Abstract:Objective: Unicompartmental knee arthroplasty (UKA) revision is usually due to the degenerative degree of knee articular osteochondral tissue in the untreated compartment. However, it is difficult to simulate the biomechanical behavior on this tissue accurately. This study presents and validates a reliable system to predict which osteoarthritis (OA) patients may suffer revision as a result of biomechanical reasons after having UKA.Design: We collected all revision cases available (n = 11) and randomly selected 67 UKA cases to keep the revision prevalence of almost 14%. All these 78 cases have been followed at least 2 years. An elastic model is designed to characterize the biomechanical behavior of the articular osteochondral tissue for each patient. After calculated the force on the tissue, finite element method (FEM) is applied to calculating the strain of each tissue node. Kernel Ridge Regression (KRR) method is used to model the relationship between the strain information and the risk of revision. Therefore, the risk of UKA revision can be predicted by this integrated model.Results: Leave-one-out (LOO) cross-validation (CV) is implemented to assess the prediction accuracy. As a result, the mean prediction accuracy is 93.58% for all these cases, demonstrating the high value of this model as a decision-making assistant for surgical plaining of knee OA.Conclusions: The results of this study demonstrated that this integrated model can predict the risk of UKA revision with theoretically high accuracy. It combines bio-mechanical and statistical learning approach to create a surgical planning tool which may support clinical decision in the future. Minimally Invasive Unicompartmental Knee Arthroplasty for Spontaneous Osteonecrosis of the Knee Joint

Abstract:Objective To explore the outcome and surgical technique of minimally invasive unicompartmental knee arthroplasty (UKA) for spontaneous osteonecrosis of the knee joint.Methods From January 2009 to June 2013,twenty-seven cases with medial compartmental spontaneous osteonecrosis treated by minimally invasive Oxford phase 3 UKA were reviewed retrospectively.Twelve knees were male and 15 were female,with an average age of 64.6±8.6 years (52-82 years).At the time of diagnosis,11 patients presented with grade Ⅲ necrosis,and 16 grade Ⅳ,following Mont's classification.The pain,range of motion (ROM) and HSS score of the knees were evaluated before and after UKA.Pre-and post-operative alignment of the low limbs were measured and compared respectively.According to the guidelines proposed by the Oxford group,postoperative radiographic assessments were made at the final follow-up.Results All of the patients were followed up for a mean time of 27.8± 15.9 months (6-59months).There were no serious adverse events such as infection,bearing dislocation,aseptic loosening,pulmonary embolism,deep venous thrombosis,cardio-cerebral vascular incident or psychogenia.There was 1 revision from unrelated causes (fracture of tibia plateau) at 3 years after arthroplasty.One femoral component was tilted with postoperative radiographic angle ＞ 10°.One radiolucent line was observed in the SONK series.There were no clinical symptoms of the two implant failures at the last follow up.VAS score was reduced from 6.9±0.9 to 2.0±1.1 (t=19.27,P=0.00).96.3％ (26/27) of pain was relieved.The mean post-operative ROM and femorotibial angle were 125.7°±9.6°,177.7°±3.1°,respectively.HSS score was increased from 61.3±9.7 to 93.0±4.8 (t=14.46,P=0.00).Of the patients,96.3％ (26/27) were satisfied with the outcome of this surgical procedure.Conclusion Minimally invasive UKA is an effective method for spontaneous osteonecrosis of the knee with fewer traumas and fast recovery.The early term outcome of UKA is encouraging.
Unicompartmental Knee Arthroplasty Versus Opening-Wedge High Tibial Osteotomy for Spontaneous Osteonecrosis of the Knee: A Retrospective Cohort Study

Background:Unicompartmental knee arthroplasty (UKA) and high tibial osteotomy (HTO) are treatment options for patients with medial spontaneous osteonecrosis of the knee (SONK).Purpose:To compare the clinical outcomes after UKA and HTO in patients with SONK.Study Design:Cohort study; Level of evidence, 3.Methods:This retrospective study included 42 patients who had undergone Oxford UKA and 40 patients who had undergone opening-wedge HTO between 2014 and 2020. All patients were diagnosed with isolated medial SONK without subchondral collapse of the femoral condyle and tibial plateau. The patients were preoperatively and postoperatively evaluated using the Lysholm knee scoring system, the Western Ontario and McMaster Universities Osteoarthritis Index, and a numeric rating scale assessing patient satisfaction.Results:Patients in the UKA group were significantly older than those in the HTO group (median age, 71.5 years [IQR, 68.0-76.5 years] vs 65.0 years [IQR, 60.0-70.0 years], respectively; P < .001). The median follow-up time was 3.78 years (IQR, 2.45-4.53 years) for the UKA group and 3.87 years (IQR, 2.90-5.60 years) for the HTO group. Significant improvements in functional scores were observed in both the UKA and HTO groups (P < .001 for all), with no significant between-group differences in scores at the final follow-up (≥2 years after surgery). The satisfaction rate was similar (80.95% for UKA and 75.0% for HTO).Conclusion:According to the study results, significant improvements in clinical outcomes were seen after opening-wedge HTO with microfracture for a younger group of patients with SONK without subchondral collapse, while Oxford UKA had a comparable effect on an older group of patients. Both UKA and HTO were found to be viable surgical approaches for SONK at short- to midterm follow-up.

Abstract:Unicompartmental knee arthroplasty (UKA), an alternative technology that has been prevailed upon Orthope?dics which was proven by time that it's not only an effective treatment for patients with anterior medial knee osteoarthritis, addi?tionally, it plays an important role in the staged treatment of knee osteoarthritis. The advantage of UKA is that patients would re?ceive minimal invasion and faster recovery and it is more suitable for elderly patients. Nevertheless, UKA was also an advisable choice of young patients due to its maximum retention of the knee joint sensation, with a reflect on higher post operation score on FJS (forgetting joint score). The ideal clinical efficacy of unicompartmental knee arthroplasty is relied on the improvements on the latest artificial joint design and surgical skill approaches, and as a consequence, indications and contraindications for UKA have renovated. A subset of patients who were previously thought to be contraindicated to perform UKA (such as medial patellofemoral joint mild arthritis) can be reassessed and performed UKA today which called relative contraindications or Non?essential contrain?dications. The main objective of this article is to provide a new concept of UKA to surgeons, and to provide the best treatment op?tions for patients with different complications according to the latest preoperative evaluation criteria. According to our experience, it is recommended that surgeons should accumulate a certain amount of total joint replacement experience and then to carry out UKA surgery, which will achieve better results.

Abstract:The osteonecrosis of femoral head (ONFH) has been a common disorder in clinical orthopaedic practice. ONFH is the relative contraindication of hip arthroplasty for the young and active patients since they suffer a high rate of revision and inferior function postoperatively. The primary importance in therapy is to prevent the progression of the disease and the collapse of the articular surface in order to keep away from hip arthroplasty. Low-intensity pulsed ultrasound (LIPUS) has been indicated to enhance the osteogenic differentiation of mesenchymal stem cells, stimulate the differentiation and the proliferation of osteoblasts, inhibit osteoclasts, improve the local blood perfusion and the angiogenesis, and accelerate the healing process of the stress fracture. We present a hypothesis that LIPUS may be an appropriate non-invasive modality for bone repair in the early stage of ONFH so that the progressive collapse of the articular surface and the subsequent arthroplasty would be avoided.

Abstract:Unicompartmental knee arthroplasty (UKA), which has many potential advantages compared with total knee arthroplasty, was widely used across the world in recent years. The introduction of the robot systems greatly makes up for the defects of the conventional UKA surgery such as higher complication rates and revision rates. MAKO system, a new image‐guided robot system relies on a preoperative computed tomography scan to assist in preoperative mapping and planning, offers an opportunity to improve the outcome of UKA surgeries. In order to have a more comprehensive and in‐depth understanding of MAKO‐assisted UKA, the studies on MAKO‐assisted UKA were summarized. MAKO‐assisted UKA is better than conventional UKA surgery on implant accuracy, soft tissue balance, patient function scores and satisfaction, complications rates, and learning curve in short‐term outcome; however, the mid‐term and long‐term outcomes of MAKO‐assisted UKA need to be further studied.

Abstract:Objective: To conclude the research progress of unicompartmental knee arthroplasty (UKA) in the treatment of elderly patients with knee osteoarthritis (KOA). Methods: The literature on UKA at home and abroad in recent years was reviewed to summarize the clinical characteristics of elderly patients with KOA, perioperative management (including evaluating indications preoperatively, intraoperative prosthesis selection, postoperative complication management, etc). Results: Through reasonable preoperative evaluation, prosthesis selection, and advanced perioperative management, for elderly patients with KOA who meet the indications, UKA can be considered. Compared with total knee arthroplasty, the incidence of postoperative complications in elderly patients undergoing UKA is lower, joint awareness is reduced, functional improvement and satisfaction are higher. Meanwhile, choosing appropriate prostheses and fixation methods can lead to a good survival rate. Conclusion: UKA can provide a safe and effective treatment option for elderly patients with KOA within a certain range of indications.
[Short-term effectiveness of unicompartmental knee arthroplasty for medial compartmental knee osteoarthritis]

Abstract:Objective: To evaluate the short-term effectiveness of unicompartmental knee arthroplasty (UKA) for medial compartmental osteoarthritis of the knee. Methods: Between January 2008 and October 2013, 18 patients with medial compartmental osteoarthritis of the knee were treated by UKA, including 11 males and 7 females with an average age of 67.2 years (range, 60-72 years). The disease duration ranged from 3 to 5 years (mean, 3.7 years). All patients had loading pain and walk-associated pain of the medial compartmental knee. The preoperative visual analogue scale (VAS) score was 6.8 ± 1.4. The full-length radiograph of lower limb and anteroposterior and lateral radiographs of the knee showed medial compartmental osteoarthritis of the knee. According to Ahlback staging, 8 knees were at stage II and 10 knees at stage III. The knee range of motion (ROM) was (112.3 ± 11.3)°, and the angle of genu varus was (13.2 ± 1.3)°. The American Hospital for Special Surgery (HSS) score was 59.0 ± 6.4. Results: Primary healing of incision was obtained in all patients, and no infection or lower limb deep venous thrombosis occurred. All of the patients were followed up 6-50 months (mean, 28 months). No prosthetic loosening and dislocation or lesions of contralateral compartment and patellofemoral joint developed. At 6 months after operation, the VAS score was significantly reduced to 2.8 ± 1.2 (t = 9.20, P = 0.00); most of patients achieved significant relief of pain. The HSS score was significantly increased to 92.0 ± 3.1 (t = 19.69, P = 0.00); and the results were excellent in 12 cases, good in 5 cases, and poor in 1 case, with an excellent and good rate of 94.4%. The knee ROM was (115.2 ± 10.2)°, showing no significant difference when compared with preoperative one (t = -0.81, P = 0.23). The alignment of limbs showed that the angle of genu varus was significantly reduced to 6.8 ± 2.1)° (t = 10.99, P = 0.00). Conclusion: UKA has satisfactory short-term effectiveness in the treatment of medial compartmental knee osteoarthritis, however, long-term effectiveness need further studies.
The safety and efficacy of unicompartmental knee arthroplasty in outpatient surgical centers: A systematic review and meta-analysis

Abstract:Journal of Orthopaedic Surgery, Volume 32, Issue 1, January-April 2024. BackgroundUnicompartmental knee arthroplasty (UKA) is an effective treatment method for knee osteoarthritis. With the development and implementation of enhanced recovery after surgery, UKA is now increasingly performed in outpatient surgical centers. However, there is ongoing debate regarding the safety and effectiveness of performing UKA in outpatient settings.MethodsThe search was performed to retrieve randomized controlled trials and cohort studies on outpatient UKA from PubMed, Cochrane Library, EMbase, CNKI, and WanFangData databases. The search was conducted from the inception of the databases until August 31, 2023. After independent screening, data extraction, and risk of bias evaluation by two researchers, meta-analysis was performed using RevMan 5.4 software.ResultsA total of eight studies involving 18,411 patients were included. The results showed that the postoperative transfusion rate in the outpatient group was lower than that in the inpatient group [OR = 0.36, 95%CI (0.24, 0.54), p < 0.00001], and the difference was statistically significant. However, there was no significant difference between the two groups in terms of readmission rate, reoperation rate, surgical site infection, and periprosthetic fracture. The differences were not statistically significant.ConclusionCompared to the traditional inpatient route, the blood transfusion rate for single-condyle replacement in the outpatient operation center is lower, and there is no significant difference in readmission rate, reoperation rate, surgical site infection, and periprosthesis fracture. The outpatient approach to UKA is safe, feasible, and highly satisfactory for patients. However, the results have certain limitations, and a rigorous preoperative complication risk assessment can minimize the risk of UKA in outpatient surgery centers.Trial registrationPROSPERO number CRD42023405373.

Abstract:[Objective]To explore the outcome and surgical technique of minimal invasive unicompartmental knee arthroplasty(UKA) for osteoarthritis of the knee. [Method]Ninety-four knees of 87 cases with medial compartmental osteoarthritis of the knee treated by minimal invasive UKA were reviewed retrospectively.The pain,range of motion(ROM) and HSS score of the knees were evaluated before and after UKA.Pre-and post-operative alignment of the low limbs were measured and compared respectively.The surgical skills of UKA were also summarized. [Result]All of the patients were followed up for a mean time of 3.4 years,2 knees were revised to total knee arthroplasty.HSS score was increased from 61.05 to 92.67.VAS score was reduced from 6.46 to 2.80.Pain ralief rate was 94%.The mean post-operative ROM of the knees was 127.53°,and alignment was 2°varus on the average.The rate of excellent and good results was 92%. [Conclusion]Minimal invasive UKA is an effective method for unicompartmental knee osteoarthritis with less trauma,quick recovery.The early term outcome of UKA is encouraging.

Abstract:Improved prosthetic design and application of strict criteria in selection of patients have resulted in wide use of unicompartmental knee arthroplasty (UKA) in surgical treatment of damaged joints. Here, we review the contemporary indications, outcomes, and complications of procedures such as lateral and medial UKA and total knee arthroplasty (TKA), and compare the severity of these complications in UKA and TKA. Patients who received unicompartmental femorotibial osteoarthritis and patients who underwent UKA and ACL reconstruction for deficiency in anterior cruciate ligament (ACL), they all showed good clinical outcomes and survival. Reliable and successful options in the treatment of patients with isolated tibiofemoral osteoarthritis include lateral and medial UKA.
[CLINICAL APPLICATION OF OXFORD MOBILE-BEARING BIPOLAR PROSTHESIS UNICOMPARTMENTAL KNEE ARTHROPLASTY FOR SINGLE COMPARTMENTAL KNEE OSTEOARTHRITIS]

Abstract:Objective: To evaluate the effectiveness of Oxford mobile-bearing bipolar prosthesis unicompartmental knee arthroplasty (UKA) in the treatment of single compartmental knee osteoarthritis. Methods: Between June 2011 and July 2013, 22 cases of single compartmental knee osteoarthritis were treated by Oxford mobile-bearing bipolar prosthesis UKA. Of 22 cases, 8 were male and 14 were female with an average age of 65 years (range, 45-80 years); the left knee was involved in 12 cases, and the right knee in 10 cases, with a mean disease duration of 32.5 months (range, 8-90 months). The mean weight was 55.2 kg (range, 50-65 kg), and the mean body mass index was 20.8 kg/m2 (range, 17-25 kg/m2). Osteoarthritis involved in the single knee medial compartment in all patients. Knee society score (KSS) and range of motion (ROM) were measured to evaluate the knee joint function. Results: Primary healing of incision was obtained in all patients, and there was no complication of infection, bedsore, or deep venous thrombosis. Postoperative follow-up was 2-4 years (mean, 3.2 years). The X-ray films showed good position of prosthesis, no prosthesis dislocation, or periprosthetic infection during follow-up. Knee ROM, KSS function score, and KSS clinical score were significantly improved at 1 week after operation and at last follow-up when compared with preoperative ones (P < 0.05), but no significant difference was shown between at 1 week and at last follow-up (P > 0.05). Conclusion: Oxford mobile-bearing bipolar prosthesis UKA is an effective method to treat single compartmental knee osteoarthritis, with the advantages of less trauma, earlier rehabilitation exercise, near physiological state in joint function, and less risk of complications.
Finite element study of stress distribution in medial UKA under varied lower limb alignment

Abstract:Knee osteoarthritis (KOA) has a high incidence among the elderly, significantly impacting their quality of life and overall health. Medial unicompartmental knee arthroplasty (UKA) is an excellent choice for treating knee single-compartment lesions, and lower limb alignment has a profound impact on medial UKA. To explore the influence of different lower limb alignments on medial UKA. In this study, we selected MR and CT data of healthy adult male knee joints to establish a complete finite element analysis (FEA) model of the knee joint. After validation, we established a finite element model of medial UKA. Subsequently, we created 60 sets of FEA models with different lower limb alignments to analyze the impact of different lower limb alignments on medial UKA. A vertical load of 1000 N was applied to the FEA models with different lower limb alignments. During the process of shifting the Mikulicz line from the midpoint of the knee joint towards the medial side, the lower limb load was primarily concentrated on the medial compartment. The stress values of the lateral meniscus, tibial cartilage, and femoral cartilage gradually decreased. ROI (region of interest) 1 and ROI 2 showed the maximum principal strain changes, while ROI 3 and ROI 4 exhibited less pronounced fluctuations, with the maximum principal strain roughly proportionally increased. During the process of shifting the Mikulicz line towards the lateral side from the midpoint of the knee joint, the stress on the lateral compartment increased observably. ROI 1, ROI 2, ROI 3, and ROI 4 showed decreased maximum principal strains, approximately inversely proportional changes, but the overall reduction was relatively small. Different lower limb alignments have a profound impact on the short- and long-term joint function after UKA. When the Mikulicz line is 10 mm inside the midpoint of the knee joint or slightly outside, there is a relatively lower risk of tibial component fractures, lower stress on the lateral compartment, and lower load on the prosthesis. During medial UKA, measures such as bone resection and prosthesis selection should be taken to ensure that the Mikulicz line is in the ideal position.

Abstract:Objective: To summarize the research progress of Oxford unicompartmental knee arthroplasty (UKA) in treating partial thickness cartilage loss (PTCL) in the medial compartment of the knee joint, aiming to further clarify the indications and optimize the effectiveness of Oxford UKA. Methods: A comprehensive review of recent domestic and international literature on Oxford UKA for PTCL in the medial compartment of the knee joint was conducted to summarize its application and research advancements. Results: Based on current researches, the main indication for Oxford UKA is full thinckness cartilage loss in the medial compartment of the knee joint. Although it has shown certain effectiveness in treating PTCL in the medial compartment of the knee joint, there are also reports of opposite conclusions. Therefore, there is still controversy over whether Oxford UKA can be chosen for PTCL, and the large-sample and multi-center studies are needed to further clarify the controversy. Studies indicate that accurate preoperative assessment of cartilage damage severity is crucial for selecting appropriate candidates for Oxford UKA to optimize postoperative effectiveness. Conclusion: Oxford UKA may represent an effective treatment for patients with PTCL in the medial compartment of the knee joint. However, strict patient selection and precise preoperative evaluation are essential to ensure surgical success and long-term effectiveness.

Abstract:Unicompartmental knee arthroplasties (UKAs) currently represent an important portion of knee arthroplasty procedures and their usage is on an upward trend. Despite offering better functional outcomes and a quicker recovery compared with total knee arthroplasties (TKAs), UKAs are often scrutinized for their longevity. This article provides an in-depth examination of the technical nuances and survival rates of medial versus lateral UKAs, drawing on recent advances and findings in the field. This manuscript thoroughly evaluates the comparability of patient populations undergoing medial and lateral UKAs, considering their anatomical, biomechanical, and demographic differences. It delves into the specific technical challenges associated with each type and systematically assesses the factors that influence failure, including the intricacies of implant design and patient-specific variables. Despite relevant anatomical and biomechanical contrasts between medial and lateral UKAs, the recent literature points to comparable survival rates. The prevalence of early failures within the initial five years after operation underscores the criticality of precise patient selection and refined surgical techniques. The paper succinctly summarizes the pivotal literature and provides essential guidance for optimizing UKA survivorship. It underscores the importance of meticulous patient selection and precise surgical techniques, alongside the identification and mitigation of potential pitfalls that impact outcomes. Finally, robotic technology in UKA has considerably enhanced the precision and reproducibility, representing a viable solution to effectively meet and achieve the recommended technical objectives.
Decision-making in unicompartmental knee arthroplasty using radiological parameters in South Asian populations

Abstract:Background: Many patients who visit orthopedic surgeons mainly complained of knee pain, which is often diagnosed as osteoarthritis affecting the medial compartment, whereas the lateral compartment and patello-femoral joint remain relatively unaffected. AIM: This study aimed to establish criteria for patient selection and validate an evidence-based approach for selecting candidates for unicompartmental knee arthroplasty (UKA). Key considerations in patient selection for UKA include identifying the presence of bone-on-bone osteoarthritis in the medial compartment, ensuring a functionally normal anterior cruciate ligament, maintaining full-thickness cartilage in the lateral compartment, verifying a functionally normal medial collateral ligament, and confirming the absence of severe damage lateral to the patello-femoral joint. MATERIALS AND METHODS: From a consecutive cohort of 390 patients with medial knee pain, preoperative radiographs of bilateral knee including anteroposterior/lateral/Rosenberg/20° valgus stress views were collected, and results were tabulated. Patients were categorized into appropriate groups. The suitability for UKA was determined based on the Oxford radiological decision aid, history, examination, and radiographic assessment including stress radiographs. RESULTS: The Oxford radiological decision aid demonstrated 92% sensitivity and 88% specificity. According to the radiographic assessment, 49% of the knees were considered suitable for Oxford UKA (OUKA), whereas 51% were deemed unsuitable. Among the 51 knees identified as unsuitable for OUKA, 40% did not meet one radiographic criterion, 38% did not meet two criteria, 22% did not meet three criteria, and 1% did not meet four criteria. CONCLUSION: The Oxford radiographic decision aid safely and reliably identifies the appropriate patients for meniscal-bearing UKA and achieves good results in this population. The widespread use of this radiological decision aid should improve the results of UKA.
A Novel Extramedullary Technique to Guide Femoral Bone Preparation in Mobile Unicompartmental Knee Arthroplasty Based on Tibial Cut and Overall Alignment.

Abstract:Abstract Background The mobile Oxford unicompartmental knee arthroplasty (UKA) implant has been widely used with an intramedullary guide for femoral preparation. We modified the femoral guide technique based on the tibial cut first and spacer block technique. This study was performed to determine the radiographic accuracy and early clinical outcomes of the extramedullary method. Methods We retrospectively evaluated 50 consecutive patients who underwent UKA using the extramedullary technique. An equal number of patients who underwent UKA with the conventional technique were matched as the control group. Clinical outcomes were evaluated in terms of the operating time, blood loss, range of motion, and Hospital for Special Surgery score. Radiographic accuracy was evaluated by the implant position and alignment in the coronal and sagittal planes. Results The mean follow-up period was 39.76 ± 5.77 months. There were no differences in the postoperative Hospital for Special Surgery score, range of motion, or hip-knee-ankle angle between the two groups. The operating time in the extramedullary group was shorter than that in the conventional group (54.78 ± 7.95 vs. 59.14 ± 10.91 min, respectively; p = 0.025). The drop in hemoglobin after 3 days was only 12.34 ± 4.98 g/L in the extramedullary group which was less than that in the conventional group (p = 0.001). No significant differences were found in the postoperative coronal and sagittal angles between the two groups. Acceptable radiographic accuracy of the implant alignment and position was achieved in 92% of patients in the extramedullary group and 96% of patients in the conventional group. Conclusions The radiographic and clinical results of the extramedullary technique were comparable with those of the conventional technique with the advantage of no intramedullary interruption, less blood loss, a shorter operating time, and more rapid recovery. As the technique depends on the accurate tibial cut and overall alignment, we do not recommend it to surgeons without high volume experiences.
