Abstract:In recent years, the life expectancy in developing countries has been on the rise, contributing to a significant growth in the older population. [ 1 ] According to a report from the National Bureau of Statistics of China, as of the end of 2021, China's elderly population, aged 60 and above, is expected to surpass 267 million, making it the largest in the world. Projections suggest that by approximately 2035, this demographic will exceed 400 million, constituting more than 30% of the total population and marking a significant transition into a stage of pronounced aging. [ 2 ] Trauma stands as a prominent contributor to global mortality, responsible for approximately 4.4 million deaths annually. This accounts for roughly 8 percent of all recorded causes of death worldwide. [ 3 ] Injuries and violence exact a daily toll, claiming the lives of approximately 12,000 individuals worldwide. [ 4 ] Past research has commonly indicated that trauma is primarily a disease affecting the young population. [ 5 , 6 ] Nevertheless, as the global population ages, there is a shift in the patterns of trauma occurrence, notably marked by a rising incidence of geriatric trauma. [ 7 ] In 2005, traumatic incidents among individuals aged 60 and over comprised about 20% of all major traumatic events reported in England and Wales. However, by 2014, this proportion had surged to surpass 50%. This significant increase indicates a notable shift towards a higher incidence of traumatic events affecting the older population over this time span. [ 5 ] Trauma is now one of the top 5 causes of death among older adults. [ 8 , 9 ] Elderly patients with trauma usually have more complications. [ 10 ] The occurrence, development, treatment and outcome of trauma in the elderly are different from those in other age groups due to the irreversible aging of physical functions and the fact that many elderly people suffer from a variety of underlying diseases. [ 11 ] In contemporary times, trauma has emerged as one of the primary contributors to mortality among older adults, securing its place among the top 5 causes of death in this demographic. [ 12 ] This paradigm shift underscores the critical need for targeted healthcare approaches and interventions specifically tailored to address the unique challenges posed by traumatic events in the aging population. It is crucial to recognize that the prevalence and impact of trauma can vary across different populations. In contrast to other developing nations, China has made significant strides in delving into the intricacies of geriatric trauma through in-depth research. However, despite these advancements, there remains a recognized insufficiency in the breadth and depth of studies. While the research landscape on geriatric trauma in China is comparatively more extensive, there is an acknowledged need for further exploration and expansion to comprehensively address the unique challenges and dynamics within this demographic. Continued efforts to enhance research initiatives in geriatric trauma are crucial for advancing both understanding and the development of targeted interventions in China. China's focus on trauma medicine underwent a significant shift only recently. In December 2019, the National Center for Trauma Medicine was established in Beijing, followed by the establishment of Gansu Province's inaugural internationally standardized trauma center at the First Hospital of Lanzhou University in April 2020. It was not until after June 2021 that Gansu saw the rollout of a robust trauma registry system. Despite the longstanding development of trauma medicine, especially in developed regions worldwide, China is just starting to establish specialized departments dedicated to trauma. Recognizing this gap and aiming to enhance the quality of geriatric trauma treatment, we embarked on a retrospective analysis of geriatric trauma cases within our hospital's trauma database. Our study focused on patient cases recorded between November 1, 2020, and October 31, 2023. By examining these cases, our goal is to contribute essential theoretical references and fill the research gap in the field of geriatric trauma not only in China but also within developing countries.
