Abstract:AIM: To sum up the clinical and pathological characteristics of solid pseudopapillary tumor (SPT) and the experience with it. METHODS: A total of 553 SPT patients reported in Chinese literature between January 1996 and January 2009 were retrospectively reviewed and analyzed. RESULTS: The mean age of the 553 SPT patients included in this review was 27.2 years, and the male to female ratio was 1:8.37. Their symptoms were nonspecific, and nearly one third of the patients were asymptomatic. Computed tomography and ultrasonography were performed to show the nature and location of SPT. Most of the tumors were distributed in the pancreatic head (39.8%), tail (24.1%), body and tail (19.5%). Forty-five patients (9.2%) were diagnosed as malignant SPT with metastasis or invasion. None of the clinical factors was closely related to the malignant potential of SPT. Surgery was the main therapeutic modality for SPT Local resection, distal pancreatectomy and pancreatoduodenectomy were the most common surgical procedures. Local recurrence and hepatic metastasis were found in 11 and 2 patients, respectively, after radical resection. Four patients died of tumor progression within 4 years after palliative resection of SPT The prognosis of SPT patients was good with a 5-year survival rate of 96.9%. CONCLUSION: SPT of the pancreas is a rare indolent neoplasm that typically occurs in young females. It is a low-grade malignancy and can be cured with extended resection. The prognosis of such patients is good although the tumor may recur and metastasize. (C) 2010 Baishideng. All rights reserved.
Prognostic Value of Ki-67 in Solid Pseudopapillary Tumor of the Pancreas: Huashan Experience and Systematic Review of the Literature.

Abstract:Solid pseudopapillary tumor of the pancreas (SPTP) is considered to have a low Ki-67 proliferation index, which may explain the generally good clinical outcome. The aim of our study was to evaluate whether Ki-67 has prognostic value in SPTP.A case series study of patients with SPTP treated in our institution from June 2002 to April 2014 was conducted. Prognostic factors for clinical outcomes were analyzed by the use of clinical decision and survival analysis. In addition, we performed a systematic review and pooled analysis to evaluate our results.The institutional data included 71 patients (13 male and 58 female) ranging in age from 12 to 64 years (median, 31 years). Three patients developed local recurrence and/or liver metastasis after initial surgery. The 5-year recurrence-free survival rate was 93.6%. One patient died of disease, with the 5-year disease-specific survival rate of 98.5%. Ki-67 index ≥ 4% was found significantly associated with the survival of SPTP. Twenty-six studies comprising 163 patients were included in the pooled analysis based on our inclusion criteria. A total of 15 cases showed a Ki-67 index ≥ 4%. Kaplan-Meier survival analysis confirmed that Ki-67 index ≥ 4% was significantly associated with poorer recurrence-free survival and disease-specific survival (both P < .001).This study highlighted a potential role of Ki-67 in predicting adverse outcome of patients with SPTP and should be considered as part of routine histological reporting of SPTP.
Grading Solid Pseudopapillary Tumors of the Pancreas: the Fudan Prognostic Index

Abstract:BackgroundKi-67 has been shown to predict outcome of patients with solid pseudopapillary tumor of the pancreas (SPTP) but has not been incorporated into a formal classification system to predict recurrence-free survival (RFS).MethodsThis is a retrospective cohort study of patients with histologically confirmed diagnosis of SPTP who had at least 1 year of follow-up at two tertiary academic centers. Survival data were assessed by Kaplan–Meier method and multivariable Cox regression model. Prognostic performance was compared among various systems.ResultsA total of 193 consecutive patients were included, ranging in age from 12 to 70 years (median 33 years). Seven patients (3.6%) developed tumor recurrence. The 3-, 5-, and 10-year RFS rates were estimated at 96.9%, 96.1%, and 94.8%, respectively. For the AJCC staging system, patients with stage I had similar prognosis to those with stage II. For the ENETS staging system, patients with stage I to III had similar prognosis. Grade based on Ki-67 was superior to both the AJCC and ENETS systems for predicting survival. Multivariate analysis revealed that large tumor size [> 10 cm; hazard ratio (HR), 6.177 95% confidence interval (CI), 1.289–29.603; P = 0.023] and Ki-67 (HR, 17.199 95% CI, 4.001–73.930; P < 0.001) were independent predictors for RFS. The Fudan Prognostic Index based on the combination of Ki-67 and tumor size showed excellent discrimination for RFS and was more accurate and informative than other grading/staging systems.ConclusionThe Fudan Prognostic Index better predicts RFS compared with either Ki-67 alone or the current AJCC and ENETS TNM-based staging systems.

[A Study of the Gene Encoding Ki-67 Antigen in Human Pancreatic Cancer Using Non-Radioactive in Situ Hybridization and Immunohistochemistry].

Abstract:OBJECTIVE:To investigate, at transcriptional and translational level in situ, the abnormal gene expression of the Ki-67 protein in pancreatic carcinoma specimens.METHOD:40 pancreatic cancer specimens, 5 normal pancreatic and 4 chronic pancreatitis tissues were studied. A 435 bp cDNA fragment located in codon 2, exon 13 of the Ki-67 antigen gene was amplified by the polymerase chain reaction (PCR). The DIG-labeled cRNA probes were transcribed using a commercially available DIG RNA labeling kit. Localization of the Ki-67 protein and the specific mRNA were studied by combining immunohistochemistry (ICH) with (DIG)-labeled in situ hybridization (ISH).RESULT:The Ki-67 protein mRNA in formalin-fixed and paraffin-embedded pancreatic tissue sections was successfully localized. Analyzing the Ki-67 mRNA transcription in 17 pancreatic cancer specimens with Ki-67 ICH labeling index > 20%, we found that stronger mRNA signals were also observed in poorly differentiated specimens with Ki-67 index > 50% than in those well differentiated with ICH labeling index 20% - 50%. A high expression of both mRNA and protein was observed in poorly differentiated pancreatic adenocarcinomas.CONCLUSION:Abnormal overexpression of the gene encoding Ki-67 protein was detected not only at the protein level, but also at the mRNA level in pancreatic tumors. The abnormal overexpression of the Ki-67 protein might be correlated with the central part, exon 13, of the gene.

Abstract:Meta-analysis of Ki-67 expression for recurrence in patients with solid pseudopapillary tumor of the pancreasWe read with great interest the comprehensive systematic review and meta-analysis by Yepuri and colleagues, 1 which investigated predictors of recurrence in patients with solid pseudopapillary tumor of the pancreas (SPTP).We admire the authors for their efforts on such an excellent review, but have some concerns about their result that Ki-67 4% was not associated with a significant odds ratio for recurrence.We would like to replicate the result of this meta-analysis on the predictive value of Ki-67 4%.Review of the literature was performed by searching PubMed, Scopus, and Web of Science databases between September 2002 and September 2017 with the same search terms listed by the authors and Ki-67.A total of 239 citations, with some duplicates among databases, were identified.3][4][5] Our meta-analysis showed that Ki-67 4% was associated with significant odds for recurrence (OR 43.56, 95% confidence interval 4.06-467.81,p = 0.002, Fig. 1).The heterogeneity within the studies was not significant (Heterogeneity Chi 2 4.51, p = 0.21).Although our results are encouraging for Ki-67 as a predictor of recurrence in patients with SPTP, prospective studies including a large number of patients are needed for validation.Overall, due to the changes we presented here, the result regarding Ki-67 of the Yepuri et al. study should be interpreted with caution.Figure 1 The forest plot of the risk of recurrence in patients with SPTP who had Ki-67 4%
Comparison of Tumor Markers for Predicting Disease-Free Survival in Surgically Resected Pancreatic Neuroendocrine Tumors

Abstract:BACKGROUND:Identification of biomarkers of pancreatic neuroendocrine tumors (PNETs) is important for stratification of the prognosis. Ki-67 index was the significant prognostic factor for PNETs. Recently, MMP-9, DJ-1, and α-1-B glycoprotein (A1BG) were shown to be the prognostic markers in some malignant tumors except for PNETs. The aim of this study was to compare these tumor markers for predicting disease-free survival in surgically resected PNETs. METHODS:A retrospective review of patients pathologically diagnosed with PNETs at our institution from January 2012 to January 2014 was conducted. Tumor specimens were stained by immunochemistry for MMP-9, DJ-1, A1BG, and Ki-67. Clinicopathologic findings and these 4 tumor markers of patients with PNETs were analyzed. Prognostic factors were determined by univariate and multivariate analyses. RESULTS:A total of 40 patients were selected. Of 40 tumors, 19 (47.5%) were positive for MMP-9, 21 (52.5%) for DJ-1, 18 (45%) for A1BG, and 24 (60%) for Ki-67 > 2%. Four (10%) tumors expressed none of the markers. MMP-9 and DJ-1 expression were more frequently observed in distant metastasis, higher WHO grade, and lymph node metastasis. Similarly, those with Ki-67 > 2% had larger tumor, higher WHO grade, lymph node invasion and larger surgical procedures. A1BG had no substantial effect on distant metastasis. Univariate analyses revealed that grade 2/3 histology, distant metastasis, lymph nodes metastasis, high expression of MMP-9, high expression of DJ-1, and Ki-67 > 2% were predictive of inferior disease-free survival for PNETs (all p < 0.05). In multivariable analysis, the significant poor factors associated with disease-free survival were grade 2/3 histology, distant metastasis, lymph node metastasis, and Ki-67 > 2% (all p < 0.05). CONCLUSIONS:Of the 4 markers studied, only Ki-67 was the independent prognostic factor. Expression of MMP-9 and DJ-1 correlated with invasion, metastasis, disease progression, and poor prognosis in PNETs, while A1BG had no prognostic value in PNETs. It is anticipated that these findings can serve as useful clinical survival predictors, especially in the setting of resected disease.
Prognostic Enigma of Pancreatic Solid Pseudopapillary Neoplasm: A Single-Center Experience of 63 Patients

Abstract:Background: Studies investigating prognostic factors of solid pseudopapillary neoplasm (SPN) have been published with conflicting findings. Methods: Retrospective analysis of 63 consecutive cases of SPN in our institution from January 2010 to December 2019 was carried out. The clinicopathological features, treatment practices along with survival associations were collected and analyzed. Results: Fifteen patients (23.8%) were male, and 48 (76.2%) were female, with a median age of 34.0 ± 14.5 years. The larger tumor size was correlated with the more mixed components (p = 0.000) and the higher Ki-67 index (p = 0.042). No recurrence was found in the nine patients whose tumors fulfilled the WHO criteria for malignancy due to the presence of at least perineural invasion (6.4%), angiovascular invasion (2.3%), and/or adjacent organ invasion (6.4%). Microscopic infiltrative growth was detected in 9 (14.3%) tumors, which was correlated significantly with the WHO criteria (p = 0.002), capsule invasion (p = 0.005), and pancreatic parenchyma invasion (p = 0.001), but not with disease-free survival (p = 0.13). CD99 was found to be positively expressed in 88.9% (40/45) of tumors and more likely to have depressed Ki-67 index (p = 0.016). After a median follow-up of 58 months, only two patients (3.2%) had a recurrence after their first operation outside of our hospital. No patient died due to tumor progression. Conclusions: Although survival is favorable with aggressive surgery, it is actually difficult to assess the prognostic factors of resected SPNs. Future investigations into the role of clinicopathological evaluation will unveil the prognostic enigma of pancreatic SPN after resection.
Solid Pseudopapillary Tumors of the Pancreas: Diagnosis and Treatment

Abstract:Objective To summarize the experience in the diagnosis and treatment of solid pseudopapillary tumors of the pancreas. Methods Ten consecutive patients who underwent surgery with pathologically confirmed solid pseudopapillary tumors of the pancreas between October 2005 and December 2008 were retrospectively reviewed. Results All of the 10 patients were female and the median age at diagnosis was 24 years (range, 11 -39 years). Abdominal discomfort or pain were the most common presenting symptoms. 4 patients had palpable abdominal mass at physical examination. The tumors appeared on ultrasonography and/or CT, MRI as solid or cystic masses. The preoperative serum biochemical parameters and tumor markers level were within the normal range. All the patients underwent surgical treatment. The tumors were located in the head/neck (n = 6) or the distal part (n = 4) of the pancreas. The surgical procedures included enucleation (n=3) , distal pancreatectomy (n=3 , two with preservation of the spleen, one combined with splenectomy, distal gastrectomy and partial colectomy) , segmental pancreatectomy with pancreaticojejunostomy (n=3) and pancreaticoduodenectomy (n = 1). Pancreatic fistula (n = 2) was observed postoperatively and resolved with conservative treatment. The median resected tumor size was 5. 9 cm. All patients were alive and remained recurrence and metastasis free after a median followk-up of 19. 2 months (range, 8～42 months). Conclusions Solid pseudopapillary tumor of the pancreas was rare neoplasm occurred predominantly in young women with low malignant potential. Aggressive resection should be attempted and could result in excellent prognosis.
Solid Pseudopapillary Tumor of the Pancreas: a Case Series of 26 Consecutive Patients

Abstract:OBJECTIVE: Solid pseudopapillary tumor (SPT) of the pancreas, which predominantly affects young women, is a relatively indolent entity with favorable prognosis. The aim of this study is to describe the clinicopathologic features and surgical management of this disease in our institution.METHODS: A retrospective study of clinical data from 26 consecutive patients with SPT managed in a tertiary academic center between January 2002 and December 2007 was performed. Clinicopathologic factors were compared between benign and malignant cases to determine what features of the tumor could suggest malignant potential.RESULTS: The 26 cases included 22 female and 4 male patients, and the average age was 32.3 years (range 15 to 64). Clinical symptoms were nonspecific and included upper abdominal pain or discomfort, abdominal distention, and back pain. The neoplasm was localized in the pancreatic head/neck in 14 patients and in the body/tail in 12 patients. The median diameter of these lesions was 6.25 cm (range 2 to 15). All of the tumors-including 8 pancreaticoduodenectomies, 10 distal pancreatectomies, 6 local resections, 1 total pancreatectomy, and 1 central pancreatectomy-were resected successfully. No patient received chemotherapy or radiotherapy after surgery. All of the patients except 1 were alive at a median follow-up of 32.5 months (range 3 to 69). One of the 2 patients with malignant SPT, in whom Ki-67 immunoreactivity was >25%, developed local recurrence with liver metastasis 4 months and died 6 months after surgery. There were no significant associations between clinicopathologic factors and malignancy.CONCLUSIONS: SPT is a rare neoplasm with low malignant potential. Characteristic computed axial tomography and magnetic resonance imaging scans combined with age and sex profile should be sufficient for the decision to operate. Patients with malignant SPT should have careful follow-up. The high proliferative index assessed by immunohistochemical staining for Ki-67 may predict poor outcome of malignant SPT. [Study on Clinicopathologic Features and Metastasizing Potential of Solid Pseudopapillary Tumor of Pancreas].

Abstract:OBJECTIVE:To study the clinicopathologic features and metastasizing potential of solid pseudopapillary tumor of the pancreas (SPT).METHODS:Forty-five cases of SPT were retrieved from the archival file and subdivided into metastasizing group (MG, n = 4), and non-metastasizing group (NMG, n = 41), according to the follow-up clinical information. The histological features were reviewed and immunohistochemical study for vimentin, alpha 1-antitrypsin, alpha 1-antichymotrypsin, CD10, neuron-specific enolase, progesterone receptor, chromogranin A, synaptophysin, AE1/AE3, beta-catenin, p53, cyclin D1, CD34 and Ki-67 was carried out. The results were correlated with follow-up data.RESULTS:There was no statistically significant difference between MG and NMG, in terms of age and gender of the patients, site, size and capsular status of the tumor. No single morphologic parameter could distinguish MG from NMG. In general, increased mitotic activity, tumor emboli in vessels and necrotic foci were more conspicuous in MG than in NMG. According to a morphologic scoring system, all cases of MG had score ≥ 5, in contrast to < 5 in 95.1% (39 cases) of NMG. Immunohistochemically, there was no statistically significant difference between MG and NMG for beta-catenin, p53, cyclin D1 and CD34 staining. Ki-67 positivity however was significantly higher in MG. Amongst the 37 cases with follow-up information available, the average duration of follow up was 37.4 months. Thirty-three patients were alive and disease-free.Four suffered from liver metastases; and none of them died of the tumor.CONCLUSIONS:Mitotic activity, presence of tumor emboli and necrotic foci represent as the useful parameters in predicting metastasizing potential of SPT, especially with application of morphologic scoring system. Immunostaining for Ki-67 can also serve as an additional prognostic indicator.

Abstract:Since solid pseudopapillary tumor of the pancreas (SPTP) was officially classified by the World Health Organization in 1996, SPTP has recently received special attention in the literature. Studies have shown that SPTP is a heterogeneous tumor, with a small percentage of patients harboring aggressive behaviors. However, criteria for malignancy grade in SPTP have not been well established. The prognosis of SPTP is generally good, with cases having a chance for long-term survival even with recurrence and/or metastasis after surgical resection. The current American Joint Committee on Cancer/Union for International Cancer Control tumor, node, metastasis staging system is not specific to SPTP. The lack of a predictive staging classification that accurately describes the heterogeneity of this disease hinders meaningful research into optimal individualized therapy. Here we summarize and discuss the associated factors proposed for appraisal of the malignant potential and adverse outcome of SPTP.
Prognostic significance of grade of malignancy based on histopathological differentiation and Ki-67 in pancreatic ductal adenocarcinoma

Abstract:Objective: Tumor cell malignancy is indicated by histopathological differentiation and cell proliferation. Ki-67, an indicator of cellular proliferation, has been used for tumor grading and classification in breast cancer and neuroendocrine tumors. However, its prognostic significance in pancreatic ductal adenocarcinoma (PDAC) remains uncertain. Methods: Patients who underwent radical pancreatectomy for PDAC were retrospectively enrolled, and relevant prognostic factors were examined. Grade of malignancy (GOM), a novel index based on histopathological differentiation and Ki-67, is proposed, and its clinical significance was evaluated. Results: The optimal threshold for Ki-67 was determined to be 30%. Patients with a Ki-67 expression level > 30% rather than ≤ 30% had significantly shorter 5-year overall survival (OS) and recurrence-free survival (RFS). In multivariate analysis, both histopathological differentiation and Ki-67 were identified as independent prognostic factors for OS and RFS. The GOM was used to independently stratify OS and RFS into 3 tiers, regardless of TNM stage and other established prognostic factors. The tumor-node-metastasis-GOM stage was used to stratify survival into 5 distinct tiers, and surpassed the predictive performance of TNM stage for OS and RFS. Conclusions: Ki-67 is a valuable prognostic indicator for PDAC. Inclusion of the GOM in the TNM staging system may potentially enhance prognostic accuracy for PDAC.

Abstract:Objective To summarize the clinicopathologic features and treatment outcomes of solid pseudopapillary tumors(SPTs) of the pancreas.Methods Twenty-seven cases of SPT of the pancreas admitted for treatment to the Peking University Cancer Hospital between September 2008 and September 2012 were retrospectively analyzed.Results The majority of the pancreatic SPT patients were young adults(median age: 29 years old) and females(85.2%).All 27 patients were treated with surgical resection using pancreaticoduodenectomy(n=4),duodenum preserving pancreatic tumor resection(n=6),middle pancreatectomy(n=5),distal pancreatectomy(n=5),or distal pancreatectomy plus splenectomy(n=7).The minimum tumor diameter was 1.5 cm,the maximum diameter was 12.0 cm,and the average diameter was 5.4 cm.Twelve patients developed pancreatic leakage and pyrexia following the operation.One patient suffered splenic artery hemorrhage.All 27 patients survived and completed follow-up.Only one patient developed recurrence,which was treated by a second surgical resection,and all other patients showed no clinical signs of recurrence or metastasis.Conclusion SPT of the pancreas has uncertain malignant potential with good prognosis.Radical resection with preservation of the surrounding tissues is an effective and safe treatment for SPT.

Abstract:Abstract Background Due to its rarity, duodenal papillary carcinoma (DPC) is seldom studied as a unique disease and no specific molecular features or treatment guidelines are provided. Methods Whole‐exome sequencing was performed to gain new insights into the DPC mutation landscape and to identify potential signalling pathways and therapeutic targets. Mechanistically, immunohistochemistry (IHC), immunofluorescence, RNA‐seq, ATAC‐seq and in vitro cell function experiments were performed to confirm the underlying mechanisms. Results We described the mutational landscape of DPC for the first time as a group of rare tumours with a high frequency of dysregulation in the chromatin remodelling pathway, particularly PBRM1‐inactivating mutations that are significantly higher than duodenal adenocarcinomas and ampullary adenocarcinoma (27% vs. 0% vs. 7%, p < .01). In vitro cell experiments showed that downregulation of PBRM1 expression could significantly promote the cancer progression and epithelial‐to‐mesenchymal transition via the PBRM1‐c‐JUN‐VIM axis. The IHC data indicated that PBRM1 deficiency (p = .047) and c‐JUN expression (p < .001) were significantly associated with poor prognosis. Meanwhile, the downregulation of PBRM1 expression in HUTU‐80 cells was sensitive to radiation, which may be due to the suppression of c‐JUN by irradiation. Conclusions Our findings define a novel molecular subgroup of PBRM1‐inactivating mutations in DPC. PBRM1 play an important role in DPC progression and may serve as a potential therapeutic target and prognostic indicator.
Prognostic Value of Programmed Cell Death Protein 1 Expression on CD8+ T Lymphocytes in Pancreatic Cancer

Abstract:Pancreatic cancer is one of the most aggressive malignancies and has a highly immunosuppressive tumour microenvironment. Immune checkpoint blockade has led to remarkable and durable objective responses in a number of malignancies and antibody-based strategies targeting programmed cell death protein 1 (PD-1) are showing promise where traditional modalities of surgery, radiotherapy, and chemotherapy have failed. In this study, we examined the clinical value of PD-1 protein expression by CD8+ peripheral T lymphocytes or tumour-infiltrating T lymphocytes (TILs) in pancreatic ductal adenocarcinoma (PDAC). Expression of PD-1 protein on CD8+ TILs correlated with overall survival and clinicopathological characteristics such as clinical stage, N classification, and M classification. Similar findings were observed for the expression of PD-1 protein on peripheral CD8+ T cells, whereas its expression on peripheral CD4+ T cells showed no significance. Comparison of the levels of PD-1 protein expressed by peripheral CD8+ T cells before and 4 weeks after surgery indicated that preoperative and postoperative status of peripheral PD-1 expression was unchanged. Our findings showed that PD-1 protein expressed by peripheral or tumour-infiltrated CD8+ T cells was a promising biomarker for diagnosis and prognosis in PDAC and might help guide future immunotherapies.
The prognostic and predictive potential of Ki-67 in triple-negative breast cancer

Abstract:Abstract As a cell proliferation biomarker, Ki-67 is principally used in ER+/HER2− breast cancer. However, the importance and the best cutoff point of Ki-67 in triple-negative breast cancer (TNBC) remains unclear and was evaluated in this study.A total of 1800 patients with early invasive TNBC between 2011 and 2016 at Fudan University Shanghai Cancer Center were consecutively recruited for this study. The optimal cutoff for Ki-67 was assessed by Cutoff Finder. Propensity score matching (PSM, ratio = 1:2) was performed to match the Ki-67 low group with the Ki-67 high group. Overall survival (OS) and disease-free survival (DFS) were compared between the two groups using the Kaplan-Meier method and Cox regression model. The most relevant cutoff value for Ki-67 for prognosis was 30% ( p = 0.008). At the cutoff point of 30%, worse DFS and OS were observed in the Ki-67 high group. In multivariate analyses, N-stage ( p 30% was significantly associated with worse prognosis, especially for stage I patients. For stage I/Ki-67 low TNBC patients, the advantage of CT is unclear, providing the basis for future de-escalation therapy.

Abstract:OBJECTIVE:To summarize the experience of diagnosis and treatment in solid-pseudopapillary tumor of pancreas (SPT).METHOD:Retrospectively analyzed 12 cases of SPT diagnosed between 2000 August and 2004 February in Peking Union Medical College Hospital.RESULTS:The average age of these 12 cases was 29.1, including 11 women and 1 men. The tumors of 6 cases were located in the pancreatic head, one tumor was in the neck of pancreas and 5 tumors were in the body and tail of pancreas. There were no characteristic clinical manifestations. Solid and solid-cystic masses of low echo were found in pancreas in ultrasonic examinations. CT scan found masses of low density in pancreas, while irregular enhancement appeared in the circumference of all tumors in enhanced CT scan sequences. Tumor markers in patients' serum were all negative. Pancreaticoduodenectomy were performed in 2 cases. Resection of tumors and pancreaticojejunostomy were performed in 3 cases. Simple resection of tumor was performed in 1 case. Segmental pancreatectomy and pancreaticojejunostomy was performed in one patient. Five patients underwent distal pancreatectomy and spleen resection. Ten patients were followed up with the average time of 14.3 months. No evidence of relapses and metastasis in these cases was found.CONCLUSION:Solid-pseudopapillary tumor of pancreas was one of rare pancreatic neoplasm with low malignant potential, affecting primarily young women. Complete resection resulted in long-term survival.

Abstract:Radical cystectomy (RC) is the gold standard treatment for patients with high-risk non-muscle-invasive bladder cancer and muscle-invasive bladder cancer (MIBC). However, despite aggressive treatment, mortality rates of patients are still high. Therefore, it is crucial to indentify some practical prognostic factors for cancer-specific survival in those patients to guide our therapeutic strategies.

Abstract:OBJECTIVE:To study the clinicopathologic and immunohistochemical features, biological behavior, diagnosis and treatment of solid pseudopapillary tumor of the pancreas (SPTP).METHODS:A retrospective clinical and clinicopathologic analysis was made on 33 cases of SPTP admitted from May 2001 to 2010 July. There were 7 male and 26 female patients, aging from 13 to 66 years with a mean of 34.3 years.RESULTS:The tumor was located in pancreatic head of 10 patients, in pancreatic neck of 5 patients, in pancreatic body and tail of 18 patients. Of the 33 patients treated with surgery, 8 underwent simple resection of pancreatic tumor, 6 underwent pancreaticoduodenectomy, 3 underwent tumor resection plus pancreaticojejunostomy, 1 underwent tumor resection plus pancreaticogastrostomy, 11 underwent distal pancreatectomy, 4 underwent distal pancreatectomy plus spleen resection (1 underwent mesohepatectomy for hepatic metastasis). Sixteen of the 33 operations were completed by laparoscopy. Histologically, tumors were composed of papillary and microcystic solid structures, with uniformed population of cells. The pancreas and blood vessels invasion were identified in 3 cases, one of them was combined with liver metastasis, and they are male. Immunohistologically, the tumors were positive for α1-antitrypsin, α1-antichymotrypsin, β-catenin, CD10, CD56 and vimentin (all cases), neuron-specific enolase (3 cases), synaptophysin (6 cases), chromogranin A (4 cases), progesterone receptor (28 cases), estrogen receptor (3 cases), S-100 (6 cases). Totally 33 cases were followed up with a median period of 49 months without tumor recurrence.CONCLUSIONS:SPTP is of low graded malignancy. It primarily affects young women. It may be located in any part of pancreas. Immunohistochemistry is very important for the diagnosis and differential diagnosis of SPTP. Surgical resection is recommended as the treatment of choice. Laparoscopic distal pancreatectomy or tumor resection is feasible and safe for some selected patients, and the prognosis is good.

Abstract:OBJECTIVE:To study the surgical management of solid-pseudopapillary tumor of the pancreas (SPTP) and its characteristics of outcome.METHODS:Fifty-eight patients with SPTP of the pancreas admitted from January 2001 to December 2010 were retrospectively analyzed. There were 7 male and 51 female patients, with an average age of 30 years (ranging 9 to 70 years). Most patients were symptomatic before admission; the most common symptom was abdominal pain. Of the 58 patients, 21 patients underwent pancreaticoduodenectomy, 30 patients underwent distal pancreatectomy, 6 patients underwent central pancreatectomy, 1 patient underwent simple tumor enucleation, and 1 patients underwent duodenum-preserving pancreatic head resection.RESULTS:The average length of stay in hospital was 23.8 days (ranging 12 to 64 days). Thirteen patients (22.4%) developed postoperative complications, including grade A postoperative pancreatic fistula of 8 cases, gastrointestinal tract bleeding of 1 case, pleural effusion of 2 cases, wound infection and fat liquefaction of 2 cases. Two patients underwent reoperation due to gastrointestinal tract bleeding or wound infection. There was no hospital death. Forty-four patients were followed-up for 7 to 136 months with an average of 41 months. All the 44 patients were alive, while 8 patients developed dyspepsia and 4 patients developed diabetes mellitus. There were no tumor recurrences or metastasis.CONCLUSIONS:SPTP is found primarily in young women. Excellent prognosis would be achieved with surgical resection.
