Abstract:•We reviewed the neuroradiological findings of 5 cases of intraspinal subependymoma.•Cervical and cervicothoracic intraspinal subependymomas typically present as heterogenous and eccentric mass with well-demarcated margin.•Faint or non enhancement of the tumor combined with a slowly deteriorated clinical course is indicative of a subependymoma.•Cap sign and syringohydromyelia can also be present but less common than ependymoma.•Ependymomas typically located centrally within the cord and display symmetric expansion with well-defined enhanced margins. Syringohydromyelia and cap sign are characteristic signs. Astrocytomas, in comparison, tend to be eccentrically positioned and may display an exophytic component with heterogeneous enhancement.
