Abstract:234 Objectives: Total parathyroidectomy with autotransplantation(tPTX+AT) is a suitable procedure for renal hyperparathyroidism(HPT),which showed significantly often normalization of serum calcium and alkaline phosphatase as well as significant improvement of clinical signs like pruritus and muscle weakness.However,the rate of persistent or recurrent disease after tPTX+AT is high, and it is usually related to hyperplasia of the autografted tissue. 99mTc-MIBI scintigraphy is a reliable non-invasive exploratory tool in the detection of recurrent parathyroid disease in the neck or mediastinum.But little is known about its role in detecting the recurrence of autografted tissue. In this study,we aim to investigate the value of 99mTc-MIBI multi-temporal SPECT and SPECT/CT in detecting the localization of recurrence after tPTX+AT. Methods: The study group comprised 18 consecutive adult patients(4 males,14 females,age range: 33-66years) from December 2011 to July 2017 who have received tPTX+AT. All patients with suspected recurrence due to clinical or biochemical findings(eg.osteodynia,serum elevated level of parathormone) underwent 99mTc-MIBI multi-temporal SPECT and SPECT/CT of the neck, mediastinum and graft area. Multi-temporal SPECT images for these areas were acquired at early phase(15min)and delay phase(60min, 90min and 120min)after injection.SPECT/CT images were obtained at early phase using a 16-slice diagnostic CT system.Imaging results were confirmed by biopsy or further clinical and radiologic work-up. The detection rates for multi-temporal SPECT and SPECT/CT to identify hyperparathyroidism lesions were calculated. The tracer uptake by the lesions in the autograft area was assessed semi-quantitatively employing region of interest ratios to normal adjacent areas. Spearman’s rank correlation analysis was used to determine the association of the tracer uptake with serum levels of intact parathormone(iPTH) ,calcium and phosphorus. Results:99mTc-MIBI multi-temporal SPECT had detected 21 lesions including 16 in the graft site and 5 in the neck.Meanwhile, SPECT/CT had shown 24 lesions including 18 in the graft site and 6 in the neck.Two lesions in the autograft site were both missed by SPECT and SPECT/CT.One false-positive lesion proved by pathology was foreign body granuloma in the graft area,and pathological results were obtained in other 20 lesions,including 4 adenomas and 16 hyperplasias.The detection rates of hyperparathyroidism lesions by multi-temporal SPECT and SPECT/CT were 77.8%(21/27) and 88.9%(24/27),respectively,without statistical significance (P>0.05).In multi-phase SPECT,all 16 lesions in the graft sites showed the highest uptake of 99mTc-MIBI in 15min scan, and gradually reduced in the delayed scan. Five lesions located in the neck were detected in delayed imaging.The Spearman’s correlation analysis showed that the the tracer uptake had no significant correlation with levels of iPTH,calcium and phosphorus (P>0.05). Conclusion: In this retrospective study, 99mTc-MIBI multi-temporal SPECT and SPECT/CT images may provide helps for localization of the hyperparathyroidism glands accurately.It results that it is not necessary to perform delay imaging for autograft area.Future prospective studies in larger patient groups are desirable for confirming our findings.
