NLR and CEA had a high sensitivity and specificity (AUC =0.746,95％CI:0.675～0.816,P＜0.05) of 69.2％ and 73.6％,respectively.Conclusions The concentrations of CA19-9,CEA,and NLR had the highest prediction value for PC.When the level of CA19-9 was ＜37 kU/L,the combined use of NLR and CEA significantly improved the diagnostic specificity.

Abstract:Objective To determine the clinical value of EUS in combination with detection of CA19-9 and CA242 for early diagnosis of pancreatic cancer.Methods General features of high-risk group were determed by EUS and the plasma levels of CA19-9 and CA242 measured by ELISA.Results The serum level of CA19-9 was abnormal in 9.80% (10/102) and that of CA242 in 12.75% (13/102) of the patients.They were both abnormal in 7 cases.Of the 7 cases, 3 were found to suffer from pancreatic cancer by EUS, 1 had the tumor that had been clinically confirmed, 1 had posterior peritoneal tumor and 2 were normal.Among 102 cases detected by EUS, 2 showed low echo in pan-creatic head, 2 semi-cyst, 3 pancreatic cancer and proved by pathology.Accurate diagnotic rate, sensi-tivity and specificity were 90.20% (92/102), 50% (4/8) and 93.62% (88/94), respectively, for serum CA19-9.For the serum CA242, the 3 parameters were 87.25% (89/102), 50% (4/8) and 90.43% (85/94), respectively.For combination of both methods, they were 98.04% (100/102), 100%(8/8) and 97.87%(92/94), respectively.Accurate diagnostic rate of the latter was significantly higher than that of the former 2(P＜0.05).Concision Combination of EUS with detection of serum CA19-9 and CA242 is of great vlaue for early screening of pancreatic cancer.

Abstract:BACKGROUND Gastric cancer is a global health concern that poses a significant threat to human well-being. AIM To detecting serum changes in carcinoembryonic antigen (CEA), carbohydrate antigens (CA) 724, CA242, and CA19-9 expression among patients with gastric cancer. METHODS Eighty patients diagnosed with gastric cancer between January 2020 and January 2023 were included in the observation group, while 80 patients with benign gastric diseases were included in the control group. Both groups were tested for tumor markers (CA724, CEA, CA242, and CA19-9]. Tumor marker indicators (CA724, CEA, CA242, and CA19-9) were compared between the two groups, assessing positive rates of tumor markers across various stages in the observation group. Additionally, single and combined detection of various tumor markers were examined. RESULTS The sensitivity, specificity, accuracy, positive predictive value, and negative predictive value observed for the combined detection of CA724, CEA, CA242, and CA19-9 were higher than those of CA724, CEA, CA242, and CA19-9 individually. Therefore, the combined detection of CA724, CEA, CA242, and CA19-9 has a high diagnostic accuracy and could reduce the occurrence of missed or misdiagnosed cases, facilitating the early diagnosis and treatment of patients. CONCLUSION CA724, CEA, CA242, and CA19-9 serum levels in gastric cancer patients significantly surpassed those in non-gastric cancer patients (P < 0.05). Their combined detection can improve the diagnostic accuracy for gastric cancer, warranting clinical promotion.

Abstract:Objective To explore the concentrations and clinical value of CA19-9 in different body fluids such as serum,pancreatic juice,bile,gastric juice and urine in patients with pancreatic cancer and benign biliary pancreatic diseases.Methods Sixteen cases of pancreatic cancer,13 cases of benign pancreatic diseases and 13 cases of common bile duct stones admitted from January to March 2007 were studied.Different body fluids were collected and CA19-9 were measured by chemiluminscent microparticle immunoassay.Results The median level of CA19-9 in serum,pancreatic juice,bile,gastric juice was higher in patients with pancreatic cancer than that in the other two groups,and gastric juice value of CA19-9 increased significantly in pancreatic cancer patients compared with benign pancreatic diseases patients(P=0.022) .The median level of CA19-9 in urine was higher in patients with common bile duct stones than that in the other two groups,and urine value of CA19-9 increased significantly in common bile duct stones patients compared with benign pancreatic diseases patients(P=0.047) .Conclusions The value of CA19-9 in body fluids may be helpful in differentiate diagnosis of pancreatic cancer and benign biliary pancreatic diseases,but the mechanism need more study.
